Association of family function and suicide risk in teenagers with a history of self-harm behaviors: mediating role of subjective wellbeing and depression.

BACKGROUND: A history of self-harm behaviors is closely associated with subsequent suicide death. Although many factors associated with suicide have been identified, it remains unclear how these factors interact to influence suicide risk, especially among teenagers with a history of self-harm behaviors. METHODS: Data were collected from 913 teenagers with a history of self-harm behaviors through a cross-sectional study. The Family Adaptation, Partnership, Growth, Affection, and Resolve index was used to assess teenagers' family function. The Patient Health Questionnaire-9 and the Generalized

Exploring the acceptability and feasibility of a blended imagery-based digital intervention for self-harm in young people (Imaginator 2.0): a co-produced qualitative study.

BACKGROUND: Around 20% of 12- to 25-year-olds experience self-harm, but interventions deemed both acceptable and effective in reducing self-harm behaviour are rare. Digital interventions can be effective; however, they often lack sustained engagement and co-design. Imaginator 2.0 is a brief, blended intervention with a co-designed app using mental imagery to help young people manage self-harm. This study explores the acceptability and perceived feasibility of Imaginator 2.0 with young people. METHODS: Semi-structured interviews were conducted via Microsoft Teams with a purposive sample of you

Harm reduction services for veterans in supportive housing: A pharmacist-led, interdisciplinary, street medicine approach.

Background: United States Veterans are disproportionately impacted by homelessness. Substance use, mental health conditions, and trauma increase the risk for homelessness and contribute to challenges in maintaining housing. Street Medicine utilizes a trauma-informed approach to provide low-barrier health and social services for those at risk for and experiencing homelessness. Limited evidence exists describing pharmacist roles on Street Medicine teams, including provision of harm reduction services. Objective: To implement a pharmacist-led Harm Reduction Program Street Medicine intervention at

A smartphone-based intervention for young people who self-harm ('PRIMARY'): study protocol for a multicenter randomized controlled trial.

BACKGROUND: Self-harm in young people is a public health concern connected with severe mental health problems, such as personality pathology. Currently, there are no specific evidence-based interventions available for young people who self-harm. Therefore, we developed PRe-Intervention Monitoring of Affect and Relationships in Youth (PRIMARY), a smartphone-based intervention, co-designed by clinicians and young people with lived experience of mental ill-health. PRIMARY combines the Experience Sampling Method (ESM) with weekly report sessions. The study aims to examine the effectiveness of PRIM

Survey of local patterns of fentanyl exposure and harm reduction strategies in the Southeastern United States.

INTRODUCTION: Fentanyl, a synthetic opioid, is frequently found as a contaminant in illicit substances, significantly increasing overdose risk. A growing trend toward intentional fentanyl use further complicates the opioid crisis. This study aimed to identify patterns of local fentanyl use and contamination and evaluate harm-reduction interventions provided by psychiatric teams to patients at high risk of opioid overdose. METHODS: This observational study combined prospective surveys with retrospective chart review. Adults with a positive fentanyl urine drug screen within 6 months of a psychi

Harm reduction and viral hepatitis C in European prisons: a cross-sectional survey of 25 countries.

BACKGROUND: Current estimates suggest that 15% of all prisoners worldwide are chronically infected with the hepatitis C virus (HCV), and this number is even higher in regions with high rates of injecting drug use. Although harm reduction services such as opioid substitution therapy (OST) and needle and syringe programs (NSPs) are effective in preventing the further spread of HCV and HIV, the extent to which these are available in prisons varies significantly across countries. METHODS: The Hep-CORE study surveyed liver patient groups from 25 European countries in 2016 and mid-2017 on national

"Take services to the people": strategies to optimize uptake of PrEP and harm reduction services among people who inject drugs in Uganda.

BACKGROUND: People who inject drugs (PWID) are at increased risk of HIV acquisition and often encounter barriers to accessing healthcare services. Uganda has high HIV prevalence among PWID and lacks integrated pre-exposure prophylaxis (PrEP) and harm reduction services. Understanding PWID experiences accessing and using harm reduction services and PrEP will inform strategies to optimize integration that align with PWID needs and priorities. METHODS: Between May 2021 and March 2023, we conducted semi-structured interviews with PWID in Kampala, Uganda. We recruited participants with and without

Alcohol, drugs and self-harm emergencies in a national unscheduled care system: a retrospective observational study.

BACKGROUND: Alcohol, drugs and self-harm are important public health issues that often coincide with socioeconomic inequalities. While national policy frameworks highlight these as important issues, data describing their prevalence in unscheduled care are lacking. AIM: To conduct the first national-level description of the number, demographic characteristics and routes of care involving alcohol, drugs or self-harm emergencies across all unscheduled care services in Scotland. METHODS: A retrospective cross-sectional study using linked data from all unscheduled care services in Scotland (NHS 2

Reducing the harms of unsafe abortion: a systematic review of the safety, effectiveness and acceptability of harm reduction counselling for pregnant persons seeking induced abortion.

BACKGROUND: Globally, access to safe abortion is limited. We aimed to assess the safety, effectiveness and acceptability of harm reduction counselling for abortion, which we define as the provision of information about safe abortion methods to pregnant persons seeking abortion. METHODS: We searched PubMed, EMBASE, ClinicalTrials.gov, Cochrane, Global Index Medicus and the grey literature up to October 2021. We included studies in which healthcare providers gave pregnant persons information on safe use of abortifacient medications without providing the actual medications. We conducted a descri

Are Facial Gunshot Wounds More Fatal When They Are Self-Inflicted or Other-Inflicted?

STUDY DESIGN: This was a retrospective cohort study of the 2014 Nationwide Emergency Department Sample (NEDS). OBJECTIVE: Intraoral and submental projectile entry points may be less fatal than other facial entry points due to the indirect access to the intracranial structures and the protection offered by the intervening maxillofacial complex. Because intraoral and submental trajectories are almost always present in the setting of attempted suicide, this study sought to determine if intent (self-harm versus other-harm) influenced mortality in facial gunshot wound (GSW) patients. METHODS: All

Prevalence and Correlates of Self-Harm in the German General Population.

The study aimed at evaluating the psychometric properties of the German version of the Self- Harm Inventory (SHI) and examining the lifetime prevalence and correlates of self-harm in a representative German population sample (N = 2,507; age mean = 48.79, SD = 18.11; range 14 to 94 years; 55.5% women) using the SHI. All participants answered the German SHI, the short form of the Barratt Impulsiveness Scale (BIS-15), the ultra-brief Patient Health Questionnaire for Depression and Anxiety (PHQ-4), and provided sociodemographic information. The one-factorial structure of the SHI was replicated usi

Detection of Self-Harm in Electronic Mental Health Records Using Privacy-Preserving Local Language Models: Methodological Study.

BACKGROUND: Self-harm is the strongest risk factor for suicide and an important outcome for mental health care. Although prevalent in clinical populations, it is often imprecisely captured in routinely collected clinical data, where it is often recorded and stored as unstructured free text. Contemporary language models, such as GPT (OpenAI) and Gemini (Google), can analyze free-text clinical notes, but such models may violate data governance of processing sensitive patient data. OBJECTIVE: This study aimed to evaluate whether a privacy-preserving language model running entirely within an inst

Comparing Differences between Two Groups of Adolescents Hospitalized for Self-Harming Behaviors with and without Personality Disorders.

Self-harm (non-suicidal self-injury (NSSI) and suicidal behavior (SB)) is frequent display during adolescence. Patients with personality disorders (PDs) frequently self-harm. However, few studies have focused on the role of PDs in self-harming adolescents. In this study, we collected 79 adolescents hospitalized due to self-harm (88.6% female; 78.5% Caucasian) and divided them into two groups, with or without a diagnosis of PD. The socio-demographic and psychological-clinical data were collected through a structured interview by clinicians. Univariate, subgroup, and multiple logistic regression
