Dental complications of herpes zoster: Two case reports and review of literature.

Herpes zoster (HZ) (shingles) results due to reactivation of varicella-zoster virus. Unusual dental complications like osteonecrosis, exfoliation of teeth, periodontitis, and calcified and devitalized pulps, periapical lesions, and resorption of roots as well as developmental anomalies such as irregular short roots and missing teeth may arise secondary to involvement of 2nd or 3rd division of trigeminal nerve by HZ. Such cases pose both a diagnostic as well as a therapeutic challenge. We report two such rare dental complications of HZ-spontaneous tooth exfoliation and osteonecrosis of the maxi

Automated assessment of periapical health based on the radiographic periapical index using YOLOv8, YOLOv11, and YOLOv12 one-stage object detection algorithms.

This study investigates the application of recent YOLO (You Only Look Once) algorithms for automated detection and classification of apical periodontitis using the Periapical Index (PAI) scoring system (1-5). A dataset of 699 digital periapical radiographs was collected from diverse sources, de-identified, and annotated by calibrated experts before splitting into training, validation, and testing sets. Three deep learning models (YOLOv8m, YOLOv11m, YOLOv12m) were trained and their performance was evaluated using Precision, Recall, F1 score, mean average precision (mAP50), Intersection over uni

Microguided Endodontics: a method to achieve minimally invasive access cavity preparation and root canal location in mandibular incisors using a novel computer-guided technique.

AIM: To present a novel miniaturized and minimally invasive treatment approach for root canal localization in mandibular incisors with pulp … pical radiographs from different projections revealed severe pulp canal calcifications and signs of periapical periodontitis. To facilitate the 'Microguided Endodontics' technique, a CBCT and an intra-oral surface scan were aligned using special software. This allowed the virtual planning of optimal access cavities up to the apical third of the root. In this technique, a …

Success rate of single- versus two-visit root canal treatment of teeth with apical periodontitis: a randomized controlled trial.

INTRODUCTION: The aim of this study was to evaluate the outcome of single- versus 2-visit root canal treatment of teeth with apical periodontitis after a 2-year follow-up period. METHODS: Three hundred maxillary and mandibular nonvital teeth with apical periodontitis were treated in either a single visit or 2 visits. The main inclusion criteria were radiographic evidence of apical periodontitis (minimum size ≥ 2.0 × 2.0 mm) and a diagnosis of pulpal necrosis confirmed by a negative response to hot and cold tests. Radiographically, all teeth showed small and irregular periapical radiolucencies

Idiopathic Bone Cavity: Clinicopathologic Features from a Case Series with Emphasis on Differential Diagnosis of Chronic Apical Periodontitis.

INTRODUCTION: Idiopathic bone cavity (IBC) is an uncommon bone lesion that usually affects youngsters as an unilocular radiolucency with predilection for the posterior mandible. Because the lesion is frequently located in proximity to the adjacent teeth, chronic apical periodontitis is commonly included as a differential diagnosis. The aim of the present study was to analyze the clinical and radiologic features of a series of IBCs diagnosed in a single service. METHODS: All cases diagnosed as IBC were retrieved from the files of an oral pathology laboratory, and the clinical and radiologic ch

Treating Apical Periodontitis With a Nonsurgical Approach.

This article presents a clinical case involving a necrotic molar with apical periodontitis (AP) that was successfully treated with appropriate root canal therapy. As supported by dental literature and clinical experience, in most cases of AP a favorable outcome can be achieved when the root canal system is properly cleaned, shaped, and obturated, followed by placement of a well-sealed restoration. The high success rate is directly related to the quality of the clinical procedure. Therefore, clinicians must implement evidence-based strategies, utilize effective techniques, and select high-quali

Association between apical periodontitis and brain inflammation: a systematic review from animals and human studies.

Apical periodontitis (AP) is an oral inflammatory disease induced by dental pulp infection. It is characterized by inflammation, destruction, and resorption of mineralized tissues located around the dental apex. Although this is a local infection, pathogens and their products in the periapical area, as well as inflammatory cytokines, can reach other regions of the body and trigger a systemic immune/inflammatory response in the host. This condition has been associated with several types of systemic diseases. This systematic review aimed to explore the association between AP and neuroinflammatio

Detection of Apical Dental Resorption Caused by Endodontic Infection in Mice Using Fluorescence and Bright-Field Microscopy.

The aim of this study was to evaluate the sensitivity, specificity, and predictive values of the fluorescence microscopy method in the detection of apical dental reabsorption after induction of apical periodontitis in animal models. Forty-first molars of mice, aged 6 to 8 weeks, had their root canals exposed to the oral environment or were maintained healthy as controls (n = 20). After 14 and 42 days, mice were euthanized and tissues were collected for histological evaluation by means of bright field and fluorescence microscopy. The accuracy of fluorescence microscopy in identifying apical ext

[Histological changes of radicular cyst epithelial lining due to chronic inflammation].

The paper deals with the investigation of the relationship between chronic inflammation in the subepithelial layer and histological changes of the epithelium of radicular cysts. Histological examination of 60 radicular cyst specimens was carried out with the aim to define this relationship. The stratified squamous epithelium with a variety of histological changes was found in 58 specimens. A high degrees of chronic inflammation in the subepithelial layer was also recorded. Histological changes of the epithelium correlated to the degree of inflammation in the subepithelial layer of radicular cy

Latest Concepts in Endodontic and Periodontal Management of Diabetic Patients.

The close interrelationship between chronic oral and dental inflammatory diseases, such as periodontal and apical abscesses (AA), with the patient's systemic condition, is one of the most interesting topics of study common between dentists and the medical staff. Chronic apical periodontitis and periodontal diseases are chronic infections of the oral cavity in which similar Gram-negative anaerobic microbiota are found and in both infectious diseases increased local inflammatory mediators, levels may influence overall systemic conditions. One of the most important systemic diseases linked to per

Analysis of obtaining apical patency indexes during endodontic treatments considering gender, age, pulpoperiradicular diagnosis and canal/tooth-A clinical study.

This study aimed to perform an analysis of obtaining apical patency indexes during endodontic treatments considering gender, age, pulpoperiradicular diagnosis and canal/tooth (n. 639/383). Following previous clinical procedures, a thin K-File (No. 20, 15, 10 or 08) was used to achieve apical patency. These specific data and some demographic and clinical information were submitted to the statistical analysis (p < 0.05). Significant statistical differences were not identified considering gender (p = 0.156) and age (p = 0.793). However, in 14.6% of the canals of vital teeth and 14.1% of the canal

Local Immunomodulatory Effects of Intracanal Medications in Apical Periodontitis.

The immune system is an extremely complex biological network that plays a crucial role in the hemostasis of periapical tissue, pathogenesis of apical periodontitis (AP), and periapical tissue healing. The successful elimination of microbial infections remains a significant challenge, mostly because of the ever-growing development of antimicrobial-resistant pathogens. The bacterial endurance in the root canal system contributes to features ranging from altered posttreatment healing to exacerbation of chronic periradicular immune response, which compromise the outcome of endodontic treatment. A
