[Effectiveness of an irrigant in the chronic periapical abscess management: a case report].

The endodontic environment has optimal conditions for the growth of microorganisms that can release by-products into the periapical region of the tooth and cause inflammatory lesions. Chemical disinfection using irritants solutions plays an clinical important role, as they are able to remove waste from contaminated organic and inorganic tissues, generating a residual antibacterial effect. The purpose of this study was to show the effectiveness of 2.5% and 5.25% sodium hypochlorite as an irrigating agent for the clinical management of a chronic periapical abscess in a tooth with apical resorpti

Differentiation of periapical granulomas and radicular cysts by digital radiometric analysis.

It is generally accepted that two of the most commonly seen radiolucencies, the periapical granuloma and the radicular cyst, cannot be differentiated radiographically. The purpose of this investigation was to determine the feasibility of identifying radiometric differences in these two pathologic conditions by analyzing the patterns of the gray-level distributions of digitized radiographic images. Radiographs used in the study had been sent by practitioners to the School of Dentistry with tissue specimens. Radiographs of lesions identified as periapical granulomas and radicular cysts in the ma

An update on human periapical cyst-mesenchymal stem cells and their potential applications in regenerative medicine.

The broad clinical applications of Mesenchymal Stem Cells (MSCs) in the regenerative medicine field is attributed to their ability to self-renew and differentiate into multiple cellular lineages. Nowadays, MSCs can be derived from a variety of adult and fetal tissues including bone marrow, adipose tissue, umbilical cord and placenta. The difficulties associated with the isolation of MSCs from certain tissues such as bone marrow promoted the search for alternative tissues which are easily accessible. Oral derived MSCs include dental pulp stem cells (DPSCs), dental follicle progenitor cells (DFP

The endoscopically assisted transcrestal sinus floor elevation with platelet-rich fibrin at an immediate implantation of periapical lesion site: A case report.

RATIONALE: The traditional maxillary sinus floor elevation has serious postoperative complications and long healing periods, for patients with insufficient residual bone height (RBH). The endoscopic technique improves the blind nature of the sinus floor elevation procedure. Platelet-rich fibrin (PRF) can promote tissue healing and prevent perforation. PATIENT CONCERN: A 25-year-old female with residual roots in the maxillary right second molar visited our hospital for dental implants. DIAGNOSE: CBCT results showed a low-density shadow at the root tip, and the height of the periapical distanc

Persistent endodontic lesion due to complex cementodentinal tears in a maxillary central incisor--a case report.

The cementodentinal tear is rarely detected by noninvasive procedures owing to its clinical picture simulating a root fracture or a periodontal or endodontic lesion. We present a case of complex cementodentinal tears in a 79-year-old woman who presented a repeated swelling at the labial mucosa of the left maxillary central incisor for 6 months. Periapical radiographs demonstrated a vertical radiolucent fracture line extending from the root apex along the mesial aspect of the root to near the middle portion of the root of the left maxillary central incisor. Because endodontic re-treatment faile

pH-driven progression: Dental caries to pulpal and periapical disorders.

OBJECTIVE: This narrative review aims to examine the role of an acidic microenvironment in the pathogenesis of dental caries, pulpitis, and … ired, and pathogenic bacteria exhibit heightened virulence. Furthermore, acidic microenvironment in periapical tissue affects the activity of osteoblasts, osteoclasts and enzymatic processes, thereby increasing the bone resorption in apical periodontitis. Acidic microenvironment also compromises the effectiveness of endodontic treatment. Although many strategies ha …

Light microscopic study of periapical lesions associated with asymptomatic apical periodontitis.

The purpose of the study has been to evaluate the degree of chronic inflammation in tissues surrounding the apex of the tooth root in patien … Histological examination often reveals morphological signs of an active inflammatory process in the periapical tissues of patients treated during clinical remission. In our opinion, the presence of viruses in the epithelial cells of the radicular cyst may contribute to the persistence of the active stage of the inflammatory process.

Response to intracanal medication in immature teeth with pulp necrosis: an experimental model in rat molars.

INTRODUCTION: The present study aimed at developing an experimental model in rat molars for evaluating treatment strategies in necrotic immature teeth. METHODS: To define the periods to be adopted in the experimental procedures and to confirm induction of periapical lesions and interruption of root embryogenesis, the left lower first molars of 4-weeks-old Wistar rats underwent pulpectomy and were left open to the oral environment. Comparisons with the right lower first molars (vital teeth) were performed in animals with ages of 7, 10, 13, and 16 weeks. In another group of animals the teeth we

BiodentineTM Full Pulpotomy in Mature Permanent Teeth with Irreversible Pulpitis and Apical Periodontitis.

Vital pulp therapy, including direct pulp capping and partial and full pulpotomy, is primarily indicated for immature or mature permanent teeth with reversible pulpitis. Mature permanent teeth with irreversible pulpitis are frequently treated with root canal therapy. This report presents two cases of full pulpotomy using BiodentineTM in mature permanent teeth with irreversible pulpitis and acute apical periodontitis. The periapical radiograph illustrated a deep carious lesion extended to the pulp with apical radiolucency lesion or widened periodontal ligament space. Full pulpotomy with a trica

Dental pulp reaction to exposure at different time intervals in open apex canine teeth of cats.

INTRODUCTION: Open-apex teeth with irreversible pulpitis require complex and difficult treatment. Providing the right environment for apexogenesis and pulp protection is vital for their long term prognosis. The young pulp of open apex tooth, however, is better equipped against irritation and assault. The aim of this study was to evaluate pulp inflammation in open apex canine teeth of cats. MATERIALS AND METHODS: In this in vivo study, twelve cats with open apex canines were used. Pulps were iatrogenically exposed and the animals were sacrificed at one, seven, thirty and ninety days after pulp

Clinical and dental computed tomographic evaluation 1 year after coronectomy.

PURPOSE: Coronectomy is performed when contact between the mandibular third molar apex and the inferior alveolar nerve is suspected. The efficacy of coronectomy compared with conventional tooth extraction has been recognized in recent years. However, few studies have reported the postoperative prognosis of roots remaining in the bone or surrounding tissue. Therefore, a clinical evaluation was performed with dental computed tomographic imaging of the coronectomy sites 1 year after the procedure. PATIENTS AND METHODS: This study investigated 101 patients (116 teeth) who underwent a coronectomy

Inflammatory lesions of the tooth pulp induce changes in brainstem neurons of the rat trigeminal subnucleus oralis.

Neuroplastic changes are known to occur in the CNS in response to injury of peripheral nerves. Previous investigation has demonstrated neuroplasticity in second-order neurons of the subnucleus oralis (SO) of the trigeminal (V) nuclear complex in association with aseptic injury to the tooth pulp. A question arises, therefore, as to whether similar changes occur in response to injury associated with inflammation induced by tooth pulp infection. The effects of tooth pulp infection on the mechanoreceptive fields (RFs) of SO neurons were examined in rats. Infection was established by exposure and r

Three-dimensional cellular visualization in mouse apical periodontitis using combined whole-mount staining and optical tissue clearing.

Apical periodontitis is an inflammatory disease involving lesions located within the jawbone. Histological evaluations generally require decalcification and sectioning, which has limited our understanding of the three-dimensional (3D) organization and spatial distribution of different immune cell types in these lesions. A recently developed technique combining tissue clearing and whole-mount immunofluorescent labeling allows us to acquire such information from the deep tissue without sectioning. However, whole-mount immunofluorescent labeling in the jawbone requires further development. Here w
