Tibial tubercle transfer on a medial periosteal pedicle--a report of a new technique.

Tibial tubercle transfer is still probably the most widely used procedure of the numerous operative procedures described to realign the patella and extensor mechanism and to prevent a recurrent dislocation. Although this procedure most likely disturbs the blood supply to the tibial tubercle and thus may lead to a delayed union. Tibial tubercle transfer is also considered to play a role in the incidence of a tibial tubercle delayed union. Furthermore, a fracture of the tibial metaphysis has been reported to occur in some cases. We therefore devised a new procedure in which the periosteum of the…

The POLPSA lesion: MR imaging findings with arthroscopic correlation in patients with posterior instability.

OBJECTIVE: To evaluate the features of the posterior labrocapsular periosteal sleeve avulsion (POLPSA) lesion on MR imaging in athletes with posterior shoulder instability. DESIGN AND PATIENTS: Six male athletes (age range 19-43 years) with avulsion of the posterior glenoid periosteum were identified on MR imaging. There were four football players, one wrestler, and one competitive weightlifter. The weightlifter had a bilateral condition so that seven shoulders were evaluated. MR imaging was performed with a 1.5 T magnet utilizing conventional and fat-saturated fast spin-echo coronal oblique …

Reconstructing the Periosteal Niche with Regenerated Cellulose Nanofibers for Endogenous Bone Regeneration.

Large bone defects remain difficult to treat because current bone substitutes largely restore mechanical integrity without reconstructing the periosteal microenvironment that orchestrates endogenous bone regeneration. Here, we developed a periosteum-inspired composite scaffold by integrating a regenerated cellulose nanofibrous membrane onto a compressed hydroxyapatite scaffold to simultaneously mimic the biological interface and mineralized framework of native bone. A cellulose acetate electrospun membrane was converted into regenerated cellulose through deacetylation while preserving its extr…

[Choice of local anesthetic volume at the infiltration anesthesia on the upper jaw].

The aim of this study was to investigate the effect of volume and pressure, at which the solution of local anesthetic is injected into the tissue, on the effectiveness of infiltration anesthesia of the periosteum on the upper jaw. We used the technique of tooth pain sensitivity thresholds. According to the rate of development and the duration of the maximum analgesia effect, the optimum volume for different levels of pressure was determined. The influence of the tissue density at the injection site on the effect criteria of analgesia was demonstrated.

Recurrent gigantic pyogenic granuloma disturbing speech and mastication: a case report and literature review.

A case of gigantic pyogenic granuloma with three recurrences in the lower anterior gingiva is presented. Surgical wide excision of the lesion is the treatment of choice. The tumour must be excised down to the periosteum and the irritants around it removed to avoid recurrence. A contributing factor to the gigantic lesion is hormonal changes during pregnancy. Long-term review for 18 months after the third surgery showed no evidence of recurrence at the surgical site.

Osteoperiosteal flap in the treatment of ozena. New technique.

An osteoperiosteal flap taken from the anterior wall of the maxilla and pedicled on its periosteum was developed to narrow the nasal cavity of 15 patients with ozena. Good results were obtained in over 80% of cases. Crusting, fetor, and epistaxis disappeared. The nasal mucosa appeared healthy. The bulge in the lateral nasal wall persisted. The method is not liable to graft rejection or shrinkage. The advantages of this new technique are discussed.

Sternoclavicular physeal fracture associated with adjacent clavicle fracture in a 14-year-old boy: a case report and literature review.

We report a very rare association of a physeal fracture of the medial clavicular growth plate with a fracture of the adjacent clavicle in a 14-year-old boy who fell on his shoulder while playing football. Clinical, radiographic and computed tomographic (with three-dimensional reconstruction) features are described. Open reduction, internal fixation of the lateral fracture (with a reconstructive plate) and suture of the periosteum were performed.

Remodeling potential of long bones following angular osteotomies.

The radius and tibia of an immature monkey remodeled 5 degrees/year until maturity after osteotomies. The periosteum and the growth plate contributed equally to the correction. Valgus, varus, and flexion deformities corrected to the same degree. Epiphysiodesis of the adjacent growth plates did not prevent correction of the shaft. Remodeling at the osteotomy site was characterized radiographically by bone deposition on the concave side but no significant resorption on the convex side.

Local abaloparatide administration promotes in situ alveolar bone augmentation via FAK-mediated periosteal osteogenesis.

Insufficient alveolar bone thickness increases the risk of periodontal dehiscence and fenestration, especially in orthodontic tooth movement. Abaloparatide (ABL), a synthetic analog of human PTHrP (1-34) and a clinical medication for treating osteoporosis, has recently demonstrated its potential in enhancing craniofacial bone formation. Herein, we show that intraoral submucosal injection of ABL, when combined with mechanical force, promotes in situ alveolar bone thickening. The newly formed bone is primarily located outside the original compact bone, implying its origin from the periosteum. RN…

Guided Internal Fixation Technique for Transconjunctival Orbital Fat Transposition.

The predominant method for transconjunctival orbital fat transposition is by conducting 1 or several temporary externalized sutures. A new method that could internally and more securely fixate the fat to the periosteum remains desirable. This manuscript introduces a technique that utilizes a titanium ring to guide transconjunctival orbital fat fixation for lower blepharoplasty. With 8 to 10 mm transconjunctival incisions, the orbicularis retaining ligaments were released. An intraoperatively made ring guided the sutures that was made transcutaneously and anchored on the periosteum to exit the …

Solitary submandibular soft tissue osteochondroma: A rare case report.

INTRODUCTION: Soft tissue osteochondromas are rare benign tumors containing bone and cartilage that form in mesenchymal tissues with no connection to adjacent bone, cartilage, or periosteum. They mimic endochondral ossification and are usually encased in a fibrous tissue capsule. The occurrence of the tumor is extremely rare in the maxillofacial region. CASE PRESENTATION: A case of soft tissue osteochondroma in the submandibular region of a 47-year-old man with a medical history of muscular dystrophy and schizophrenia is presented here. The tumor had been gradually growing for 11 years before…

Periosteal stripping in high ankle sprains: An association with osteonecrosis.

OBJECTIVE: We present a series of 20 cases of a sleeve-type injury of the distal tibia characterized by traumatic periosteal stripping cause … tibial periosteal stripping may result in avulsion of the extra-osseous vasculature supplied by the periosteum, leading to osteonecrosis. This pattern of injury has not been emphasized in the literature. Our findings underscore the importance of the integrity of the periosteum for maintaining adequate vascularity of the distal tibia.

Improvement of Nasojugal Groove and Wrinkles Following a Skin-Muscle Flap Elevation Through a Subciliary Approach to the Orbital Rim.

The aim of this study is to see whether the nasojugal groove and wrinkles can be improved following a skin-muscle flap elevation through a subciliary approach to the orbital rim.Fifty-seven patients having fractures of the orbital floor, wall, or orbital rim were included. A subciliary incision was made 3 to 5 mm below the cilia. A skin-muscle flap was elevated and a 5-mm width of the periosteum of the anterior surface of the maxilla was exposed. Thereafter, the origin of the orbicularis oculi muscle was released from the underlying bony origin. An incision was made at the arcus marginalis. Af…
