Detailed investigation of alaryngeal speech to elucidate etiology of variation in quality.

Multiple parameters in the postlaryngectomy patient were statistically investigated with regard to acquisition and quality of alaryngeal speech. The 15 most significant variables are discussed. Esophageal peristalsis occurred during attempts to inject air for vocalization in 100% of the poor or nonspeakers. This may serve as an early indication to proceed with a synthetic vocal prosthesis.

Intussusception of vermiform appendix. A case report and review of the literature.

Intussusception of the appendix into the caecum is an uncommon condition and the diagnosis is rarely made preoperatively. Specific anatomic requirements as well as pathological conditions causing irritation or increasing peristalsis should be present for this event to occur. A case is discussed in which the diagnosis was made of an ileocolic invagination, which appeared peroperatively to be an appendicular invagination.

Control of chemotaxis in Physarum polycephalum.

Plasmodia migrate towards those situations which increase the frequency of their alternations in streaming, and away from those which decrease the frequency. Therefore peristalsis-like waves in Physarum move in the direction opposite from the net movement of the organism. The mechanism is fundamentally related to other known types of chemotaxis.

Microbiota-A Rescuing Modulator in Children Struggling with Functional Constipation.

Constipation affects around 30% of children and in 95% of cases is functional (FC), a consequence of alterations in digestive tract peristalsis, modulated by the immune and nervous systems, bile acid metabolism, and the gut microbiota. The aim of this review was to assess the role of gut microbiota and the use of probiotics in children with constipation. The current treatment involves education, toilet training, and oral laxatives, effective in only 50% of patients. In chronic FC, the composition of the microbiota is altered, with increased abundance of Bacteroidetes, Enterobacteriaceae, and F…

Intestinal crypt-derived enteroid coculture in presence of peristaltic longitudinal muscle myenteric plexus.

The role of enteric neurons in driving intestinal peristalsis has been known for over a century. However, in recent decades, scientists have begun to unravel additional complex interactions between this nerve plexus and other cell populations in the intestine. Investigations into these potential interactions are complicated by a paucity of tractable models of these cellular relationships. Here, we describe a novel technique for ex vivo coculture of enteroids, so called "mini-guts," in juxtaposition to the longitudinal muscle myenteric plexus (LMMP). Key to this system, we developed a LMMP cult…

Esophageal motor function and response to acid perfusion in patients with symptomatic reflux esophagitis.

There is much evidence to suggest that peristaltic function is defective in esophagitis patients and that this defect may contribute to prolonged acid exposure, promoting esophageal mucosal injury. Abnormal peristalsis may also be related to the generation of reflux symptoms. We evaluated primary peristalsis and its relationship to symptoms under basal conditions and during saline and HCl perfusion in 15 symptomatic reflux patients with gross esophagitis and 15 healthy controls. In the basal state, LES pressure (15.3 vs 25.1 mm Hg) and peristaltic amplitude (74.2 vs 104.8 mm Hg) were significa…

Paralytic ileus associated with capecitabine.

Paralytic ileus is a temporary arrest of intestinal peristalsis. We report on two patients with breast cancer who developed paralytic ileus following treatment with capecitabine. The pathophysiology of this disorder and its possible connection to capecitabine are described with the aim of promoting the early recognition of the clinical picture so that unnecessary surgery can be avoided.

Esophageal motility disorders other than achalasia.

Esophageal motility disorders can have a major impact on quality of life. Dysphagia is the most commonly reported symptom; however, patients with esophageal dysmotility can also present with other symptoms such as chest pain and tightness, food impaction, regurgitation and heartburn. It is important to be aware of the spectrum of esophageal motility disorders so that timely and accurate diagnosis can be made. The Chicago Classification uses a hierarchical classification system that divides motility disorders as disorders of outflow obstruction and disorders of peristalsis. The disorders of eso…

Management of the patient with esophagogastric junction outflow obstruction.

PURPOSE OF REVIEW: Esophagogastric junction outflow obstruction (EGJOO), defined as elevated integrated residual pressure (IRP) with preservation of esophageal peristalsis, is a common finding on high-resolution esophageal manometry. RECENT FINDINGS: The recent Chicago classification version 4.0 proposes changes to the criteria for diagnosing EGJOO, making this diagnosis more restrictive to now include elevated IRP in both supine and upright positions (with preservation of esophageal peristalsis), presence of obstructive symptoms, and confirmatory tests for EGJOO, such as timed barium esophag…

Uterine peristaltic activity during the menstrual cycle: characterization, regulation, function and dysfunction.

Unlike other smooth muscle organs, the uterine muscle was regarded to be normally functional for only a brief period, following a lengthy gestation. However, recently it has been shown that uterine peristalsis constitutes one of the fundamental functions of the non-pregnant uterus. Its morphological basis is the archimyometrium, which is the muscular component of the archimetra and which preserves a functional bipartition of the primarily unpaired uterus. Three types of uterine peristaltic contractions can be distinguished: cervico-fundal, fundo-cervical and isthmical peristaltic activity, whi…

Tailoring the small-bowel follow-through examination postoperatively in gastroschisis patients.

The purpose of this paper is to establish a small-bowel follow-through (SBFT) protocol in post-operative gastroschisis patients. In 15 years … hisis patients consist of (1) fluoro-evaluation of esophagus, stomach, and duodenum; (2a) if normal peristalsis is noted, then an overhead film at 30 min, or (2b), if altered peristalsis or little movement of contrast medium is noted on the 30-min overhead film, then an overhead film at 4 and 12 h. This is followed by overhead films every 24 h if required. Th …

[Evaluation of 2 homeopathic products on the resumption of transit after digestive surgery. A multicenter controlled trial].

The effects of two homeopathic preparations, Opium and Raphanus, given in dilutions of 10(30) (15 HC) and 10(10) (5 HC) respectively, on the resumption of intestinal peristalsis after operations on the digestive tract were evaluated in a controlled, double-blind therapeutic trial involving patients from 12 hospitals. The patients were allocated at random into 4 groups of 150 each: 1 untreated group and 3 groups who received either Opium and Raphanus, or Opium and the placebo of Raphanus or the placebos of the two products. The results were assessed on the time elapsed between closure of the ab…

[Prenatal diagnosis of an inguino-scrotal hernia in a fetus with trisomy 18].

In spite of its prevalence in neonates, an inguino-scrotal hernia is a rarely detected condition in prenatal life. The sonographic findings, i. e., a scrotal mass with intestinal peristalsis, the differential diagnosis, and possible associations between a scrotal hernia and chromosomal aberrations are described by presenting the case of a fetus with trisomy 18 and major structural anomalies.

[Clinikomorphological features of complicated colon diverticulosis].

79 surgical and postmortem cases of colon diverticulosis with complications were studied clinicomorphologically. Sequence in the development of inflammatory complications was revealed: diverticulum perforation-peridiverticulitis-pericolitic infiltrate, sometimes with the abscess-fistula. Segmental colitis may join the process. The complications onset depends mainly on discoordination of the intestinal peristalsis and intramural vascular deficiency.

Provisional model for propagation of electrical activity in upper urinary tract.

A theory on the function of the renal pelvis as a pacemaker for ureteral peristalsis is given. The bioengineering "black box" approach is used to describe peristaltic behavior monitored at the ureter and thr renal pelvis, by both peristaltic pressure and electromyographic methods. The model provides a stimulus to further thought on the nature and significance of electrical activity in the upper urinary tract.

Functional intestinal obstruction due to deficiency of argyrophil neurones in the myenteric plexus. Familial syndrome presenting with short small bowel, malrotation, and pyloric hypertrophy.

In 3 infants functional intestinal obstruction, associated with a short small intestine, malrotation, and pyloric hypertrophy, was shown to be due to failure of development of the argyrophil myenteric plexus, with the absence of ongoing peristalsis. 4 infants with similar clinical features have been described previously, and there is evidence for an autosomal recessive mode of inheritance of this syndrome.

Current status of small-bowel ultrasound.

Ultrasonography offers direct imaging of the bowel wall and allows dynamic evaluation of peristalsis. It helps to differentiate eosinophilic gastroenteritis from regional enteritis and lymphoma, displays a typical appearance in intussusception and is quite specific in the afferent loop syndrome, closed-loop obstruction and lymphedema. It may be helpful in ischemia of the bowel and in the evaluation of acute appendicitis.

Intramural gastric diverticula: a report of three cases.

Intramural gastric diverticula are true diverticula, entirely contained within the wall of the stomach without deformity of the serosa. All of the 13 known reported cases are at the greater curvature of the distal antrum. This is true of our three cases as well. The unique location and characteristic change in shape with peristalsis distinguish them from pathological entities.
