Combined common peroneal and tibial nerve injury after knee dislocation: one injury or two? An MRI-clinical correlation.

OBJECT Knee dislocations are often accompanied by stretch injuries to the common peroneal nerve (CPN). A small subset of these injuries also affect the tibial nerve. The mechanism of this combined pattern could be a single longitudinal stretch injury of the CPN extending to the sciatic bifurcation (and tibial division) or separate injuries of both the CPN and tibial nerve, either at the level of the tibiofemoral joint or distally at the soleal sling and fibular neck. The authors reviewed cases involving patients with knee dislocations with CPN and tibial nerve injuries to determine the localiz

Deep peroneal nerve palsy after opening wedge high tibial osteotomy: A case report.

RATIONALE: Peroneal nerve injury is one of the major complications that may occur after closing wedge high tibial osteotomy (CWHTO). In contrast, the reduced risk of the peroneal nerve injury has been considered as one of the advantages of opening wedge HTO (OWHTO). PATIENT CONCERNS: A 61-year-old male who underwent OWHTO showed a dropped big toe immediately after surgery. DIAGNOSIS: Injury of deep peroneal nerve was confirmed by electrodiagnostic study. It was probably caused by a posterolaterally protruded screw. INTERVENTIONS: The protruded screw was replaced with a shorter one 3 weeks a

Mast cell-associated Renaut bodies in the peroneal nerve of a dog with a history of mastocytosis.

Multiple Renaut bodies were identified by light microscopy in the biopsied common peroneal nerve of a dog with generalized neuropathy, hypothyroidism and a history of cutaneous mastocytosis. In addition, numerous granulated cells were associated with most of the Renaut bodies. Electron microscopic examination confirmed these to be mast cells, both central and peripheral to Renaut bodies, a phenomenon never previously reported. Endoneurial fibrosis, myelinated fiber loss, as well as evidence of axonal degeneration, demyelination and remyelination were observed. 'Vacuolation' of endoneurial fibr

Peripheral neuropathy associated with malignant neoplasms in dogs.

Common peroneal and ulnar nerves of 21 dogs between 2 and 15 years of age with malignant tumors were examined for possible paraneoplastic effects. Percent of abnormalities in single-teased fiber studies exceeded the confidence limits (P = 0.99) established for age-matched control dogs in 16 of 21 dogs (76%) with malignancies, none of which manifested clinical signs of polyneuropathy. The incidence of abnormalities was higher in common peroneal nerve (73%) than in ulnar nerve (57%). Lesions were characterized by a mixture of demyelination/remyelination and axonal degeneration; however, the form

Unilateral common peroneal nerve palsy following renal transplantation: a case report of tacrolimus neurotoxicity.

Neurologic complications are not uncommon in renal transplant recipients. Acute femoral neuropathy, lumbosacral plexopathy, and sciatic neuropathy have been reported after kidney transplantation probably due to perioperative nerve compression and ischemia. To the best of our knowledge, common peroneal nerve (CPN) palsy has not been described in the early postoperative period following renal transplantation. Also, mononeuropathy due to tacrolimus (TAC) therapy has not been described so far. We report a case of isolated CPN palsy presenting as unilateral foot drop following renal transplantation

F-wave conduction velocity in alcoholic polyneuropathy.

We used the F-wave conduction velocities to estimate the motor nerve conduction along the proximal segment (spinal cord to knee) of the axon of tibial and common peroneal nerves in ten patients with alcoholic polyneuropathy and compared these to the motor nerve conduction velocities in ten normal subjects. All the studies so obtained showed a normal conduction in the proximal segments even though in three patients the distal nerve conduction values (knee to ankle) revealed moderate to severe neuropathy. These data support the hypothesis that in alcoholic polyneuropathy, motor conduction in pro

Prediction of Functional Outcome in Axonal Guillain-Barre Syndrome.

OBJECTIVE: To identify the factors that could predict the functional outcome in patients with the axonal type of Guillain-Barre syndrome (GBS). METHODS: Two hundred and two GBS patients admitted to our university hospital between 2003 and 2014 were reviewed retrospectively. We defined a good outcome as being "able to walk independently at 1 month after onset" and a poor outcome as being "unable to walk independently at 1 month after onset". We evaluated the factors that differed between the good and poor outcome groups. RESULTS: Twenty-four patients were classified into the acute motor axona

Neurological Sequelae After Sacral Radiofrequency Ablation: A Case of Acute Cauda Equina Syndrome.

Radiofrequency ablation (RFA) is a minimally invasive technique that uses high-frequency electrical currents to generate heat, causing controlled destruction of targeted tissues through ionic agitation. RFA-associated neuropathy is a rare but recognized complication. We present a case of cauda equina syndrome and sensorimotor neuropathy following RFA of a sacral bone metastasis. Electromyography confirmed peripheral neuropathy, particularly affecting the peroneal nerve, and MRI revealed degenerative changes and a pelvic mass near the sacrum. The patient was treated with corticosteroids and neu

The medial plantar and medial peroneal cutaneous nerve conduction studies for diabetic polyneuropathy.

Objective of this study was to determine which nerve conduction is more sensitive electrophysiologically in the diagnosis of polyneuropathy in diabetics by evaluating the sensory conduction in medial plantar nerve and medial peroneal (dorsal) cutaneous nerves. Additionally to investigate the relation between Neuropathy Symptom Score (NSS) and Neuropathy Disability Score (NDS) values used in the diagnosis of these conduction studies. Forty patients with diagnosis diabetic neuropathy were included into this study. In diabetic polyneuropathic patient group, both medial plantar and medial dorsal c

Correlation of vitamin D and diabetic peripheral neuropathic pain: A cross-sectional study.

ObjectiveAlthough ample evidence exists on the association between vitamin D deficiency and diabetic peripheral neuropathy, only a few studies have distinguished between painful and painless forms. This study compared serum vitamin D levels in type 2 diabetes patients with and without painful neuropathy.MethodsWe enrolled 46 patients with diabetes (painless diabetic peripheral neuropathy, n = 15; painful diabetic peripheral neuropathy, n = 16; non-neuropathy, n = 15) and 15 healthy controls. Assessments included neurologic exams, Michigan Neuropathy Screening Instrument, electrophysiology, ski

Coexisting secondary intraneural and vascular adventitial ganglion cysts of joint origin: a causal rather than a coincidental relationship supporting an articular theory.

OBJECTIVE: To introduce the clinical entity of an intraneural ganglion cyst coexisting with a vascular adventitial cyst arising from the same joint. DESIGN: Retrospective review. PATIENTS: Two patients presented with predominantly deep peroneal neuropathy due to complex superior tibiofibular joint-related cysts. In addition to having peroneal intraneural ganglion cysts, these patients had vascular adventitial cysts: one involving a capsular arterial branch, the other a capsular vein [as well as a large, recurrent, intramuscular (extraneural) ganglion]. We then reviewed MRIs of 12 other conse

Peripheral Nerve Stimulation for the 21st Century: Sural, Superficial Peroneal, and Tibial Nerves.

OBJECTIVE: Peripheral nerve stimulation (PNS) of the lower extremity has progressed significantly over the last decade. From the proof of concept that ultrasound-guided, percutaneous implantation was possible to advances in waveforms, the field has been rapidly evolving. While most nerves in the lower extremity can be PNS targets, consideration must be given to the ergonomics of pulse generator placement, patient comfort, and avoidance of lead migration. For this paper, we examine some of the conditions amenable to lower extremity PNS, review the evidence and history behind PNS for these condi

Motor demyelinating tibial neuropathy in COVID-19.

Ten patients suffering from residual symptoms after the resolution of COVID-19, which manifested as fatigue in the lower limbs, have been submitted to nerve conduction studies. Motor demyelinating neuropathy features mainly of the tibial nerves but also the peroneal, median, and ulnar nerves were objectified. These findings might be considered as new neurological characteristics of SARS-CoV-2 infection.

Superficial peroneal nerve entrapment in a young athlete: the diagnostic contribution of magnetic resonance imaging.

Fascial entrapment of the superficial peroneal nerve produced severe pain in the ankle and foot of a 16-year-old female athlete after several sprains of the same ankle. The pain coexisted with erythema and sensory alteration of the area involved. Magnetic resonance imaging confirmed the diagnosis of this unusual neuropathy. Limited fasciectomy, at the point where the nerve becomes subcutaneous, relieved all symptoms.
