Persons with HIV Develop Spike-Specific Lymph Node Germinal Center Responses following SARS-CoV-2 Vaccination.

COVID-19 disproportionately affects persons with HIV (PWH) in worldwide locations with limited access to SARS-CoV-2 vaccines. PWH exhibit impaired immune responses to some, but not all, vaccines. Lymph node (LN) biopsies from PWH demonstrate abnormal LN structure, including dysregulated germinal center (GC) architecture. It is not clear whether LN dysregulation prevents PWH from mounting Ag-specific GC responses in the draining LN following vaccination. To address this issue, we longitudinally collected blood and draining LN fine needle aspiration samples before and after SARS-CoV-2 vaccinatio

Development and validation of the Brazilian Version of Lay Persons' Social Judgements about Cleft-lip Scale (B-LSojCleft-S).

The aim of this study was to develop and analyze the psychometric properties of the Brazilian Version of Lay Person's Social Judgement about Cleft-lip Scale (B-LSojCleft-S). A standardized photograph of a 16-year-old girl was manipulated by using photo editing software, to simulate different cleft-lip situations. The cleft-free image was used as a control. The researchers structured an initial questionnaire that was evaluated by experts and a sample of the target population to evaluate the construct. After analysis by the researchers, a final version of the B-LSojCleft-S containing 14 items wa

Development of a rare disease algorithm to identify persons at risk of Gaucher disease using electronic health records in the United States.

BACKGROUND: Early diagnosis of Gaucher disease (GD) allows for disease-specific treatment before significant symptoms arise, preventing/delaying onset of complications. Yet, many endure years-long diagnostic odysseys. We report the development of a machine learning algorithm to identify patients with GD from electronic health records. METHODS: We utilized Optum's de-identified Integrated Claims-Clinical dataset (2007-2019) for feature engineering and algorithm training/testing, based on clinical characteristics of GD. Two algorithms were selected: one based on age of feature occurrence (age-b

Utilisation of skin blood flow as a precursor for pressure injury development in persons with acute spinal cord injury: A proof of concept.

People with spinal cord injury (SCI) are at high risk of developing a pressure injury. It is unclear why some people with SCI develop pressure injury while others with similar predisposing risk factors do not during acute hospitalisation. This may hinder healthcare utilisation to prevent pressure injuries. The purpose of the study was to examine the proof-of-concept objective bedside skin blood flow measurements before a pressure injury develops in spinal cord injured patients during acute hospitalisation. This was an observational study. All participants had acute traumatic SCI and were press

Measuring the preferences of injectable opioid agonist treatment (iOAT) clients: Development of a person-centered scale (best-worst scaling).

BACKGROUND: Injectable opioid agonist treatment (iOAT) is effective for opioid use disorder (OUD), yet little is known about client preferences for accessing iOAT (e.g., with diacetylmorphine, hydromorphone, buprenorphine, fentanyl, etc.). Best-worst scaling (BWS) is a preference elicitation method from health economics that has never been applied to addiction care broadly, or iOAT specifically. We describe the stages of developing a BWS scale that assesses iOAT clients' treatment delivery preferences to inform program planning and maximize healthcare efficiency. METHODS: We underwent several

Incorporating health workers' perspectives into a WHO guideline on personal protective equipment developed during an Ebola virus disease outbreak.

Background: Ebola virus disease (EVD) health facility transmission can result in infection and death of health workers. The World Health Organization (WHO) supports countries in preparing for and responding to public health emergencies, which often require developing new guidance in short timelines with scarce evidence. The objective of this study was to understand frontline physicians' and nurses' perspectives about personal protective equipment (PPE) use during the 2014-2016 EVD outbreak in West Africa and to incorporate these findings into the development process of a WHO rapid advice guide

Developing personal attributes of professionalism during clinical rotations: views of final year bachelor of clinical medical practice students.

BACKGROUND: Medical professionalism as a set of behaviours that transcends personal values, beliefs and attitudes to incorporate ethical and moral principles is considered a covenant between society and the practice of medicine. The Bachelor of Clinical Medical Practice (BCMP) a three year professional degree was launched at the University of the Witwatersrand in January 2009 in response to a documented shortage of doctors especially in the rural areas of South Africa. The BCMP programme is unique in its offering as it requires a teaching approach that meets the needs of an integrated curricul

Using guiding-idea theories of the person to develop educational campaigns against drug abuse and other health-threatening behavior.

Educational campaigns against health-threatening behaviors such as drug abuse may lose effectiveness because they tend to be narrow, atheoretical and to stress negative effects. As a corrective, it is proposed that health campaigns take into account the broad range of human needs and consider how the health-threatening behavior may be gratifying to each need. This allows taking into consideration not just the bad effects (which tend to be obvious) of such behaviors as drug abuse, but also the gratifications they provide to a variety of human needs, by their associated lifestyles as well as by

Linear Regression Model for Predicting Dressing-Change Pain in Diabetic Foot Ulcers: Development, Internal Validation, and Personalized Analgesic Care.

This study included 232 patients with diabetic foot ulcers (DFUs) to construct and validate a linear regression model to predict pain during dressing changes and to evaluate the effectiveness of a model-based personalized analgesic care regimen. In stage 1,160 patients were divided into a training set (n = 113) and an internal validation set (n = 47); multivariable linear regression identified ulcer area, ulcer duration, duration of diabetes, and the Self-Rating Anxiety Scale (SAS) score as predictors of continuous dressing-change VAS. The model yielded an adjusted R2 of 0.458 and an RMSE of 1

[Transition to personalized software mpMR/US fusion prostate biopsy: a stratified retrospective analysis of 322 patients with development of a CHAID model].

INTRODUCTION: MRI-targeted biopsy is the standard method for prostate cancer diagnosis; however, it is typically supplemented with perilesional or systematic biopsy to minimize the risk of false-negative results. Nevertheless, several studies have demonstrated the feasibility of performing MRI-targeted biopsy alone in carefully selected patients. OBJECTIVE: To identify factors allowing MRI-targeted biopsy to be performed alone without compromising the detection of clinically significant prostate cancer (csPCa). MATERIALS AND METHODS: A retrospective analysis was conducted in patients who und

Development of a novel intervention using a person-based approach to support physical activity among families of children with cystic fibrosis in the UK.

OBJECTIVES: Cystic fibrosis (CF) is an inherited condition, affecting approximately 150 000 people worldwide. Physical activity (PA) is an integral component in the management of CF. However, it is estimated that only a third of young people (with and without CF) achieve UK Chief Medical Officer guideline recommended levels of activity. The aim of this research was to use the person-based approach to develop an intervention supporting families with young people (aged 6-12 years) with CF to incorporate PA as a sustainable habit in their lives to increase the likelihood of sustained PA levels go

MMpowerment: Empowering patients with multiple myeloma for shared decision-making by developing an intervention to integrate personal preferences into digital care pathways.

OBJECTIVE: As part of a quality improvement initiative in the context of value-based health care we aimed to optimize the shared decision-making (SDM) process in the care pathway for Multiple Myeloma as part of a digital care pathway (DCP). For this, more insight was needed in health care professionals' (HCPs') perspectives on SDM, and how SDM elements could be addressed in a DCP for MM to facilitate HCPs' performance of SDM. METHODS: HCPs were interviewed as per the theory of planned behaviour and the model of organizational context and SDM (phase 1). Multidisciplinary development sessions w
