Pyriform turbinoplasty (PT) is a surgical option for the management of turbinate hypertrophy. The philosophy and goal of the procedure are to improve the symptoms of a restricted airway while preserving function. We report a case of surgical management of inferior turbinate (IT) hypertrophy with PT and lateral nasal wall lateralization (LNWL). PT and LNWL improve nasal airflow, providing a wider nasal cavity by the removal of the bone of the IT. Sustained symptomatic improvement has been documented and is less susceptible to the influence of turbinate hypertrophy with other techniques.

Impactions with severely dilacerated root is seldom reported, especially the maxillary incisor. It is probably because of the high clinical difficulty of bringing the dilacerated tooth into position: most patients probably would choose extraction with replacement by prosthesis instead. However, the philosophy seems to have changed recently. We report two cases one in which an impacted central incisor was brought in the arch by orthodontic traction. In the second case the impacted premolar was brought into the arch and was shaped as a mandibular canine for achieving a good functional occlusion.

Operative treatment is considered to be the primary method of treatment for supraintercondylar fractures of the distal end of the humerus. Anatomical reduction and stable fixation of the fracture allows for early postoperative functional treatment and, subsequently, better results than other methods. Two series of patients with supraintercondylar fractures of the humerus, one from 1969 to 1980, and the second from 1981 to 1988, are presented. Although these series do not differ widely in their final outcomes, certain philosophies and operative techniques did change for the better.

The identification of individuals at increased risk to toxic substances in the workplace has developed into a major issue amongst OSHA, industrial management, and labor. What is needed is the development of a consistent philosophy which is designed to deal with the issues of genetic screening, job placement, and protection of high risk groups with appropriate federal exposure standards. Present evidence does not justify the use of programs designed to identify individuals with hereditary conditions (e.g., G-6-PD deficiency, sickle cell trait) for job denial and/or transfer.

Typhoid ileal perforation (TIP) is a common surgical emergency in low-middle income countries (LMICs). Its high surgical morbidity and mortality is due to its often late presentation or diagnosis, the patient's malnutrition, severe peritoneal contamination and unavailability of intensive care in most peripheral hospitals. This prompted the philosophy of minimizing the crisis by avoiding any repair or anastomosis, limiting the surgery in these physiologically compromised patients and performing only a temporary defunctioning ileostomy (DI) which could then be closed 10-12 weeks later.

BACKGROUND: The primary element of a crooked nose is a midline deviation of the nasal pyramid. To date, no surgical strategies have been described as compatible with the philosophy of dorsal preservation. The dorsal preservation technique differs from the Joseph structured rhinoplasty because it preserves both the keystone area and the continuity of the cartilaginous vault.
