Investigation of phonatory characteristics using ex vivo rabbit larynges.

Quantitative analysis of phonatory characteristics of rabbits has been widely neglected. However, preliminary studies established the rabbit larynx as a potential model of human phonation. This study reports quantitative data on phonation using ex vivo rabbit larynx models to achieve more insight into dependencies of three main components of the phonation process, including airflow, vocal fold dynamics, and the acoustic output. Sustained phonation was induced in 11 ex vivo rabbit larynges. For 414 phonatory conditions, vocal fold vibrations, acoustic, and aerodynamic parameters were analyzed a…

Immediate effectiveness of humming on the supraglottic compression in subjects with muscle tension dysphonia.

AIMS: To verify whether humming corrects supraglottic compression in muscle tension dysphonia (MTD) patients. METHODS: We enrolled 23 MTD participants (13 male, 10 female) showing supraglottic compression. Each individual was instructed to perform 3 types of phonation under transnasal laryngofiberscopy: natural phonation, humming phonation without pitch change and subsequent um-hum phonation, i.e. humming with pitch glide up as if agreeing with someone. The degree of supraglottic compression was estimated with 2 parameters. The false vocal fold and anterior-posterior indices (the FVF and AP i…

Vibratory source, vocal quality and fundamental frequency following pediatric laryngotracheal reconstruction.

OBJECTIVE: Surgeons who perform pediatric laryngotracheal reconstruction (LTR) have traditionally measured outcomes based on successful airw … nts were rated as having some degree of voice disorder. Those who used primarily supraglottic/mixed phonation exhibited significantly worse overall severity, roughness, and pitch deviance ratings than did those who used primarily glottic phonation. Significant differences in strain were also noted; however, poor inter-rater reliability rating of strain conf …

Dynamic extremes of voice in the light of time domain parameters extracted from the amplitude features of glottal flow and its derivative.

In the first part of the study, the subjects (4 females, 4 males) produced sustained breathy, normal and pressed types of phonation. The maximum AC flow (f(ac)) and the negative peak amplitude (d(min)) of the first derivative of the glottal flow were extracted from glottal volume velocity waveforms estimated by inverse filtering. The ratio between f(ac) and d(min) (amplitude quotient, AQ), which gives an estimate of the closing time of the glottal flow, and its normalised version (NAQ) were computed. The NAQ values correlated strongly with the closing quotient of glottal flow. In the second pa…

Effect of deep brain stimulation on different speech subsystems in patients with multiple sclerosis.

The effect of deep brain stimulation on articulation and phonation subsystems in seven patients with multiple sclerosis (MS) was examined. Production parameters in fast syllable-repetitions were defined and measured, and the phonation quality during vowel productions was analyzed. Speech material was recorded for patients (with and without stimulation) and for a group of healthy control speakers. With stimulation, the precision of glottal and supraglottal articulatory gestures is reduced, whereas phonation has a greater tendency to be hyperfunctional in comparison with the healthy control data…

Effect of the ventricular folds in a synthetic larynx model.

Within the human larynx, the ventricular folds serve primarily as a protecting valve during swallowing. They are located directly above the sound-generating vocal folds. During normal phonation, the ventricular folds are passive structures that are not excited to periodical oscillations. However, the impact of the ventricular folds on the phonation process has not yet been finally clarified. An experimental synthetic human larynx model was used to investigate the effect of the ventricular folds on the phonation process. The model includes self-oscillating vocal fold models and allows the compa…

Objective assessment of autophony in patients with patulous Eustachian tube.

The objective of the present study is to evaluate the Eustachian tube (ET) acoustic patency during phonation. The sound level in the EAC during phonation of the "A" and "N" sounds was measured by microphones in the bilateral EACs of nine normal subjects and 31 patients with patulous ET. The measured sound pressure differences between the right and left ears were correlated with the differences in severity of autophony between the bilateral ears assessed by a visual analogue scale (VAS). The patulous condition was often remarkable when the "N" sound was phonated. In some patients with patulous …

Effect of subglottic pressure on fundamental frequency of the canine larynx with active muscle tensions.

The relation between subglottic pressure and the fundamental frequency of vocal fold vibration was studied by means of evoked phonation in an in vivo canine model. The evoked-phonation model involved electrical stimulation of the midbrain that resulted in consistent responses by respiratory and laryngeal musculature, accompanied by phonation. The dynamic stiffness properties of the vocal folds, especially the "cover," were investigated by delivering various amounts of air pressure to the larynx from an opening in the trachea. The fundamental frequency of vocal fold vibration increased linearly…

[Voice disorders in children with velopharyngeal valving problems].

The purpose of this study was to examine if velopharyngeal valving problem can affect voice production (phonation) to some extent. Seventeen cleft palate children, mean age 7.0 had developed hoarseness for years without any treatment. In this study, each subject received a complete voice evaluation and the velopharyngeal (VP) function assessment. Voice evaluation included vocal cords condition and four voice parameters (quality, intensity, fundamental frequency and phonation behaviors). V-P function, classified into 3 levels (adequate, marginal & inadequate), was based on speech evaluation and…

Evaluation, treatment, and analysis of a rare case of motor speech systems dyscoordination syndrome.

This report describes an unusual presentation of a voice disorder arising from inability to coordinate the three components of motor speech: respiration, phonation, and articulation. These systems were individually intact, as demonstrated by laryngoscopy, motor speech examination, and treatment methods achieving success under controlled conditions. Following initial programming of his deep brain stimulation (DBS) device, a 62-year-old male, diagnosed with Parkinson's disease (PD) 14 years previously, abruptly experienced a vocal disorder characterized by pressed, very low frequency creaky voic…

Estimating subglottal pressure using incomplete airflow interruption.

OBJECTIVE/HYPOTHESIS: Subglottal pressure (SGP) measurements are often used to assess laryngeal function; however, current methods to measure SGP are invasive or do not estimate SGP during phonation. The purpose of this study was to devise a noninvasive technique to estimate SGP without interrupting phonation. STUDY DESIGN: This was a methodologic study designed to calculate the SGP of a human subject. METHODS: A new technique to estimate SGP was developed in which subject airflow was partially interrupted by introducing two different impedances (pneumatic resistors) through a balloon valve …

Normal values and predictive equations for aerodynamic function in British Caucasian subjects.

Many useful objective measurements of air volume and flow may be made with a high degree of validity and reliability using present-day respirometers within a speech clinic. We have studied measures of aerodynamic function in 76 British Caucasian subjects and derived reference equations for mean flow rate (MFR), phonation quotient (PQ), vocal velocity index (VVI), phonation time and the phonation volume/vital capacity ratio. Measures from resting breathing patterns have also been investigated. Age was found to be an important determinant in the reference equations. MFR, PQ and VVI were all grea…

Acoustic and perceptual comparison of chronic and incipient spastic dysphonia.

Vocal symptoms of patients with chronic and incipient spastic dysphonia were compared on a number of acoustic and perceptual parameters. Patients with incipient spastic dysphonia displayed less severe strain-strangle phonation, effort, rhythm, and stress. Harshness was the only perceptual parameter on which incipient spastic dysphonics were rated higher than chronic spastic dysphonics. Changes in the acoustic measures of laryngealization, harmonic change, mean vowel duration, and duration ratio between stressed and unstressed vowels accompanied changes in listeners' perception of strain-strang…
