Effects of jaundice and phototherapy on gastric emptying in the newborn.

Gastric emptying rate was measured in term infants without jaundice, with mild jaundice, and with moderate jaundice before and after phototherapy. Gastric emptying was significantly delayed in the moderately jaundiced infants (serum unconjugated bilirubin 233-332 mumol/l) but returned to normal following phototherapy. Mild jaundice (serum unconjugated bilirubin 170-204 mumol/l) had no effect on gastric emptying. Jaundice appears to inhibit gastric emptying, and phototherapy influences emptying indirectly by lowering serum bilirubin concentration. This effect is likely to be of clinical signifi…

Phototherapy-associated diarrhea. The role of bile salts.

The concentration of fecal bile salts was measured in 14 jaundiced neonates who received phototherapy (PT group) and their 14 nontreated matched controls (C). Before initiation of phototherapy, mean bile salt concentrations in stool specimens from the two groups were similar. At 12 hours of 'lights on', stool specimens from PT babies showed a significantly increased mean bile salt concentration, whereas in the C babies there was no change (3.65 +/- 0.39 vs 2.62 +/- 0.22 mmol/l; p less than 0.01). At 24 hours after 'lights off', stool specimens from the PT infants had a mean bile salt concentra…

Dose-response relationship of phototherapy for neonatal hyperbilirubinemia.

The data on dose-response relationship of phototherapy published by Tan [1977] were inserted into an e function, with energy in the exponent. The following features emerged: a) no real saturation point was reached when the spontaneous bilirubin decline was also taken into consideration, and b) a direct proportionality became evident between the two different, superimposed spectral ranges of irradiance measured and evaluated by Tan. Analogous proportionalities should be calculated for other spectral ranges and for other radiometers to simplify the comparison of irradiance data given by differen…

Dependence of the efficiency of phototherapy on plasma bilirubin concentration.

407 newborns with idiopathic transitory hyperbilirubinaemia were examined with regard to the decrease in serum bilirubin levels during 24 hours of intermittent phototherapy (12 hours of light exposure). The photoeffect (i.e. decrease of serum bilirubin concentration after 24 hours of therapy) showed a unique and predictable nonlinear correlation with the plasma bilirubin concentration before treatment. This relationship can be used for individualizing the duration of phototherapy and the dose of light. The apparent effect of birth weight, gestational age, and postnatal age on the efficiency of…

The pattern of bilirubin response to phototherapy for neonatal hyperbilirubinaemia.

The pattern of bilirubin response to phototherapy was studied using lamps with an emission spectrum closely similar to the bilirubin absorption spectrum. A dose-response relationship was demonstrated, with increased intensity of phototherapy resulting in greater response; however, the rate of bilirubin decline progressively decreased with increasing radiance till a 'saturation' point was reached, beyond which no further response occurred with further increase in radiance, suggesting a curvilinear relationship of the nature of an asymptotic regression. At the 'saturation' point, a 24-h decline …

Efficacy of phototherapy and/or exchange transfusions in neonatal jaundice.

In a review of treatment of neonatal jaundice, the authors studied the impact of phototherapy on outcome. From comparing exchange transfusion treatment over a 39-month period prior to the availability of phototherapy to a 39-month period incorporating phototherapy in the treatment regimen, the authors conclude that phototherapy (1) acts slowly but constantly to reduce bilirubin levels and (2) is effective even in severe hemolytic jaundice, but its effectiveness is inversely related to the degree of hemolysis. While phototherapy can never totally replace exchange transfusion, it can certainly r…

Effect of phototherapy on neonatal fluid and electrolyte status.

Three groups of "Работает' fullterm infants with hyperbilirubinaemia exposed to continuous phototherapy of different intensities, increased their oral intake of milk formula by 33%, 48% and 52% and 24, 48 and 72 h, respectively, irrespective of the intensity of exposure; this was significantly more than in the controls, who were swaddled. Serum osmolality and electrolyte values demonstrated no significant change from pre-phototherapy values, and were similar to those of the control group except for significantly raised potassium levels at 72 h in 2 of the 3 phototherapy groups. These raised val…

A comparison of fluorescent and nonfluorescent light sources for phototherapy.

We have compared fluorescent and nonfluorescent light sources for phototherapy for newborn infants with hyperbilirubinemia. Phototherapy was provided by a tungsten halogen lamp and conventional fluorescent light with identical radiant flux of 6 microW/sq cm. For 22 infants treated with the nonfluorescent lamp the mean duration of phototherapy was 33.77 hours and the mean reduction of bilirubin was 3.84 mg/100 ml/day. This did not differ significantly from infants treated with conventional fluorescent lights. The nonfluorescent light can be utilized for infants in incubators or on radiant warme…

Effect of various phototherapy regimens on bilirubin decrement.

Continuous phototherapy in full-term newborns was found to be more effective than intermittent illumination. Treatment efficacy was also related to age and the initial bilirubin level of the infants. In fact, the reported data indicate an increased therapeutic effect in newborns affected with nonhemolytic hyperbilirubinemia who had an initial bilirubin level greater than 15 mg/dl as compared to neonates with an initial bilirubin level less than 15 mg/dl. The light treatment was also more effective in infants older than 3 days, possibly because of an increased ligandin and conjugating capacity.…

Phototherapy for neonatal hyperbilirubinemia in "Работает" and "ill" infants.

The effectiveness of phototherapy for nonhemolytic hyperbilirubinemia was evaluated in two groups of closely matched newborns, one group consisting of "Работает" and the other "ill" infants. Phototherapy was equally effective in reducing bilirubin levels in both groups of infants. It would seem to be the treatment of choice in all infants with nonhemolytic hyperbilirubinemia, especially ill infants in whom its simplicity, safety, and effectiveness would make it an ideal form of therapy.

Long-term effect of phototherapy on visual function.

Computer-averaged electroretinogram records were used to examine scotopic retinal functioning of a group of 4-year-old children who were treated for neonatal hyperbilirubinemia by exposure to at least 42 consecutive hours of continuous phototherapy. Dark adaptation functions of the children were similar to those previously found for control subjects, suggesting that no permanent damage to rod function had been incurred during exposure. Statistical analysis showed no significant differences in the final electroretinographic amplitudes of light-treated and control subjects. Ophthalmologic and ne…
