Plasma exchange (PE) is the treatment of choice in emergencies arising in patients suffering from the hyperviscosity syndrome, factor VIII antibody haemophilia, autoimmune haemolytic anaemia or post-transfusion purpura. Apart from these clear indications, the efficacy of PE in other hematological disorders such as thrombotic thrombocytopenic purpura, immune thrombocytopenia, cryoglobulinaemia, rhesus incompatibility, autoimmune cytopenia and AB0 incompatibility with bone-marrow transplantation is less well defined. In such disorders PE can be performed when conventional treatment has failed.

41 patients with pemphigoid entered a multicentre randomised study of the efficacy of plasma exchange. All patients received 0.3 mg/kg daily oral prednisolone, increased weekly if the disease remained active. 24 patients received plasma exchanges in addition (eight large-volume exchanges over 4 weeks), and 17 had prednisolone only. 4 patients, 2 in each group, were withdrawn from the study. Lysagth et al have reported a semplified model of spontaneous plasmapheresis by placing between an A-V shunt a conventional plasmafilter. On the basis of this experience we tried to make a further semplification of this apparatus using a single venous puncture, obtaining by gravity sufficient transmembrane pressure for plasma separation. By alternatively lowering and elevating the system, plasma is separated from the blood and packed red cells are reinfused after a new pass through the filter with FFP or plasma substitutes.

Single radial immunodiffusion has been used to evaluate immunoglobulins and secretory piece (SP) in human seminal plasma. Samples were collected from 90 healthy volunteers, 202 subjects submitted to vasectomy for contraceptive purposes, tested at various intervals after surgery, and 725 patients grouped according to selected andrological disorders. Plasma exchange has been explored as an adjunctive form of therapy in the management of a variety of autoimmune diseases including Systemic Lupus Erythematosus, Goodpasture's syndrome, Rapidly Progressive Glomerulonephritis, Polyarteritis Nodosa, Wegener's Granulomatosis, and Rheumatoid Arthritis. The Evans-blue distribution volume, haematocrit, and plasma protein concentration were investigated in non-hydrated (control), hydrated, and acutely nephrectomized hydrated, anaesthetized dogs. Plasma testosterone concentrations were normal in 17 hospitalized alcoholics after a large dose of alcohol and in 16 Skid Row alcoholics. The findings fail to support the idea that the direct effects of alcohol on steroid metabolism cause gynecomastia and testicular atrophy.

The plasma testosterone values were determined by radioimmunoassay in a group of 42 patients with a severe form of hypospaida, in one case of epispadia, in a control group of 69 boys with an adequate development of sex organs, and in 48 well fertile men.
