Bacteraemia with Haemophilus pneumonia is uncommon. To determine its incidence and features case notes of patients in whom Haemophilus spp. were isolated from blood and pleural fluid over a five-year period were reviewed. Eight adult patients with H. influenzae bacteraemia were identified, five of whom had pneumonia on clinical and radiographic criteria. In this paper we describe a case of bilateral broncho-pneumonia due to invasion by mycoplasma pneumoniae, leading to ARDS. A high serum level of cold agglutinins, a mild haemolysis and a rash resembling that of an erythema multiforme were noted. The initial treatment was non-specific. After pathogen identification tetracycline therapy was initiated. Bacterial superinfections were treated with appropriate antibiotic schemas. The observed biphasic clinical course could be explained as an immunological phenomenon.

Roentgenographic findings of bilateral interstitial involvement without segmental consolidation and with regional lymphadenitis, when occurs after infancy, are always suggestive of pneumonia caused by Mycoplasma pneumoniae, as this is one of the most frequent typis of pneumonia in children. M. pneumoniae can be presumed from the discrepancy between the extensive roentgenographic findings and the generally good condition of the patient. Before therapy is decided upon it is important to know what the etiology is.

The damage caused by aspiration depends on the volume, nature and pH of the aspirate and the type of pathogen involved. Aspirate with a pH below 2.5 is much more likely to be toxic than less acidic material. Anaerobic organisms are common pathogens, but aerobic bacteria frequently appear in the hospital setting. Steroids may be useful if administered within minutes of the aspiration. Lavage with large amounts of alkaline fluid has not proved helpful.

A case of pneumonia and empyema caused by Neisseria meningitidis is described in which the diagnosis was initially missed. Meningococcal pneumonia and empyema are rare. It is important to recognize unusual manifestations of meningococcal infection promptly because of the risk of spread to contacts including health care personnel and other patients.

A patient with ulcerative colitis developed eosinophilic pneumonia following treatment with Salazopyrin. The pneumonia resolved following discontinuation of the drug and the start of treatment with prednisolone. When the treatment with prednisolone was stopped the patient developed eosinophilic pneumonia again. When the treatment with prednisolone was resumed all side effects disappeared definitively.
