Everyone wants to help the uninsured, but who will pay the bill? Why a national lottery may make sense.

There is a growing dissatisfaction with the American health care system that results primarily from the problem of the uninsured. This problem threatens not only the health of the uninsured, but also the health of our hospitals and ultimately of all Americans. There is a growing support for a universal health care plan, but less support for new taxes to pay for such coverage. One method to pay for universal health care may be to use the proceeds of a national lottery. In addition to possibly being more politically acceptable, there are a number of specific benefits that may follow from such a

"You can't do that 'round here": a case study of the introduction of medical abortion care at a University Medical Center.

Mifepristone medical abortion was introduced in 2002 into the University of New Mexico Medical Center clinic system through a joint effort of the departments of Family Medicine and Obstetrics and Gynecology. A stepwise approach to the integration of medical abortion, manual vacuum uterine aspiration, and first trimester obstetric ultrasound was successful in overcoming a series of educational, political, economic and logistical challenges. The integration of medial abortion care into resident training in FM and Ob/Gyn may expand access to reproductive services in primary care settings.

The privatization of human services: myths, social capital and civil society.

It is fashionable to point to privatization and the involvement of for-profits as the parties responsible for many, if not most, of the ills that plague the social welfare system today. This article takes a contrary point of view. Three arguments are made. First, private sector human service delivery and the use of for-profits in the United States predate privatization as a defined public policy. Second, the privatization of the human services is a world wide phenomenon that transcends politics and ideology. Third, the privatization of human services helps to promote civil society and generate

Old diseases and contemporary crisis. Inherited blood disorders in the Sultanate of Oman.

This contribution draws on ethnographic research conducted in Oman on inherited blood disorders (IBD). By interpreting results from population genomics studies that trace mutation processes over centuries of human activities, lay-representations of IBD often consider them historical evidence. The perceived spread of IBD in Oman may thus provide unusual historical depth in a country where past conflicts have been erased from historiography and representations of time are politically prescribed. Through the notions of crisis and diversification, this contribution shows how IBD's chronicity chall

[The significance of psychology in medicine].

It is shown in a retrospective review how the specific political climate twenty years ago helped to establish psychosomatics, psychotherapy and medical psychology as disciplines in their own right. The "reform-minded" trend gave psychotherapy recognition as a treatment method to be acknowledged as cost-refundable under the Federal German health insurance system. Furthermore, it prompted the Federal German government to start an inquiry into the present situation of psychiatry and psychosomatic medicine, whereas research workers at the base developed models of psychosocial working teams and of

Local health policies under the microscope: consultants, experts, international missions and poliomyelitis in Spain, 1950-1975.

One of the main focuses of analysis of this paper concerns the missions of international health agency experts to Spain to report on the situation, the activities in the fight against physical disabilities in children and on the actions taken to cope with the problem. The Spain-23 Plan was the instrument used by WHO and other agencies to start the process of change in a country undergoing a period of transformation under the enduring Franco dictatorship. As key sources, the paper uses unpublished reports of WHO experts on the subject, which resulted from visits to the country between 1950 and

Of moths and men: Theo Lang and the persistence of Richard Goldschmidt's theory of homosexuality, 1916-1960.

Using an analogy between moths and men, in 1916, Richard Goldschmidt proposed that homosexuality was a case of genetic intersexuality. As he strove to create a unified theory of sex determination that would encompass animals ranging from moths to men, Goldschmidt's doubts grew concerning the association of homosexuality with intersexuality until, in 1931, he dropped homosexuality from his theory of intersexuality. Despite Goldschmidt's explicit rejection of his theory of homosexuality, Theo Lang, a researcher in the Genealogical-Demographic Department of the Institute for Psychiatric Research

Body dissatisfaction is improved but the ideal silhouette is unchanged during weight recovery in anorexia nervosa female inpatients.

BACKGROUND: Public health is a core governmental responsibility, with ministries or departments of health responsible for setting and ensuring adherence to standards, managing performance and instituting reforms as required. Although North Queensland (NQ), Australia has a well-developed health infrastructure, the COVID-19 pandemic exposed significant vulnerabilities in its public health surveillance and response system. Globally, research has highlighted how human and cultural elements ("system software") influence the effectiveness of infrastructure, governance, and data systems ("system hard

The impacts of gender and race on workplace violence against nurses and nursing assistants: A scoping review of the literature.

BACKGROUND: Workplace violence against health care workers is a global phenomenon has serious consequences for individuals involved, and for health care systems. Though this phenomenon has been widely studied, rates of violence remain high. Many studies tend to characterize workplace violence as a problem between individuals, largely failing to examine underlying social structures, including gendered power imbalances, that put some health care workers at greater risk of violence, both from recipients of care (patients and relatives/visitors) and from other health care workers. Intersectionalit

Whose Knowledge Counts? Gendered Knowledge and Community-Based Pollinator Conservation in Semi-Arid Ghana.

Pollinator decline poses significant challenges for biodiversity conservation and the sustainability of smallholder agricultural systems. Addressing these challenges requires not only ecological interventions but also governance arrangements that shape whose knowledge and perspectives influence conservation decisions. This study examines how gendered power relations affect ecological knowledge, conservation practices, participation, and authority in Community Resource Management Area (CREMA) and non-CREMA communities in semi-arid northern Ghana. Guided by Feminist Political Ecology, the study

Behavioural Systems Mapping of Solid Waste Management in Kisumu, Kenya, to Understand the Role of Behaviour in a Health and Sustainability Problem.

Poor solid waste management in Kisumu, Kenya, contributes to adverse health, social, and environmental outcomes as a result of open burning, illegal dumping, and reliance on landfills. Taking Kisumu as a case study, we use behavioural systems mapping (BSM) for the purpose of understanding the role of behaviour in this complex problem. We qualitatively analysed transcripts from focus groups and interviews with 45 stakeholders in Kisumu to construct a BSM of the perceived actors, behaviours, and behavioural influences affecting waste management, as well as causal links. Influences were analysed

Response of developing countries to biological threats: the case of the Republic of Haiti.

In spite of scientific progress, the world is still facing major biological threats. Not only are there epidemics caused by wildlife pathogens under natural conditions, there are also those caused accidently when researchers handle highly hazardous organisms stored in research laboratories, and those caused when countries use these organisms as biological weapons of war or when criminal groups use them for bioterrorism. Developing countries tend to be more vulnerable to such threats than developed countries owing to the poor resilience of their animal health systems, their advanced state of en

Priorities for reproductive health: assessing need in the older population in the Asia-Pacific region.

Best practice in healthcare entails the identification of priority health needs so that scarce resources can be most appropriately deployed in the development of services. In this light, the priorities for health of older women and men in the Asia-Pacific region are considered in terms of reproductive health. A broader definition of "reproductive health" is used here, which considers conditions that affect the reproductive systems of older women and men. Although previous fertility history may influence some of these conditions, such as breast or cervical cancer or uterine prolapse, others --

Co-creation as a knowledge mobilization process in digital health regulation in East Africa: The making of the DigiReg compass.

Regulating digital health in low- and middle-income countries requires policy-makers to organize and use multiple forms of knowledge, including academic research, technical expertise and experiential insights, to make tough decisions under conditions of existential uncertainty. We examine co-creation as a mode of knowledge mobilization in digital health regulation in Kenya, Rwanda and Uganda. Using content analysis and a multi-country stakeholder workshop, we trace how regulators, clinicians, technologists and civil society actors collectively interpreted comparative evidence on existing frame

The global response to HIV/AIDS in sub-Saharan Africa: achievements, challenges, and perspectives for the future.

Over four decades, the global response to HIV/AIDS has transformed the disease into a manageable chronic condition, driven by advances in antiretroviral therapy, global financing, and ambitious targets like the 90-90-90 and 95-95-95 goals. Yet HIV remains a major challenge, especially in sub-Saharan Africa, which bears the highest burden worldwide. While AIDS-related mortality has declined, the number of people living with HIV continues to grow, placing significant strain on health systems and financial resources. Persistent challenges include high rates of new infections, late diagnoses, ineq

Neuroenhancement and mood enhancement – Physiological and pharmacodynamical background.

Pharmacological neuroenhancement and mood enhancement are gaining tremendous importance in society. The main motivation for neuroenhancement and mood enhancement is the anticipated increase in attention and vigilance, better performance in learning and memory and mood stability to meet the complex demands of an exacerbating meritocracy. Most users apply drugs originally designated for attention disorders, sleep disorders or dementia. Application of related drugs in terms of enhancement strategies in healthy individuals is off-label per se, the acquisition and distribution illegal. Here, we fir

[Consumption of ultraprocessed foods : key factors and challenges].

Ultraprocessed foods (UPF) play an increasingly important role in modern diets and are associated with a higher risk of chronic illnesses. This article introduces the Nova classification system and evaluates the socioeconomic factors that influence the consumption of UPFs. The individual, socioeconomic, environmental, cultural, and political factors that shape eating habits are also explored. In addition, the other factors influencing UPF eating habits, such as increased availability, affordability, convenience, and marketing strategies, are described. Despite some conflicting scientific evide

Chronic functional gastrointestinal symptoms in Holocaust survivors.

In Nazi-occupied Europe (1939-1945), Jews were submitted to extreme mental and physical hardships (the Holocaust). This study was designed to investigate the impact of the severe protracted suffering on the development of chronic functional gastrointestinal symptoms. Thus, we studied 623 consecutive patients of Eastern European origin who had been admitted for nongastrointestinal complaints. They filled out detailed questionnaires, and were divided into the following two groups: A) Holocaust survivors [237 subjects who had been for at least 6 months in either German concentration/extermination

[Chilean health system: health response and challenges in the face of COVID-19].

This study analyzes the Chilean health system response to COVID-19 from 2020 to 2023, highlighting moments (according to their epidemiological and sociopolitical context) and its main measures. It also reflects on the opportunities and limits of the response of this system to the epidemic to reform it. This case study included a literature review, document analysis, and 19 semi-structured interviews with relevant institutional stakeholders. Results indicate that, amidst political instability, national health capacities received investments. The adopted measures highlighted the relevance of the

Rehabilitation doxa and practitioner judgment. An analysis of symbolic violence on health care provision in the Scottish prison system.

This paper presents an analysis of the symbolic conditions which govern health care provision in the Scottish prison system. The paper considers the wider context of Scottish prisons, where health care provision follows a similar structure both in juvenile and adult prisons. Our intention is to provoke a debate about the doxa (Bourdieu, 1977), which underlies decision making in respect of health care in prison, in a political environment where pragmatism, allied to the 'pathologisation' of social policies, health and criminal justice has been a hegemonic force.
