Local anesthetic intoxication is a medical emergency with high mortality. These drugs have not spesific antidotes, quick differential diagnosis and supportive treatment required in the case of exposure to toxic doses. We report the complications and anesthetic management of a patient who was scheduled for right carpal tunnel syndrome surgery under regional anesthesia, but mistakenly received injection of 140 mg procaine added to 10% 10 mL lidocaine (10 mL=1000mg) instead of 2% lidocaine (10mL=200 mg) as part of an axillary plexus blockade.

This study examined the effects of cocaine on cardiac output in conscious freely-moving rats. Although pressor responses were similar at all doses, 14 of 32 rats had consistent declines in cardiac output (> 15%) and greater increases in systemic vascular resistance after administration of cocaine (5 mg/Kg, i.v.). Procaine (10 mg/Kg i.v.) did not mimic this effect in either subgroup. We propose that a subpopulation of rats exists with an enhanced susceptibility to cocaine-induced cardiac and systemic vascular alterations at higher doses.

The authors report six cases of acute urethritis and three cases of acute cervicitis caused in all instances by Neisseria meningitidis group B. The patients, five heterosexual men, one homosexual man, and three female prostitutes were seen at a venereal clinic in Mexico City. All of them were initially diagnosed as having gonorrhea. Treatment with procaine-penicillin G cured all cases, both clinically and bacteriologically. These experiences reinforce the need to distinguish N. meningitidis from N. gonorrhoeae in the setting of sexually transmitted diseases.

An in vivo study of the response of the dog ureter to local anaesthetics administered systemically and by ureteric perfusion, revealed that lidocaine, procaine, and mepivacaine, caused hyperperistalsis. We studied the effects of immune sera, an antibiotic, and a corticosteroid used alone or in combination on leptospira infection in Mongolian gerbils (Meriones unguiculatus). The results suggest that the combined used of immune serum and corticosteroid (pledonizoron: PZ) inhibits the effects of serum and the marked effects of antibiotic (procaine penicillin-G: PC-G) on leptospirosis. Administration of succinylcholine (SCh) to chicks produces rigid paralysis and death due to respiratory impairment. The mechanism of the SCh effect is probably related to the multiple innervation of muscle fibres, leading to excessive intracellular accumulation of calcium. Penicillin concentrations in cerebrospinal fluid (CSF) were measured in 40 asymptomatic patients with syphilis, 10 of whom had neurosyphilis. The patients were treated with 2.4 MIU procaine penicillin a day intramuscularly in combination with 500 mg probenecid every six hours orally. This intramuscular treatment regimen did not consistently yield treponemicidal penicillin concentrations in the CSF (subtreponemicidal CSF concentrations were found in 17 patients, four of whom had neurosyphilis).
