Impact of Repeated Questioning on Interviewers: Learning From a Forensic Interview Training Project.

Forensic interviewers have a difficult job with high risk for career burnout and secondary trauma. Few studies have addressed how new forensic interviewers or trainees experience repeated questioning and multiple interviews. This study simulated the process of training new forensic interviewers through the creation of two interview videos in which social work graduate students participated as actors portraying the roles of interviewer and child. These films served as instructional aids preparing graduate social work students for professional child welfare roles while promoting research-based a

The interpersonal context of client motivational language in cognitive-behavioral therapy.

Previous research has found that client motivational language (especially arguments against change or counterchange talk; CCT) in early therapy sessions is a reliable predictor of therapy process and outcomes across a broad range of treatments including cognitive-behavioral therapy (CBT). Existing studies have considered the general occurrence of CCT, but the present study differentiated 2 types of CCT in early CBT sessions for 37 clients with generalized anxiety disorder: (a) statements that are uttered to express ambivalence regarding change versus (b) statements that are intended to oppose

"Yes, of course it hurts when buds are breaking": therapist reactions to an adolescent client's sexual material in psychodynamic psychotherapy.

This article focuses on the therapist's emotional reactions to a young female client's revelations about her fear of being raped that came up in the course of an open-ended psychodynamic psychotherapy. The client suffered from depression and emotional disturbance related to the overwhelming developmental tasks of adolescence, including individuation and psychosexual development. The patient's fears and nightmares not only raised ethical dilemmas in the therapist regarding how to handle the implications of these revelations but also reactivated the therapist's own issues from her adolescent per

Guidelines for the family therapeutic approach to eating disorders.

After a brief historical overview of the literature on family therapy in eating disorders, the authors' own clinical experience is summarized in some practical guidelines: give the family therapist a central position within the team, engage the family and/or spouse as co-therapist, explore the message of the eating disorder on different levels, neutralize the symptoms outside of the family interaction, promote direct communication, facilitate the separation-individuation process, and be available for a long time. Finally some special issues are discussed: sexual and/or physical abuse in the fa

Self-help groups.

Many people in developed countries see medicine as threatening health, not only by malpractice, clinical iatrogenesis, and inappropriate treatment, but also by diverting attention from social and environmental causes of ill-health. There is growing hostility towards health care that undermines the power of individuals to care for themselves or shape their own environments, and a corresponding rapid and substantial growth of self-help groups.

Health professionals and patients' perspectives on person-centred maternal and child healthcare in Burkina Faso.

CONTEXT: The person-centred approach (PCA) is a promising avenue for care improvement. However, health professionals in Burkina Faso (hereafter referred to as caregivers) seem unprepared for taking into consideration patients' preferences and values in the context of healthcare provision. OBJECTIVE: To understand the meaning attributed to PCA in the Burkina Faso context of care and to identify the challenges related to its adoption from the perspective of caregivers and women service users (hereafter referred to as patients). METHODS: An ethnographic qualitative research design was used in t

'Eating disorders are not about food, they're about life': Client perspectives on anorexia nervosa treatment.

Poor success rates and high levels of dropout are common features in the treatment of anorexia nervosa. Using semi-structured interviews, this study elicited the views of 12 women who were recovered, or in recovery, for anorexia nervosa and had received treatment. Results derived from a thematic analysis revealed the women's high degree of dissatisfaction with treatment and their perception that the treatment system is overly focused on, and driven by, food and weight. In contrast, what the women really wanted was to be seen and treated as a 'whole person' and to have a 'real' relationship wit

Revisiting poor insight into illness in anorexia nervosa: true unawareness or conscious disagreement?

BACKGROUND: To investigate and validate a novel approach to distinguishing between two possible sources of poor insight in anorexia nervosa: true unawareness, in which a patient is not aware that other people think there is a problem, and disagreement, in which a patient does recognize that others think there is a problem. METHODS: Thirty-nine patients with anorexia nervosa or eating disorder not otherwise specified-anorexia nervosa were given two versions of the Scale of Unawareness of Mental Disorder (SUMD), one in which they were asked about their own opinion and one in which they were ask

Preparing pharmacy students and pharmacists to provide tobacco cessation counselling.

ISSUES: Tobacco use and abuse is a major health risk for people across the world and is responsible for nearly 500 000 deaths in the USA annually. Currently, the accepted role of pharmacists is the provision of pharmaceutical care, which includes health promotion and prevention of disease. Pharmacists should work collaboratively with other health-care professionals to provide tobacco-cessation counselling to smokers. APPROACH: Recently, in the USA, a curriculum has been developed by faculty at a school of pharmacy and distributed to pharmacy schools across the nation in a train-the-trainer fo

Maternal behaviour during spontaneous and directed pushing in the second stage of labour.

It has become custom and practice within the midwifery and obstetric professions in the English-speaking western world for women to be required to follow very specific instructions on how to push in the second stage of labour. There is very little literature on the behaviour of women using spontaneous pushing in the second stage of labour. In a pilot study of a randomized controlled trial comparing spontaneous (n = 15) with directed (n = 17) pushing, the behaviour of the women was observed and recorded. The findings from the observational part of the study are reported in this paper. When push

Physician motivations for nonscientific drug prescribing.

Although there is increasing concern about inappropriate physician prescribing and how to devise programs to improve drug therapy decisions, little research has been published documenting the reasons for such misprescribing. We analyzed the motivations reported by 141 physicians who were part of a large multi-state randomized controlled trial of 'academic detailing.' The physicians were identified from state Medicaid prescribing records as moderate to high prescribers of cerebral or peripheral vasodilators, propoxyphene, or cephalexin, and were visited by clinical pharmacists serving as outrea

Psychological issues in genetic counselling.

In the case of prenatal diagnosis and its possible consequences, there is discussion on the limits of freedom of decision. Nevertheless, there is considerable agreement that the individual family should be helped to make its own decisions and to live with these decisions. To attain these goals psychological issues are necessarily of importance. This paper presents concepts on these psychological issues and how they have to be taken into account in counselling. The character of mutuality in providing information is emphasized as well as cognitive and affective processes determining the understa

Healing of genital injuries.

Child sexual abuse as well as accidental trauma may cause acute injuries in the anogenital area. Most data on residual findings following genital trauma come from longitudinal studies of children who have been sexually assaulted, undergone surgical procedures, or experienced accidental trauma. Like injuries in other part parts of the body, such injuries undergo a predictable pattern of healing. Most superficial injuries heal without any residual evidence. Deeper injuries, as well as those that become infected or experience repeated disruption, may produce permanent changes. While the presence

Burnout and its causes in Finnish dentists.

In the spring of 1986 burnout and its relation to social and physical environments and the nature of work were studied using a questionnaire sent to a sample of 232 dentists aged under 62 living in the province of Uusimaa, Finland. Most male and female dentists (71 and 67%, respectively) were working in group practices and most (88%) employed an assistant. Professional problems were generally (71%) solved by consulting colleagues. Half of those responding were very satisfied with their relationship to other dental staff. All but 9% of dentists experienced problems in their physical working env

Rude awakenings.

A new nurse recently complained in a national nursing publication that her manager, a 22-year veteran, refers to her staff as"dodos," "dipsticks" and "resident idiots." The vexed grad wanted to know whether she had to put up with this verbal abuse. It's amazing that she even had to ask. The Los Angeles Times reports that rudeness is epidemic in America, as "embedded in the workplace culture as time cards, fluorescent lighting and computers." EMTs and paramedics are prime targets. You endure it from patients, their families, bystanders, coworkers, managers, ED staffers and cops. You no doubt ta

The Experience of Pain Dismissal in Adolescence.

CONTEXT: Pain in adolescence has psychosocial effects lasting into adulthood, including increased likelihood of pain in adulthood and higher rates of depression and anxiety. Many adolescents with chronic pain describe skepticism of pain reports, but this has not yet been evaluated in a sample of typically developing adolescents. OBJECTIVE: The current study sought to describe the experience of perceived pain dismissal during adolescence in a community sample. METHOD: Nearly 2000 (1931) emerging adults were recruited from the community and answered open-ended questions regarding whether they
