Facial asymmetry and the oral surgeon.

Facial asymmetry is often caused by deformities of the mandible or maxilla. Mandibular deformities are often pronounced, and various surgical procedures have been developed to treat such conditions. Deformities of the maxilla are similarly deviated from the normal. Although they are not always so noticeable as those of the mandible, they adversely affect function, esthetics and mental wellbeing. There are instances where both jaws are at fault and require treatment. More recently, osteotomies in the anterior of the maxilla or mandible have been performed with success. If the blood supply to th

A tall rostral hook in a medieval horse premolar tooth.

Development of dental abnormalities due to improper occlusal wear is common among modern domestic horses. This phenomenon often is attributed to jaw conformation. Rostral mandibular hooks may develop in horses with underjet or mandibular prognathism, a condition where the lower jaw protrudes forward, beyond the upper jaw. Less abrasive diet, free of phytoliths and matrix-like plant fibers, also may promote enamel and focal overgrowths of equine dentition. Here we report a rostral mandibular hook in a lower premolar tooth of a medieval horse, found in a spring deposit in Levänluhta, Osthrobothn…

Nonextraction treatment of a skeletal Class III malocclusion.

This case report describes the nonsurgical, nonextraction therapy of a 16-year-old boy with a skeletal Class III malocclusion, a prognathic mandible, and a retrusive maxilla. He was initially classified as needing orthognathic surgery, but he and his parents wanted to avoid that. The Class III malocclusion was corrected with a rapid palatal expander and a maxillary protraction mask followed by nonextraction orthodontic treatment with fixed appliances, combined with short Class III and vertical elastics in the anterior area. The height of the maxillary alveolar process and the vertical face hei

Radiographic changes of the temporomandibular joint after oblique sliding osteotomy of the mandibular rami.

64 patients with mandibular prognathism were operated upon with oblique sliding osteotomies of the mandibular rami. The patients were followed with cephalometric and temporomandibular joint (TMJ) radiographs for 3 years. 60% of the TMJs showed skeletal remodelling 3 years postoperatively. The condylar remodelling was significantly correlated to the degree of postoperative displacement of the condyle. The average condyle displacement postoperatively was about 3 mm inferiorly and anteriorly, but the position was almost completely back to normal during the control period, due to a combination of …

Changes in facial appearance during cyclosporin treatment.

Substantial changes in facial appearance developed in a group of children treated with prednisone and cyclosporin for immunosuppression after renal transplantation. In general, the children's features became coarse. There was thickening of the nares, lips, and ears, puffiness of the cheeks, prominence of the supraorbital ridges, and mandibular prognathism. The extent of the changes varied, but all eleven children followed up for more than 6 months from transplantation showed some of the changes. Patients treated with prednisone and azathioprine did not show any changes, clearly implicating cyc…

[Dysplasia of the corneal limbus, the mesodermal iris layer and the jaw skeleton in a family].

Ten members of the family B./A. (belonging to two generations) who had dysplasia of the anterior mesodermal layer of the iris were examined. Several of them also had moderate microcornea. The shape of the skull was remarkable, with micrognathia, in some cases combined with mandibular prognathism and early loss of the teeth. Members of the older generation suffered from pannus, although only pingueculae had been present in youth and adolescence. The anomaly of the anterior part of the eye combined with "dysostosis maxillofacialis" (Peters and Hövels) may represent an abortive form of Rieger's s…

Spontaneous Temporomandibular Joint Dislocation.

Dislocation of the temporomandibular joint (TMJ) is the displacement of the head of the condyle from its normal position in the glenoid fossa. Anterior dislocations are the most common type of TMJ dislocations. Prognathism of the lower jaw, anterior crossbite, and open bite are the classic features in bilateral TMJ dislocations. The treatment of acute TMJ dislocation involves manual reduction with or without general anesthesia. The use of maxillomandibular fixation is reserved for chronic, more complex dislocations. Determining the presence of a predisposing factor is very important in guiding …

Impacted canine in a prehistoric skull.

Impacted teeth are frequently found in humans today, but current data suggest their existance in skulls originating from the prehistoric age. This report describes the skull of an adult female that was found during excavation in the Croation enolithic site of Vucedol (2700-24--BC). The skull is well preserved and, according to cranial and facial index calculation, is classified as dolichocephalic and mesoprospic. Cephalometric analysis revealed bimaxillary prognathism. The maxillary left canine is impacted and its crown peak is visible through the alveolar maxillary bone fenestration. The posi…

Myhre-LAPs syndrome and intubation related airway stenosis: keys to diagnosis and critical therapeutic interventions.

OBJECTIVES: Myhre-LAPS syndrome is a recently recognized disease caused by a mutation in the SMAD4 gene. This results in a range of pathology including laryngotracheal stenosis, arthropathy, prognathism and short stature, or LAPS syndrome. We aim to delineate the role of intubation in development of airway stenosis in these patients as well as provide insight into diagnosis and management of this syndrome. Herein we present four patients with Myhre-LAPS syndrome complicated by airway stenosis and perform a systematic review of all cases of Myhre-LAPS syndrome with reported airway pathology. S…

A dual-laminate snore reduction appliance: a report of four cases.

This article describes a snore reduction appliance that can be constructed to advance and hold the mandible in a comfortable prognathic position. The tongue is advanced concomitantly, producing more space in the pharynx while changing the relative positions of the soft palate, posterior tongue, and pharyngeal walls to reduce the incidence of snoring and ameliorate sleep apnea. A dual-laminate appliance, soft inside and hard outside, is vacuformfitted to casts of the maxillary and mandibular teeth and luted in a protrusive relation with cold pressure-cured acrylic. The appliance maintains the m

Impaction of maxillary canines and congenitally missing third molars. Description of an ancient skull (7250-6700 BC).

This report describes an ancient skull, in which both maxillary canines were impacted and mandibular third molars were missing. During the excavations at the early village site of Cayönü (Turkey), the subject of this report was found inside a skull building (charnel house) together with the fragments of 75 skulls, and was the least damaged skull of the Cayönü series (Ozbek, 1988). The skull showed skeletal and dental Class I relationship, with an orthognathic profile and a reduced bimaxillary alveolar prognathism. Assessment on the skull confirms the findings of the previous studies and report…

Combined multisegmental surgical-orthodontic treatment of bialveolar protrusion and chin retrusion with severe facial asymmetry.

LeFort I osteotomy, anterior segmental osteotomy, bilateral sagittal split ramus osteotomy, and genioplasty are frequently used methods for correcting facial deformities. However, in patients with an abnormally shaped maxilla or mandible, more complex surgical techniques or multiple combinations must be considered for improved esthetic results. This article presents a patient with bialveolar protrusion, mandibular prognathism, chin retrusion, a long face, and severe facial asymmetry. A combination of LeFort I asymmetric impaction, anterior segmental osteotomy, and 3-piece segmentation of the m…

Elsahy-Waters syndrome: Evidence for autosomal recessive inheritance.

Elsahy-Waters or branchioskeletogenital syndrome is a rare MCA/MR syndrome characterized by moderate mental retardation, hypospadias and characteristic craniofacial morphology, which includes brachycephaly, facial asymmetry, exotropia, hypertelorism/telechantus, broad nose, concave nasal ridge, underdeveloped midface, prognathism, and radicular dentin dysplasia. Here we report on a 44-year-old woman and her 45-year-old brother, born to consanguineous parents, who show a striking resemblance to the earlier described patients. The hitherto reported patients were male and in one pedigree parents …

Bimaxillary osteotomies in correction of dentofacial deformities.

In 342 patients with dentofacial deformities cephalometric analyses were preoperatively performed. A study of occlusion on cast models as the simulated new position of the facial soft tissues was done on the photographs. Surgical treatment using bimaxillary osteotomies for correction of dentofacial deformities was done in 38 patients: 31 with prognathism and maxillary hypoplasia, 5 with facial asymmetry and laterognathism and in 2 with otomandibular dysostosis. The LeFort I type osteotomy was used for the upper jaw and Obwegeser technique for the lower jaw. Intermaxillary and circumzygomatic i…

Fixed prosthodontic rehabilitation using a facially driven fully digital workflow of a patient with syndromic amelogenesis imperfecta associated with a rare form of ectodermal dysplasia, tricho-dento-osseous (TDO) syndrome.

Tricho-dento-ossseous (TDO) syndrome is a rare form of ectodermal dysplasia (ED) characterized by defects in the hair, bones, nails, and teeth. Dental findings consistently seen in patients with TDO syndrome include amelogenesis imperfecta and taurodontism, but additional findings such as periapical abscesses, mandibular prognathism, and impacted teeth have been reported. Because of the rarity of this disorder, the prevalence is unknown and limited research is available regarding its dental management. This clinical report describes the fixed prosthodontic implant rehabilitation of an adult pa…

Long-term stability of surgical-orthodontic treatment for skeletal Class III malocclusion with mild asymmetry.

Skeletal Class III malocclusion with mandibular prognathism is often associated with mandibular asymmetry, and patients with Cl III deformity and asymmetry may undergo orthognathic surgery to improve facial function and esthetics. However, the long-term stability of mandibular setback surgery has been considered "problematic". We report a 25-year-old female patient who was diagnosed as having a skeletal Class III relationship, mandibular asymmetry and unilateral crossbite. She underwent bilateral sagittal split-ramus osteotomy and orthodontic therapy, and this led to improvement of her dentofa…

Surgery-first approach in correcting skeletal Class III malocclusion with mandibular asymmetry.

This case report describes a surgical orthodontic case that used the recently introduced surgery-first approach to correct a severe skeletal Class III malocclusion. A 19-year-old woman presented with severe mandibular prognathism and facial asymmetry; she had been waiting for growth completion in order to pursue surgical correction. After prediction of the postsurgical tooth movement and surgical simulation, 2-jaw surgery that included maxillary advancement and differential mandibular setback was performed using a surgery-first approach. Immediate facial improvement was achieved and postsurgic…

Supernumerary teeth and dental management in Sotos syndrome.

Sotos syndrome is a genetic disorder characterized by early and excessive growth, acromegalic features, mental retardation, advanced bone age, and a distinctive facial appearance. The oral findings commonly seen with Sotos syndrome include: premature eruption of teeth; high, arched palate; pointed chin; and, more rarely, prognathism. The purpose of this case report was to review the diagnostic characteristics of Sotos syndrome, highlighting an interesting case involving six supernumerary teeth, and emphasize the importance of a multiprofessional dental intervention in combination with active f…

Chromosome subband 17p11.2 deletion: a minute deletion syndrome.

Interstitial deletion of the short arm of chromosome 17 was detected in three unrelated patients with mental retardation and multiple congenital malformations. These patients were identified at a single centre over a six month period suggesting that del(17) (p11.2p11.2) is not a rare constitutional chromosome rearrangement. Comparison of the phenotypic features in a total of 19 patients with del(17)(p11.2p11.2) shows a consistent clinical phenotype with moderate to severe mental retardation, microbrachycelphaly, prominent forehead, broad face, flat midface, prognathism, short, broad hands, and…
