[Vitamin D-related progressive renal insufficiency in an elderly patient with postsurgical hypoparathyroidism associated with extensive brain calcification].

We present a 76-year-old female patient with dementia who has postsurgical hypoparathyroidism associated with extensive brain calcification and progressive renal insufficiency. She had been treated with vitamin D combined with calcium or vitamin D alone due to hypoparathyroidism for 8 years. However, intermittent hypercalcemia including hypercalcemic crisis (serum Ca 15.2 mg/dL) and progressive renal dysfunction had developed. This patient was transferred to our long-term care hospital because of worsening dementia. Since laboratory data at admission revealed hypercalcemia and azotemia, alfaca

Pneumonia caused by co-infection with Mycobacterium tuberculosis and Pneumocystis jirovecii leading to acute respiratory distress syndrome in an HIV-negative immunocompromised patient: a case report and literature review.

BACKGROUND: Pulmonary tuberculosis (PTB) and Pneumocystis jirovecii pneumonia (PJP) often occur in immunosuppressed populations, particularly in individuals with human immunodeficiency virus (HIV) infection. However, co-infection with these two pathogens resulting in acute respiratory distress syndrome (ARDS) has been less frequently reported, especially in HIV-negative patients. CASE PRESENTATION: We report the case of a 68-year-old immunosuppressed male patient with pneumonia caused by co-infection with Mycobacterium tuberculosis and Pneumocystis jirovecii, leading to ARDS, who was successfu

A Japanese-American female with rapidly progressive interstitial lung disease associated with clinically amyopathic dermatomyositis.

Patients with clinically amyopathic dermatomyositis (CADM) have a risk of developing rapidly progressive interstitial lung disease (ILD). CADM-ILD is associated with the anti-MDA-5 antibody. In the USA, however, patients with CADM have these antibodies less frequently than those in Japan. In addition, those with this disorder are less often complicated with rapidly progressive ILD than those in Japan. We present a case of a 42-year-old Japanese-American female with a 3-month history of a rash on her hands and face with joint pain. Based on the negative results from lupus tests, her primary car

Fournier's gangrene: evolving understanding and modern management of a necrotising surgical emergency.

Fournier's gangrene (FG) is a rapidly progressive, life-threatening necrotising soft tissue infection of the perineum and external genitalia that remains a surgical emergency with a high mortality despite advances in critical care. Once considered idiopathic, contemporary evidence demonstrates that FG most often arises from identifiable anorectal, urogenital, or cutaneous sources in patients with significant comorbidities, particularly diabetes mellitus and other states of immunocompromise. Mortality remains substantial, typically in the range of 20-30% and is strongly influenced by delays in

The influence of ethnicity on the extent of, and reasons underlying, delay in general practitioner consultation in patients with RA.

OBJECTIVE: Delay in assessment by rheumatologists of patients with new-onset RA is an important determinant of delay in treatment initiation. The influence of ethnicity on delay in assessment has not been addressed. We studied the extent of delay in patients of South Asian origin compared with other patients and the reasons underlying this delay. METHODS: Data were collected from 272 patients with RA at the time of assessment by a rheumatologist; 43 were of South Asian origin. Delays were recorded at the level of the patient seeking advice from the general practitioner (GP), the GP referring

Multidisciplinary management of Duchenne muscular dystrophy from childhood to adulthood.

Duchenne muscular dystrophy (DMD) is a severe, progressive neuromuscular disorder characterized by muscle degeneration and weakness, leading to significant physical disability and multisystem complications. The management of DMD requires a coordinated, multidisciplinary approach to address the evolving needs of patients from childhood through adulthood, with a focus on patient quality of life. Early diagnosis and proactive interventions are critical to optimizing outcomes and may include physical therapy, respiratory support, cardiac monitoring, and orthopedic care. In children, the focus is o

Dental management of long-term amyotrophic lateral sclerosis: case report.

Amyotrophic lateral sclerosis (ALS) is a progressive motor neuron disease. Methods of dental treatment of a young male patient with ALS are presented. This case is unusual in several respects: the early age of onset, the long survival time, and the period of time in which the case was followed in our dental clinic. Aspects of ALS which are of concern to dentistry, as related to clinical care and strategies for effective oral health delivery, are presented.

Understanding critically ill patients hemodynamic response to mobilization: using the evidence to make it safe and feasible.

In today's critical care environment, we face a difficult but essential task. We must provide comprehensive, compassionate, complex, technological care without causing harm to our patients. To foster a patient-safe environment, we must examine care practices and processes to reduce the chance of error. Successful early mobilization of critically ill patients can reduce several complications including atelectasis and ventilator-associated pneumonia and shorten ventilator time along with cognitive and functional limitations that linger 1 to 5 years after discharge from the intensive care unit. A

Utilization of Rituximab for Refractory Rowell Syndrome.

Rowell syndrome describes the occurrence of erythema multiforme-like lesions in patients with cutaneous lesions of lupus erythematosus. The clinical picture of atypical erythema multiforme-like lesions, presence of chilblains, speckled ANA pattern, anti-Ro/SSA, or anti-La/SSB antibodies, and absence of infectious or pharmacologic triggers in a patient with systemic lupus erythematosus are some of the classic clinical and serologic features. Histopathologic and serologic findings can help differentiate this process from erythema multiforme. We present a case of young woman with systemic lupus e

Reflections on the natural history of lipomyelomeningocele.

A retrospective analysis of patients with lipomyelomeningocele cared for at two referral centers was completed to derive relationships between neurological function and patient age. Thirty-seven percent of 177 patients had intact neurological function on initial examination. Neurological deficits were progressive and linked with a logarithmic relationship to increasing patient age. Each child with intact examination retained normal bladder function following lipomyelomeningocele repair and release of cord tethering; complications of surgery were limited. Our analysis suggests that surgery on p

Diagnosis and management of progressive ataxia in adults.

Progressive ataxia in adults can be difficult to diagnose, owing to its heterogeneity and the rarity of individual causes. Many patients remain undiagnosed ('idiopathic' ataxia). This paper provides suggested diagnostic pathways for the general neurologist, based on Ataxia UK's guidelines for professionals. MR brain scanning can provide diagnostic clues, as well as identify 'structural' causes such as tumours and multiple sclerosis. Advances in molecular genetics, including the wider and cheaper availability of 'next-generation sequencing', have enabled clinicians to identify many more cases w

Flemish network on rare connective tissue diseases (CTD): patient pathways in systemic sclerosis. First steps taken.

Despite the low prevalence of each rare disease, the total burden is high. Patients with rare diseases encounter numerous barriers, including delayed diagnosis and limited access to high-quality treatments. In order to tackle these challenges, the European Commission launched the European Reference Networks (ERNs), cross-border networks of healthcare providers and patients representatives. In parallel, the aims and structure of these ERNs were translated at the federal and regional levels, resulting in the creation of the Flemish Network of Rare Diseases. In line with the mission of the ERNs a

Rapid Progression to Acute Respiratory Distress Syndrome: Review of Current Understanding of Critical Illness from Coronavirus Disease 2019 (COVID-19) Infection.

The coronavirus disease 2019 (COVID-19) outbreak that started in Wuhan, Hubei province, China in December 2019 has now extended across the globe with >100,000 cases and 3,000 deaths reported in 93 countries as of 7 March 2020. We report a case of COVID-19 infection in a 64-year-old man who developed rapidly worsening respiratory failure and acute respiratory distress syndrome (ARDS) that required intubation. As the clinical spectrum of COVID-19 ranges widely from mild illness to ARDS with a high risk of mortality, there is a need for more research to identify early markers of disease severity.
