Despite Freud's dictum that dreams are the royal road to the unconscious, the use of dream analysis by therapists working with Borderline Personality Disorder and other severe psychiatric conditions has in the past two decades has fallen into a state of decline, if not outright neglect. The reasons why are not altogether clear, though some have said that the growing popularity of ego psychology and other movements in the domain of psychoanalysis have perhaps pushed dream analysis to one side. To me this marginalization seems unjustified. I hope to demonstrate in this article the enduring utili

There is a significant lag in the development of evidence based approaches for eating disorders in children and adolescents despite the fact that these disorders typically onset during these developmental periods. Available studies suggest that psychotherapy is the best available approach to these disorders. Specific studies support the use of family based interventions, adolescent focused individual therapy, and developmentally adapted cognitive behavioral therapy in this age group. The current report summarizes the available evidence supportive of each of these treatment modalities, as well …

Clinical and research applications of motivational interviewing (MI) have grown at a remarkable pace over the past 25 years. Most of this work has targeted the addictions and health-related behaviors. The series of articles in this issue highlight a rapidly accelerating recent trend: the application of MI to other problems typically seen in clinical practice. This introductory article describes MI, its core principles, treatment methods, and the variety of ways in which it has been employed. The 6 case reports in this issue are then described. They illustrate how MI can be employed with genera

OBJECTIVE: Psychotherapy has long been recommended as adjunctive to pharmacotherapy for bipolar disorder, but it is unclear which interventions are effective for which patients, over what intervals, and for what domains of outcome. This article reviews randomized trials of adjunctive psychotherapy for bipolar disorder.
METHOD: Eighteen trials of individual and group psychoeducation, systematic care, family therapy, interpersonal therapy, and cognitive-behavioral therapy are described. Relevant outcome variables include time to recovery, recurrence, duration of episodes, symptom severity, and …

Against expectation, the experience of setting up a psychotherapy service for adult epidermolysis bullosa (EB) patients at St Thomas' Hospital in London has been that take up is slow and wary. Prior indications from the wider team were that need is high. This may reflect differences in the experience of need on the part of patients and staff. Published research on the psychosocial aspects ofEB is minimal. This article proposes a type of genetic trauma particular to this patient group. It then examines some clinical psychodynamics with regard to the capacity of some severely affected patients t…

Research on psychotherapy for older adults with psychiatric disorders has demonstrated its effectiveness, although the majority of research has been conducted on major depression. Recent advances in extending this research to additional diagnostic categories and treatment settings are reviewed. Psychotherapy appears promising in the treatment of minor depression, dysthymia, anxiety disorders, depression with comorbid personality disorders, depression with comorbid cognitive impairment, and as an adjunctive treatment in psychotic disorders. Psychotherapy also has been successfully transported t…

OBJECTIVE: The authors compared a combination of clomipramine and psychodynamic psychotherapy with clomipramine alone in a randomized controlled trial among patients with major depression.
METHODS: Seventy-four patients between the ages of 20 and 65 years who were assigned to ten weeks of acute outpatient treatment for major depression were studied. Bipolar disorder, psychotic symptoms, severe substance dependence, organic disorder, past intolerance to clomipramine, and mental retardation were exclusion criteria.
RESULTS: Marked improvement was noted in both treatment groups. Combined treatm…

Traditionally, in psychodynamic forms of treatment, the patient's relationship with the therapist is central to eliciting and exploring the patient's characteristic ways of thinking, feeling and behaving. Research has increasingly demonstrated the significance of the therapeutic relationship to the treatment outcome. Since little has been written about the therapist-patient relationship in the treatment of pathological gambling, we shall describe what we have found to be some of the common and problematic transference and countertransference reactions and interactions. Both positive and negati

Studies of the effectiveness of psychotherapy and pharmacotherapy in the treatment of bulimia nervosa have been accumulating over the last decade. Only recently, however, have investigations been reported comparing the two modalities directly, assessing whether one might be more effective than the other, or whether a combination of the two approaches might be superior to either alone. This paper reviews the three combined treatment studies that have been published to date and discusses their implications for clinical practice and for future research.

The one-year supportive group-psychotherapy (3 groups with 8-9 patients) was carried out once-weekly by two therapists in 26 patients who met the DSM-III-R criteria for panic disorder and agoraphobia. Patients filled in various ratings at the beginning and at the end of the therapy and a visual analog scale before as well as after each session. Twelve months after the end of the group-therapy the Longitudinal Interval Follow-up Evaluation (LIFE) was carried out. During the group-psychotherapy panic sensations, anxiety, nervousness and fears as well as psychosocial disability decreased. Duratio…

Over the past several decades, both individual and family-oriented approaches to child therapy have been shown to be of moderate success in reducing children's psychological and behavioral problems. However, the data from comparative outcome research do not yet allow us to determine whether one approach is superior to the other in a broad sense; nor has the research addressed the more prudent question of whether there are specific conditions under which one approach may be preferable to the other. In this article, family factors related to child psychopathology are identified, and it is argued

Transsexuals, as a client group, are increasingly coming to the attention of mental health professionals. This paper offers a general discussion of gender dysphoria and sex-reassignment, including an examination of their psychosocial aspects. The role of psychotherapy in relation to this client group is explored, and reactions of the therapist to the transsexual are considered.

Depressives responding to initial treatment were maintained on amitriptyline for eight months, withdrawn double-blind to placebo after two months, or withdrawn overtly onto no medication. In each group half the patients received weekly psychotherapy and half were seen once monthly. Effects on symptom ratings were examined. Maintenance amitriptyline gave a significant advantage over early withdrawal in preventing symptom recrudescence. There were no differences between double-blind or overt withdrawal. There were no interactions between drug withdrawal and psychotherapy. Psychotherapy produced …

One hundred of 150 patients with non-psychotic functional psychiatric disorders were benefited by the use of LSD psychotherapy. The dosage of LSD employed was 25 to 2000 micrograms intramuscularly per session for from one to 10 sessions. On this regimen four patients became psychotic and required electroconvulsive therapy. None were permanently harmed.Indications for and contraindications to this form of treatment and a procedure involving a doctor and a nurse as co-therapists are discussed. In particular, LSD is considered to permit "perceptualization of the transference".LSD possibly extends…
