Using the new definition of intraamniotic infection - is there morbidity among the women left out?

BACKGROUND: Recently, ACOG released updated definitions for intraamniotic infection (IAI): maternal fever (≥38.0 °C) and ≥1 symptom (fetal tachycardia, maternal white blood cell count >15,000/mm3 or purulent discharge). Treatment was no longer recommended for women with fever <39.0 °C plus maternal tachycardia or fundal tenderness (previous criteria). The objective of this study was to compare postpartum infectious morbidity among women meeting previous criteria (but not the new IAI criteria) to women meeting new IAI criteria. METHODS: Retrospective cohort of women delivering vaginally at a s

Rediscovering Ignaz Philipp Semmelweis (1818-1865).

Ignaz Philipp Semmelweis was a Hungarian obstetrician who discovered the cause of puerperal or childbed fever (CBF) in 1847 when he was a 29-year-old Chief Resident ("first assistant") in the first clinic of the lying-in division of the Vienna General Hospital. Childbed fever was then the leading cause of maternal mortality, and so ravaged lying-in hospitals that they often had to be closed. The maternal mortality rate (MMR) from CBF at the first clinic where Semmelweis worked, and where only medical students were taught, was 3 times greater than at the second clinic, where only midwives were

Endometritis: Managing Persistent Post-Breeding Endometritis.

Endometritis was rated as the third most common medical problem encountered in adult horses in North America. It is the leading cause of subfertility in broodmares and is a major contributor to economic loss in the horse breeding industry, with pregnancy rates reported to be as low as 21% in mares with severe endometritis. Endometritis may be categorized as: endometrosis (chronic degenerative endometritis), acute, chronic, active, dormant, subclinical, clinical, and persistent post-breeding. These classifications are not mutually exclusive, and mares may change categories within breeding seaso

Approach to a lump in the breast: a regional perspective.

The clinical presentation, investigation and treatment of women with breast lumps are greatly influenced by geographical factors. These impact disease patterns and their presentation (e.g. socioeconomic/educational status and cultural/religious beliefs) and the availability of specialist health care personnel and facilities. The differential diagnoses of a breast lump include breast cancer, benign solid and cystic lumps and inflammatory conditions. The likelihood of a diagnosis relates more to geographical area, age, ethnic origin, family history of breast cancer, presence of high-risk patholo

Varied presentations of sporadic group A streptococcal bacteremia: clinical experience and attempt at classification.

The epidemiologic, clinical, and laboratory aspects of group A streptococcal bacteremia were studied in 33 patients seen at two urban hospitals in the Tel Aviv (Israel) area, over an 8-year period. Most patients (two-thirds) were female. Clinically significant bacteremia was observed in 26 patients, two of whom acquired their infection (puerperal sepsis) during hospitalization. A portal of entry, mainly cutaneous, was recognized in 61% of the patients, and a chronic underlying condition was observed in 69%. The case-fatality rate was 27%, with death occurring predominantly in patients admitted

Paeniclostridium sordellii-associated peripartum metritis in goats.

Paeniclostridium sordellii is involved in enteric and histotoxic infections in several animal species. In humans, P. sordellii has been linked to gynecological disease, an association not previously investigated in animals. To unveil a potential association of P. sordellii with veterinary reproductive disease, a retrospective search of the database of the California Animal Health and Food Safety Laboratory System (1990-2020) was conducted and identified 9 cases of goats with P. sordellii-associated metritis or endometritis that were confirmed by immunofluorescence antibody test and/or bacteria

[Puerperal streptococcal sepsis].

A series of 8 cases of puerperal sepsis caused by beta-haemolytic Streptococcus from group A is analysed, including two cases of death--of a mother and her newborn. Among the women in seven cases serotype 2 of beta-haemolytic Streptococcus from group A has been identified. The cases have been observed in the Delivery department as a result of the existing favorable conditions for the transmission of the infection. Most probably among these conditions are the negligence of the sterility of the obstetric service from the moment of admission in the ward as well as the possibility for personal con

[Semmelweis and his outstanding contribution to medicine: washing hands saves lives].

Ignaz Semmelweis was a Hungarian obstetrician who in the nineteenth century, preceding the discoveries of Pasteur and Lister, proposed the infectious etiology of puerperal sepsis. With a simple antiseptic procedure, he achieved marked reduction of the prevalence of this disease. However, he needed to fight against the reluctance of his colleagues who didn't accept his observations although they were, for the first time in the history of science, supported by statistical significance analysis. This report describes biographical data of this revolutionary physician and the circumstances of his s
