Q-Vax®, a whole cell formalin inactivated vaccine, is currently the only licensed Q fever vaccine for humans world-wide. Efficacy is high, although vaccine failures have been described for those vaccinated within the incubation of a naturally acquired infection. In Australia, it is widely used to prevent occupational acquisition of Q fever and is the mainstay for outbreak control. A retrospective review of all notified cases of acute Q fever to the Victorian department of health, 1993-2013, revealed 34 of 659 cases were previously vaccinated and 10 cases were positive on pre-vaccination screen…

Modern diagnostic methods enable clinicians to look beyond a diagnosis of chronic Q fever and discern whether patients instead have persistent focalized Coxiella burnetii infection(s). Use of these methods and development of criteria to define and treat such infections, especially cardiovascular infections, will improve the prognosis for patients previously thought to have chronic Q fever.

Q fever is an infectious zoonotic disease characterized by sudden fever, headache, and atypical pneumonia, caused by Coxiella burneti--an obligatory intracellular parasite. Based on phylogenetic analysis of the genes sequences, the classification was changed and C. burnetii species was included to the gamma subgroup of the proteobacteria, Legionellales order and Coxiellaceae family. This analysis showed more than 99% sequence similarity of 16SrRNA gene among the strains isolated in different regions of the world. Q fever is a widespread in the world zoonosis. Its main reservoir in the rural en…

Q fever is an acute febrile illness due to Coxiella burnetii. In the Balkans, Q fever in humans has been reported since World War II, and in countries such as Bulgaria the number of cases has increased since the early 1990s. We report an investigation of an outbreak in the town of Botevgrad, Western Bulgaria. Overall, 220 cases were identified between May 1 and June 9, 2004. Of the cases, 168 were from Botevgrad; the others were from neighbouring towns. This has been the largest outbreak in Bulgaria in the last 20 years. Q fever outbreaks in urban areas are not common. Flocks of sheep and goat…

The occurrence of Q fever in a population of Zadar region (2787 km2) which has five microregions (Zadar, Benkovac, Biograd, Obrovac and Pag microregion) is presented. In 1986, nine cases of Q fever were reported. Five patients were detected in Zadar community, one in Pag microregion and 3 in Biograd community and microregion, respectively. In 1989, two cases in Zadar and, in 1988 and 1991, one case in each year from the territory of Zadar were found. A total of 13 cases of Q fever were treated and reported. The personnel of the Zadar Veterinary Stations were not able to provide information sin…

Coxiella burnetii, the etiological agent of Q-fever has recently been isolated from sheep in southern Sweden. In this region 24-30% of sheep farmers have been exposed to the organism as shown by serological measurements. In veterinarians, another group with high risk of exposure to C. burnetii, about 12% have antibodies to the bacteria. The seropositive veterinarians are scattered all over the country. In two non-risk groups, draftees and hospital employees, 5-7% were found to be positive. This survey showed that Q-fever is a domestic disease which is spread throughout Sweden.

The first outbreak of Q fever in Uruguay occurred in 1956. The infection, which is caused by Coxiella burnetti, occurs in sheep, cattle, swine, and horses, but not in fowl or guinea pig. Most of the cases studied have been traced to cattle. Fourteen outbreaks of this disease were studied between 1975 and 1985. All of the 1,358 clinically suspected cases (814 serologically confirmed) reported in Uruguay occurred in workers at meat-processing plants. Diagnoses of cases and serologic surveys were made by use of complement fixation, capillary agglutination, and layer microagglutination techniques.

In the summer of 1981 29 patients living in Gwent, South Wales, had Q fever; 20 were diagnosed by routine clinical and laboratory services and 9 were discovered by active case-finding. The date of onset of symptoms spanned a 2-month period. The peak incidence was at the end of June. A case-control study showed an association with one area on the outskirts of Newport. Most patients had no direct contact with farms of farm animals and a common source of infection could not be identified. The most likely explanation of the outbreak was that farm vehicles had disseminated contaminated straw, manur…

An epidemiological study of Q fever infection in a Brisbane meatworks was undertaken. A retrospective 10-year study (1968 to 1977) revealed an average annual incidence of 1% for the nine years excluding 1969, while an outbreak with a 7.9% incidence occurred in 1969. No seasonal distribution was found. Results indicated that cattle form the major source of Coxiella burneti (infectious agent of Q fever) in the meatworks and that the highest risk of infection exists on the cattle slaughter floor. The 1969 outbreak showed a departure from the average pattern of infection in that the highest incide…
