PURPOSE: A tear of the triangular fibrocartilage complex (TFCC) is the most frequent soft tissue injury associated with fractures of the distal radius, and repair of the deep ligamentous portion of the TFCC is considered when the tear contributes to instability of the distal radioulnar joint (DRUJ). The purpose of this prospective cohort study was to identify predictors of DRUJ instability accompanying unstable distal radius fractures.
METHODS: Between 2002 and 2007, we prospectively treated 163 consecutive patients with unstable distal radius fractures with the volar locking plating system. …

Distal radius fractures are the most frequent lesions encountered during clinical practice. The treatment is controversial and still debated in the literature. For a correct management of these lesions many authors recently emphasised the importance of anatomical reduction, a stable fixation and early joint mobilisation. We report our experience in the daily management of these lesions. The fractures are evaluated considering fracture type, fracture reduction criteria, adequacy of reduction criteria and overall fracture stability. The best treatment option must be decided in accordance to the …

Volar locking plate fixation via open reduction and internal fixation is an increasingly accepted method for managing displaced distal radius fractures. Volar plating offers biomechanically stable fixation, allows early rehabilitation, and enables fixation of comminuted or osteopenic bone. The literature reporting complications of volar plate fixation is limited primarily to case reports and small case series. The surgeon must be mindful of potential soft-tissue, neurovascular, and osseous complications, such as extensor tendon and flexor tendon injury, flexor pollicis rupture, carpal tunnel s…

BACKGROUND: The treatment of unstable fractures of the distal radius remains controversial between nonoperative and operative management and between different methods of internal and external fixation. The aim of the study is to evaluate the early functional and radiologic outcomes of using Association for the Study of Osteosynthesis (AO) (Synthes) distal radius volar locking plate for unstable dorsally displaced distal radius fractures.
METHODS: Seventeen consecutive fractures in 16 patients were included in the study. The average age of the patients was 46 years. There were 11 women and 5 m…

The purpose of the current study was to examine the functional outcomes of C3.1 and C3.2 distal radius fractures. A trauma registry was used to identify patients with C3.1 and C3.2 distal radius fractures (n = 33). Clinical follow-up consisted of completion of the Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire and assignment of a Gartland and Werley Demerit Point System score. Average follow-up duration was 62 months. Mean DASH score was 14 points; mean Gartland and Werley score was 6 points. Results were 14 excellent, 12 good, 10 fair, and one poor. Patients regained an aver…

PURPOSE: Nonbridging external fixation is becoming popular for distal radius fractures, although its biomechanical characteristics have not been documented. This study evaluated the biomechanical characteristics of nonbridging external fixators for distal radius fractures.
METHODS: We tested 3 currently available nonbridging fixators (F-Wrist fixator, Hoffman II Compact, and Pennig Dynamic Wrist Fixator) and determined their relative stiffness under axial compression, torsion, and bending moments (dorsal, volar, radial, and ulnar aspects) using a uniform unstable distal radius fracture model.…

Wrist arthroscopy for the distal radius fractures is an effective adjunct to evaluate the reduction of intraarticular fragments and soft tissue injuries. In recent years, volar locking plate fixation has become popular, and arthroscopic procedures for distal radius fracture reduction have become problematic because vertical traction has to be both on and off during surgery. We developed a plate presetting arthroscopic reduction technique to simplify the combination of plating and arthroscopy. The fracture was reduced, and anatomic alignment was regained under an image intensifier, and then the…

UNLABELLED: The relationship between radiographic and functional outcomes in older patients with distal radius fractures is controversial. We explored this relationship by assessing the influence of radiographic displacement and fracture comminution on the functional outcomes of these fractures. We also asked whether operative intervention and demographic factors (age, gender, duration of followup) influenced outcome. We examined 53 patients older than 55 years with distal radius fractures with various functional assessments: range of motion (ROM) and strength measurements, three subjective su…

Dorsal plating emerged as an effective treatment for dorsally displaced distal radius fractures in the late 1980s. In addition to some mechanical advantages, this method provided a clear view of the articular surface and the ability to restore the anatomy. However, because of the frequent occurrence of extensor tendon complications in the early designs of dorsal plates, the volar approach gained favor for repairing these types of fractures. Recent improvements in dorsal plating designs have yielded increasingly thinner, precontoured plates with rounded edges and low-profile flush screw heads. …

Distal radius fractures are common injuries. Multiple classification systems have highlighted the evolution of the understanding of distal radius fractures. Understanding the classifications of distal radius fractures is important in identifying the important aspects that affect their outcome. Surgical indications of distal radius fractures can be divided into the following categories: patient factors, fracture reduction, fracture stability, and the presence of associated injuries.

Between 1997 and 2001, 58 patients received dorsal plating for comminuted distal radius fractures. In 8 of these patients, subsequent collapse led to palmar flexion deformity, loss of rotation, and hardware prominence. In retrospectively reviewing this subgroup's range of motion, radiographic volar tilt, and complications, including tenosynovitis and extensor tendon rupture, we found that (1) with both palmar and dorsal comminution of distal radius fractures, dorsal plating may not prevent palmar cortex collapse; (2) deformity of the distal radius fragment causes palmar displacement of the rad…

Elevated pressures within the carpal canal are known to occur after distal radius fractures. Controversy exists regarding prophylactic carpal tunnel release after open reduction with internal fixation of distal radius fractures. The purpose of this study was to determine the tissue pressures within the carpal canal after volar plating of distal radius fractures. This study was a prospective, observational, IRB approved research study. Ten consecutive patients undergoing volar plating of distal radius fractures were enrolled. After the distal radius fractures were reduced and fixed with volar p…

The fracture management principles of anatomic or near anatomic reduction, fracture stabilization, minimal operative trauma, and early joint motion are paramount in man-aging unstable distal radial fractures. The operative approach and plate selection should correlate with the fracture configuration. Plates have the advantages of providing secure fixation throughout the entire healing process without protruding wires or pins and allowing early and intensive forearm, wrist, and digital exercises. Disadvantages include additional operative trauma, including fragment devascularization; some addit

External fixation is a versatile and useful tool for management of complex fractures. There is little to choose between the various types of commercially available fixators, and it is important to use one that allows the surgeon adequate versatility and follows sound biomechanical principles. Ligamentotaxis can be used effectively to reduce the most difficult fractures; however, over-distraction and prolonged traction are harmful and should be avoided. Certain types of fractures do not respond to treatment with ligamentotaxis alone and require adjunctive treatment, such as limited internal fix

Sixty patients with unilateral distal radius fractures were managed conservatively. Clinical assessment included objective and subjective evaluations of the outcome of treatment. Radiographic evaluation included fracture classification using the AO/ASIF system and measurement of volar tilt, radial inclination and radial height shortening at the end of treatment. Clinical signs of carpal tunnel syndrome were confirmed electrodiagnostically in 12 patients (20%) during the recovery period. Each patient had electrodiagnostic studies of both upper limbs performed to confirm the diagnosis. The mean …

Internal fixation of distal radius fractures often shows the problem of secondary dislocation due to dorsal comminution and osteoporosis. Although dorsal plating systems provide good stabilization, the intraoperative control of reduction is difficult in the comminuted area with high incidence for the need of cancellous bone graft. Occurrence of extensor tendon complications including tendonitis and rupture is not uncommon. The use of fixed angle devices by a palmar approach has demonstrated the advantage of better visualization and control at the fracture side. The subchondrale support of the …

Stable internal fixation and early motion has not been routinely available for distal radius fractures. Difficulties with the dorsal approach discourage surgeons from internally fixing the most common fracture types. The introduction of a new volar plate with subchondral support fixation allows the treatment of most distal radius fractures with stable internal fixation and early motion while avoiding the complications inherent in the dorsal approach.

Thirteen patients with malunited fractures of the distal radius developed symptoms of pain and instability of the midcarpal joint. In six cases, a recurrent voluntary midcarpal subluxation was also present during ulnar deviation. These symptoms were first noticed several weeks, and at times several months, after all immobilization for the treatment of the original fracture had been discontinued. We believe that the loss of the normal palmar tilt of the distal articular surface of the radius prepositions the carpus in a dorsal collapse alignment, which enables this instability to develop.
