We examined partner notification among syphilitic pregnant women in Nairobi. At delivery, 377 women were found to be rapid plasma reagin (RPR) reactive. Data were available for 94% of the partners of women who were tested during pregnancy; over 67% of the partners had received syphilis treatment while 23% had not sought treatment mainly because they felt healthy. Six per cent of the women had not informed their partners as they feared blame and/or violence. Following intraperitoneal sensitisation of rats with rat serum containing reaginic antibody, intravenous injection of blue dye and intraperitoneal challenge with antigen caused a release of histamine, slow-reacting substance of anaphylaxis (SRS-A) and dye into their peritoneal fluids. The times taken to reach peak concentrations after challenge were less than 2 min for histamine and between 5 and 10 min for SRS-A, whilst concentrations of dye were still increasing after 2 1/2 h. The amounts of histamine released by antigen were sufficient to account for about 60% of this extravasation of dye.

A 47-year-old man with HIV infection presented 10 years after initial secondary syphilis diagnosis and treatment for routine follow-up. His HIV was well controlled on antiretroviral therapy. Rapid plasma reagin was 1:1, and TP-PA was reactive. Physical examination revealed a wide pulse pressure, a systolic murmur, and an early diastolic decrescendo murmur. Echocardiogram revealed moderate to severe aortic regurgitation, and subsequent computed tomography angiogram showed a 6.8-cm fusiform aneurysm of the proximal ascending aorta.
