[Emphasis on perioperative ocular surface management in cataract surgery].

Cataract surgery has fully evolved from traditional vision-restoring surgery to refractive cataract surgery, highlighting the growing clinical importance of maintaining perioperative ocular surface microenvironment homeostasis. Based on the"Chinese expert consensus on prevention and treatment of dry eye disease during perioperative period of cataract surgery (2021)" and with further in-depth exploration, this article elaborates on four aspects: the relationship between the imbalance of the ocular surface microenvironment homeostasis and dry eye disease during the perioperative period of catara

Ocular Causes for Headache.

Ocular causes of headaches include headaches associated with refractive error (HARE), convergence insufficiency (CI), and accommodative spasm (AS). HARE is more severe in patients with a high amount of refractive error. CI or AS patients can have diplopia and strabismus. Both CI and AS can be associated with head trauma or other systemic disorders. Headaches due to ocular causes are frontally localized occurring near the end of the day and are associated with increased amount of near work. HARE is treated with glasses while CI or AS may need other therapies such as prism, eye drops, surgery, o

Origins of Refractive Errors: Environmental and Genetic Factors.

Refractive errors are the product of a mismatch between the axial length of the eye and its optical power, creating blurred vision. Uncorrected refractive errors are the second leading cause of worldwide blindness. One refractive error currently attracting significant scientific interest is myopia, mostly owing to the recent rise in its prevalence worldwide and associated ocular disease burden. This increase in myopia prevalence has also been rapid, suggesting environmental influences in addition to any genetic influences on eye growth. This review defines refractive errors, describes their pr

Accuracy and repeatability of autorefraction in young adults: a comparison of cycloplegic and non-cycloplegic methods.

CLINICAL RELEVANCE: Cycloplegic refraction remains crucial in young adults, where accommodative activity can obscure accurate refractive status. Autorefractor repeatability is essential for precise optical correction and refractive error categorisation in clinical and research settings. BACKGROUND: Cycloplegic autorefraction is recommended for individuals up to 20 years to prevent hyperopia underestimation and myopia overestimation. This study evaluated differences between non-cycloplegic and cycloplegic spherical equivalent refraction following cyclopentolate hydrochloride (cycloplegic agent

[Dynamic refraction in schoolchildren].

A technique for dynamic refraction measurement in natural conditions is proposed using open-field refractometer. Dynamic refractometry was studied in 101 schoolchildren with emmetropia and different degree ametropia. Based on the data obtained accommodation input for 5, 2.5, 1, 0.5 and 0.33 m was countered. It was found that as the object becomes closer spheric power and astigmatism degree increase and axis changes. Accommodation input in patients with hyperopia was close to calculated values, in emmetropia decrease of input by mean 49% was found and in myopia--by 74% compared to calculated va

The role of the lens in refractive development of the eye: animal models of ametropia.

Research with young mammals and chicks has shown that the visual environment can affect the refractive development of the eye by enhancing or slowing axial eye growth, but the effect on the refractive components of the eye, the lens and cornea, are less clear. A review of the literature indicates that the lens is minimally affected, if at all, and results vary depending on whether the lens is studied in an isolated state or with the accommodative apparatus intact. Research has shown that the development of myopia or hyperopia in young chicks alters lens focal length and magnitude of the accomm

Racial variations in vision.

This study determined the distributions of uncorrected visual acuity and of refractive error in representative groups of Australian Aborigines and Australians of European origin aged 20-30 years. The methodology used in this study and its verification are described in detail. As a group, the Aborigines have significantly better visual acuity than the Europeans. This was true for both monocular and binocular vision. Some Aborigines have acuities below the previous postulated threshold levels. Aborigines as a group also have the previous postulated threshold levels. Aborigines as a group also ha

[Biometrics and aphakia (author's transl)].

The combination of long axis measurement (ultrasonic) and corneal measurement is well adapted to determination of refraction in aphakia. In a group of 35 persons the results of this method were compared with subjective refraction measurement and skiascopy. "Ultrasonic" refraction was less than one dioptre different from subjective refraction in 62% of cases. We found that ultrasonic-determined axis length was consistently 0.4 mm shorter than the axis length calculated from subjective refraction measurements. The clinical application and accuracy of the method are described. With the help of a

IMI Accommodation and Binocular Vision in Myopia Development and Progression.

The role of accommodation in myopia development and progression has been debated for decades. More recently, the understanding of the mechanisms involved in accommodation and the consequent alterations in ocular parameters has expanded. This International Myopia Institute white paper reviews the variations in ocular parameters that occur with accommodation and the mechanisms involved in accommodation and myopia development and progression. Convergence is synergistically linked with accommodation and the impact of this on myopia has also been critiqued. Specific topics reviewed included accommo

Pre- and postoperative refraction after cataract extraction with implantation of standard power IOL.

The pre- and postoperative refraction results are reported in 99 patients receiving an anterior chamber lens of standard 19.0 D power after intracapsular cataract extraction. The mean refraction after the operation was -0.76 D, SD 2.13. Apart from eight patients with suspected lenticular myopia, 5% of the patients ended up with a refraction that differed more than +/- 5D from the preoperative value. This variation was due to variation in the power of the biological lens removed at surgery, the mean value of which was 22.8 D, SD 3.3. As a measure of the aniseikonia induced at surgery, the ocula

Refractive surgical approaches to keratoconus: A systematic review and network meta-analysis.

Advancements in diagnostic methods and surgical techniques for keratoconus (KC) have increased non-invasive treatment options. Successful surgical planning for KC involves a combination of clinical science, empirical evidence, and surgical expertise. Assessment of disease progression is crucial, and halting the progression should be the focus if it is progressive. While surgeons used to rely on experience alone to decide the surgical method, comparing the network of primary factors, such as visual acuity, across studies can help them choose the most appropriate treatments for each patient and

Automated and subjective refraction with monofocal, multifocal, and EDOF intraocular lenses: review.

Automated refraction (Scheiner principle) is universally used to start a visual examination. Although the results are reliable in eyes implanted with monofocal intraocular lenses (IOLs), they may be less precise with multifocal (mIOL) or extended depth-of-focus (EDOF) IOLs and can even indicate a refractive error that does not clinically exist. Autorefractor results with monofocal, multifocal, and EDOF IOLs were investigated through literature search analyzing the papers reporting the difference between automated and clinical refraction. The average difference ranged between -0.50 diopter (D)
