Various machine learning techniques have been developed for keratoconus detection and refractive surgery screening. These techniques utilize inputs from a range of corneal imaging devices and are built with automated decision trees, support vector machines, and various types of neural networks. In general, these techniques demonstrate very good differentiation of normal and keratoconic eyes, as well as good differentiation of normal and form fruste keratoconus. However, it is difficult to directly compare these studies, as keratoconus represents a wide spectrum of disease. More importantly, no

Image-based modeling is a popular approach to perform patient-specific biomechanical simulations. One constraint of this technique is that the shape of soft tissues acquired in-vivo is deformed by the physiological loads. Accurate simulations require determining the existing stress in the tissues or their stress-free configurations. This process is time consuming, which is a limitation to the dissemination of numerical planning solutions to clinical practice. In this study, we propose a method to determine the stress-free configuration of soft tissues using a Gaussian process (GP) regression.

Traditionally, keratoconus has been managed with glasses when mild, contact lenses when moderate and keratoplasty when severe. When cornea-based refractive surgery was first developed it appeared to be a useful option for keratoconus until reports of post-operative progressive ectasia emerged and thus keratoconus was considered a contraindication to refractive surgery. However, improvements in older techniques and the development of new techniques mean that there are now several viable options to avoid keratoplasty in contact lens-intolerant patients. This review discusses the risks and benefi

Descemet membrane detachment (DMD) is a rare complication that occurs mainly after cataract surgery. We report the case of a 55-years-old woman who underwent refractive lensectomy. The Day-1 postoperative examination was unremarkable, the Day-7 examination showed corneal oedema related to DMD, which was confirmed by Visante(®) OCT. An intracameral injection of SF6 expansive gas allowed the reapplication of the membrane and the receding of corneal oedema. Through this observation, we discuss the pathogenesis of this uncommon entity and the contribution of Visante(®) OCT.

PURPOSE OF REVIEW: There are several options for correcting astigmatism at the time of cataract surgery. They include incision placement on the steep axis of corneal astigmatism, single or paired peripheral corneal relaxing incisions, and toric intraocular lens implantation. The aim of this review is to update readers on advances reported during the last year.
RECENT FINDINGS: Phacoemulsification incision placement on the steep corneal axis corrects small amounts of astigmatism and is sufficient for most eyes. Peripheral corneal relaxing incisions correct greater amounts of astigmatism. They

Errors of refraction of the eye can be corrected not only by means of spectacles, contact lenses or intra-ocular lenses, but also by operative procedures on the cornea. At present, correction of myopia is accomplished with radial keratotomy (RK), a procedure that achieves the best results in the range up to - 6.0 diopters. However, on account of its side effects, in particular poor predictability, the method is no true alternative to the use of optical aids. Epikeratoplasty done to correct hyperopia and aphakia, is even more imprecise, and should only be employed in exceptional cases. Laser su

Implantation of intraocular lens with Iris-fixation is a safe, efficient and predictable surgical procedure, which empowers the refractive surgeon with singular capabilities. Among their advantages are the reversibility, preservation of accommodation and a broad spectrum of ametropic correction. This lens also appears to be a valid option, with a favorable complication rate, for the treatment of aphakic eyes without capsular support. This article is a review of iris-fixated intraocular lenses and considers their principal indications, complications, and outcomes.

PURPOSE: To report two cases of late ocular trauma resulting in laceration of corneal flaps 7 and 4 years (cases 1 and 2, respectively) after LASIK.
METHODS: A 49-year-old man sustained laceration of the flap and interface foreign body debris from construction material in the right eye 7 years after uneventful LASIK (case 1). A 33-year-old man had partial dislocation of the flap 4 years after LASIK in the left eye (case 2) due to a penetrating wood chip accident. Surgical procedures were performed to remove the foreign bodies and reposition the flaps.
RESULTS: In case 1, postoperative uncorr

Recent advances in surgical techniques and intraocular lens implants have transformed cataract surgery into a true refractive procedure, enabling many patients to forgo optical correction. "Premium" IOL's offer multiple options for correcting refractive errors, including astigmatism, and addressing the effects of presbyopia. While cataract surgery is often beneficial for glaucoma patients, glaucomatous optic neuropathy can impair visual quality and impact the outcomes of premium IOL's, particularly multifocal IOL's. This concise review provides practical recommendations for the use of premium

BACKGROUND: Residual refractive errors, especially high-grade astigmatism after penetrating keratoplasty, often lead to a significant loss of vision. If high anismetropia could not be corrected with glasses or contact lenses, different kinds of surgical procedures are available for visual rehabilitation (intraocular lens exchange, astigmatic keratotomy, Excimer laser treatment, intrastromal corneal ring segment implantation and additive intraocular lens implantation). Toric add-on IOLs are especially designed for sulcus implantation and correcting high astigmatism in pseudophakic eyes. All tor

UNLABELLED: Surgical correction of refractive errors is becoming increasingly popular. In the 1990s, the excimer laser revolutionized the field of corneal refractive surgery with PRK and LASIK, and lately refractive lenticule extraction (ReLEx) of intracorneal tissue, using only a femtosecond laser, has become possible. Two new procedures were developed, ReLEx flex (FLEX) and ReLEx smile (SMILE). Until this thesis, only a few long-term studies of PRK with a relatively limited number of patients had been published; therefore, this thesis intended to retrospectively evaluate long-term outcomes a
