Contraceptive strategies for young women in the 21st century.

Safe, effective and affordable contraceptives have been available for a few decades, yet, in many countries, the struggle for reproductive rights continues. Children, still, are forced to give birth to children in many nations. In most industrialized countries, where contraceptive counselling and abortions have been options, fertility rates have reached all time lows. Effective contraception improves health and well-being as well and may be used for conditions other than birth control. Young girls often initially take oral contraceptives primarily to reduce menstrual pain and blood loss; they …

The marked dextro-dominance causes accentuated morphophysiological variations in the female reproductive organs of the bat Molossus molossus.

Molossus molossus is an insectivorous molossid bat that is important in the control of nocturnal insects. It is the nominal and the most representative species of the family. However, there are few studies about its reproduction. Thus, this study aimed to evaluate variations of its female reproductive organs during the different reproductive phases. Twenty adult females, divided into four sample groups (non-reproductive, early and advanced pregnancy and lactation), were submitted to morphological and morphometric analyses. Results show that the female reproductive system of M. molossus is comp

A relationship between particular reproductive endocrine disorders and the laterality of epileptiform discharges in women with epilepsy.

Cerebral limbic structures modulate the endocrine function of the hypothalamopituitary axis in both animals and humans. In an investigation of 30 women who had complex partial seizures with unilateral temporal lobe epileptiform discharges and reproductive endocrine disorders, there was a significant difference between the EEG laterality distributions associated with polycystic ovarian syndrome (PCO) and those associated with hypogonadotropic hypogonadism (HH). There was a strong predominance of left-sided discharges (15 versus one) with PCO, and right-sided discharges (12 versus two) with HH.

Intersectoral interventions: integration for impact on preterm birth.

PROGRESS: The last two decades have seen a growing focus on intersectoral interventions to improve maternal and newborn health and well-being outcomes, as reflected in efforts to achieve the Millennium Development Goals (MDGs) and advance the Sustainable Development Goals (SDGs). Preterm births are linked to cross-sectoral determinants that affect health outcomes and human capital across the life-course, necessitating an intersectoral approach that addresses these multifaceted challenges. PROGRAMMATIC PRIORITIES: Recognizing that social, biological and economic determinants significantly infl

Should older and postmenopausal women have access to assisted reproductive technology?

In vitro fertilisation and other assisted reproductive technologies (ART) now enable many women to have children, who would otherwise have remained childless. The most obvious application for these technologies is to help physically infertile, but otherwise healthy young women to have children. However, increasingly, other groups are seeking access to ART to conceive, raising ethical questions about who should be allowed to use these technologies to bear children. In particular, the question of access to ART by lesbian couples and single groups has roused considerable ethical, legal and public

Reproductive endocrine disorders in women with partial seizures of temporal lobe origin.

Of 50 consecutive women with partial seizures of temporal lobe origin (temporal lobe epilepsy [TLE]) evaluated for reproductive dysfunction, 28 had menstrual problems. Of those, 19 had reproductive endocrine disorders. Polycystic ovarian syndrome and hypogonadotropic hypogonadism occurred significantly more often in women with TLE than in the general female population. Polycystic ovarian syndrome was associated with predominantly left-sided lateralization of interictal epileptic discharges; hypogonadotropic hypogonadism was more commonly found with right-sided discharges. Hyposexuality occurre

Intimate partner violence, reproductive health outcomes, and responsiveness among women living with disabilities in Southern Ethiopia: a qualitative study.

BACKGROUND: Intimate partner violence (IPV) is a major public health concern that disproportionately affects women with disabilities. While studies have identified factors associated with IPV, there is limited documentation on its impact among women with disabilities in Ethiopia. This gap hinders the development of targeted interventions. This study aimed to explore the impact of IPV on women with disabilities, focusing on the intersection of gender, disability, and reproductive health outcomes, as well as the responsiveness of services in Southern Ethiopia. METHODS: A community-based ethnogr

The long-term reproductive health consequences of female genital cutting in rural Gambia: a community-based survey.

This paper examines the association between traditional practices of female genital cutting (FGC) and adult women's reproductive morbidity in rural Gambia. In 1999, we conducted a cross-sectional community survey of 1348 women aged 15-54 years, to estimate the prevalence of reproductive morbidity on the basis of women's reports, a gynaecological examination and laboratory analysis of specimens. Descriptive statistics and logistic regression were used to compare the prevalence of each morbidity between cut and uncut women adjusting for possible confounders. A total of 1157 women consented to gy

Short-Term Effects of a School-Based Comprehensive Sexuality Education Intervention Among Very Young Adolescents in Three Urban Indonesian Settings: A Quasi-Experimental Study.

PURPOSE: To evaluate the effects of a comprehensive sexuality education (CSE) intervention on short-term psychosocial outcomes related to healthy sexuality among very young adolescents in urban Indonesia. METHODS: A quasi-experimental study was conducted between 2018 and 2021 with students aged 10-14 years at 18 schools in Indonesia (Lampung, Denpasar, Semarang). Three schools per site were purposefully selected to receive the SEmangaT duniA RemajA intervention, a two-year, rights-based teacher-led CSE intervention delivered in classrooms (or online after the 2020 COVID-19 outbreak); and matc

Patients Need Doctors with Consciences.

"Conscience clauses" define conscience as "religious beliefs" or "moral convictions," and they come up, therefore, usually in relation to women's reproductive rights. This article argues that conscience is better understood as a feeling of integrity, rightness, and self, and that we need it especially now, as huge corporations take over health care. After an illustrative story, the author reviews the history of patients' rights and also the health-care consumer movement, which introduced the idea that health care is a commodity, and the doctor, therefore, simply a tradesman, whose duty is to p…

Climate change and sexual and reproductive health: A mixed methods scoping review of possible mechanisms.

This scoping review synthesized empirical evidence on the relationships and possible pathways through which climate change could impact sexual and reproductive health and rights (SRHR) in low- and middle-income countries (LMICs). Eligible sources had to investigate climate change and SRHR using any study design. Two independent reviewers conducted screening; data extraction captured exposures, outcomes, associations/relationships, and possible mediators and contextual moderators. From 40,503 database-identified sources, we included 256 peer-reviewed publications across 108 LMICs. We identified

Barriers to access and use of sexual reproductive health and human immunodeficiency virus services among adolescent girls and young women in Maputo City, Mozambique.

Adolescent girls and young women (AGYW) in Mozambique face a high risk of sexually transmitted infections (STIs) and unintended pregnancies, yet face substantial barriers to sexual and reproductive health and rights (SRHR) and HIV services. This qualitative study explored AGYW experiences in accessing SRHR and HIV care at Zimpeto and Primeiro de Junho Health Facilities (HF) in Maputo between June 12 and 28, 2023, using semi-structured interviews. Data were analyzed in NVivo 14 and interpreted using the socio-ecological model. Barriers were mainly institutional and individual. Institutional cha

A Constructivist Vision of the First-Trimester Abortion Experience.

How might the abortion experience look in a world without the existing regulatory constraints? This paper critically assesses the evidence about how a high-quality abortion experience might be achieved in the first trimester. There would need to be positive obligations on states in pursuance of women's reproductive rights. The onus would be on states and state actors to justify interferences and constraints upon a woman's right to terminate in the first trimester of her pregnancy. In this vision, abortion is person-centered and normalized as far as possible. High-quality information about abor…

Implementing and expanding safe abortion care: An international comparative case study of six countries.

We conducted a comparative case study-based investigation of health sector strategies that were useful in expanding or establishing new abortion services. We selected geographically diverse countries from across the human development index if they had implemented new abortion laws, or changed interpretations of existing laws or policies, within the past 15 years (Colombia, Ethiopia, Ghana, Portugal, South Africa, and Uruguay). Factors facilitating the expansion of services include use of a public health frame, situating abortion as one component of a comprehensive reproductive health package,

Emergency contraception - a human rights issue.

Emergency contraception is the only resource that women can use to avoid becoming pregnant after having sexual intercourse without contraceptive protection. It could be a powerful means to prevent unwanted pregnancies and their devastating consequences for women's health, social wellbeing and life project, and for the unwanted child, if all people had ample access to good quality information, education and services for sexual and reproductive health. In spite of the preventive medicine value of emergency contraception, conservative sectors oppose its availability, appealing to moral values tha

Are women with disabilities less likely to utilize essential maternal and reproductive health services?-A secondary analysis of Pakistan Demographic Health Survey.

BACKGROUND: People with disabilities deal with widespread exclusion from healthcare services, including sexual and reproductive health (SRH) rights. Studies analyzing the relationship between disability and key SRH utilization outcomes have often reported mixed findings. In Pakistan, very little to no literature is available on this topic, therefore we aim to determine inequalities in the utilization of essential maternal and reproductive health services between women with and without disabilities in Pakistan. METHODS: This was a secondary analysis of Pakistan Demographic Health Survey 2017-1

Understanding the intersection of perinatal HIV and sexual and reproductive health: A scoping review protocol.

With the increasing prevalence of adolescents and young adults living with perinatally acquired HIV (AYA-PHIV) globally, there is a pressing need for a holistic management approach that addresses the sexual and reproductive health and rights (SRHR) of this population. The proposed scoping review aims to systematically map the existing literature on perinatal HIV and SRHR, thereby enhancing our understanding of the global response to these issues and informing future research. The review will be conducted through a comprehensive electronic search across various databases. To facilitate the scre

The ripple effect: impacts of climate change on menstrual health and paths to resilience.

Girls and women face greater threats and severe ramifications from climate change, with studies consistently finding that women experience more health risks from climate change than men do. Climate change endangers girls and women's sexual and reproductive health and rights, including their menstrual health and hygiene practices. However, menstrual health and hygiene is rarely discussed in the context of climate change. We scoped the existing evidence to describe the interlinkages between climate change and menstrual health and hygiene, and outline services that anticipate, respond to, cope wi
