Pontine tuberculoma presenting with horizontal gaze palsy.

A 38-year-old immunocompetent man presented with a horizontal supranuclear gaze palsy as the only neurologic manifestation of a pontine tuberculoma. Although a biopsy of the brain lesion was not performed, it was attributed to tuberculosis because of chest x-ray evidence. The patient was given empirical anti-tuberculous therapy. After one month, the gaze palsy had fully recovered and repeat MRI showed a decrease in the size of the lesion. This is the first reported case of supranuclear gaze palsy without diplopia as a manifestation of a tuberculous brain stem lesion.

Striking differences in transmission of corticospinal excitation to upper limb motoneurons in two primate species.

There is considerable debate as to the relative importance, for cortical control of upper limb movements, of direct cortico-motoneuronal (CM) versus indirect, propriospinal transmission of corticospinal excitation to cervical motoneurons. In the cat, which has no CM connections, a significant proportion of corticospinal excitation reaches forelimb motoneurons via a system of C(3)-C(4) propriospinal neurons (PN). In contrast, in the macaque monkey most motoneurons receive direct CM connections, and, under the same experimental conditions as in the cat, there is little evidence for PN transmissi

Clinical and electroencephalographic evidence for sites of origin of seizures with diffuse electrodecremental pattern.

A diffuse electrodecremental ictal pattern (DEP) has been associated with tonic seizures and, less often, with other forms of epilepsy and has been considered to reflect a generalized seizure disorder of diffuse cortical or subcortical (brainstem) origin. In some seizures associated with DEP, however, focal ictal manifestations have been observed. We reviewed the records of all patients admitted to our seizure monitoring unit for 3 years and detected 39 patients with seizures associated with DEP. In 23 of 39 patients, clinical ictal behaviors resembled seizures of unilateral supero/mesiofronta

The effect of systemic cocaine on the responses to noxious stimuli and spontaneous activity of medial bulboreticular projection neurons.

The effect of antinociceptive doses of cocaine (25 mg/kg, i.p.) on unit responses to noxious somatic stimuli and spontaneous activity of antidromically identified projection neurons in the medial medullary reticular formation (MRF) was studied in the rat. Thirty-three antidromically activated neurons were recorded from the medullary raphe, gigantocellular, or paragigantocellular nuclei in an acute anaesthetized preparation; 25 cells projected to the spinal cord and 8 neurons had rostral projections through the medial forebrain bundle (n = 4) or the medial thalamus (n = 4). After cocaine admini…

New brain stem and bone marrow abnormalities in victims of sudden infant death syndrome.

The study looked for new abnormalities in 31 victims of sudden infant death syndrome (SIDS). The focus was on respiratory control centers in the brain stem, because some SIDS victims have had abnormalities in respiratory control during sleep. A major respiratory control area (lateral reticular nucleus) of the medulla was hypomyelinated in 9 of the 31 SIDS victims. In a second study, the size of the 12th cranial nerve nucleus and its neuronal composition were analyzed because this nucleus regulates tongue movements, and the tongue has been postulated to help obstruct the airway in some SIDS vic

Arginine-vasopressin inhibits centrally induced pressor responses by involving hippocampal mechanisms.

Administration of arginine-vasopressin (AVP) or prolyl-leucyl-glycinamide (PLG) into a lateral cerebral ventricle reduced the magnitude of systolic blood pressure increase (pressor response) induced by electrical stimulation of the mesencephalic reticular formation (MRF) in urethane-anesthetized rats. Bilateral destruction of the dorsal hippocampus prevented the action of AVP on the pressor response. However, the effect of PLG was only slightly reduced by hippocampal lesion. Microinjection of AVP in the dentate area of the dorsal hippocampus mimicked the action of intracerebroventricularly adm…

Inhibition and excitation of thoracic spinoreticular neurons by electrical stimulation of vagal afferent nerves.

Neurons in spinal segments T1-T3 of cats projecting to the region of the nucleus reticularis gigantocellularis were tested for their responses to electrical stimulation of the vagus nerve. The effects of vagal efferent activity were blocked with methylatropine, thereby limiting vagal input to the effects of afferent fibers. Seventeen spinoreticular (SR) neurons were inhibited by vagal stimulation; one was excited; four were both inhibited and excited, and nine were unaffected. Half of the SR neurons were not spontaneously active; they were excited by somatic stimuli for 20- to 30-s periods in

Attenuation by arginine- and desglycinamide-lysine-vasopressin of a centrally evoked pressor response.

Arginine8-vasopressin (AVP) administered in a lateral cerebral ventricle attenuated the pressor response evoked by electrical stimulation of the mesencephalic reticular formation (MRF) in urethane-anesthetized rats. The blockade appeared to be dose-dependent in the range of 3-25 ng. The maximal effect was a 40% reduction of the pressor response. Desglycinamide9-lysine8-vasopressin (DG-LVP) exerted a similar effect on the pressor response. Both AVP and DG-LVP failed to affect basal blood pressure upon intraventricular administration. It is suggested that centrally acting neuropeptides related t…

[Supranuclear progressive paralysis (or oculo-facial-cervical dystonia)].

The authors describe 14 personal cases of progressive supranuclear palsy, 8 of them including a neuropathological study. The analysis of this material confirms the characteristics of this nosological entity, which is now well individualized amongst the apparently idiopathic degnerative diseases of the CNS. The clinical picture was characterized in all cases by a supranuclear opthalmoplegia, a pseudo-bulbar syndrome, an axial dystonia and a 'subcortical' dementia of a particular type. The average duration of the disease was 4 years for the 12 patients followed until death; the first symptoms ap

Clinical approach to brainstem lesions.

The brainstem consists of the midbrain, pons, and medulla. The cerebellum is attached to the dorsal surface of the pons and upper medulla. The brainstem contains 9 of the 12 cranial nerves and is crossed by ascending, descending, and cerebellar pathways and their nuclei as well as the reticular formation. Numerous and rare crossed brainstem syndromes have been described in recent years, many of them without clinical significance. The aim of this article is to provide a brief clinical description of some conditions affecting the brainstem.

[Electroencephalographic analysis of the central action of benzonal].

Chronic experiments on conscious rabbits demonstrated that benzonal suppresses the spontaneous bioelectrical activity of the motor division of the cortex of large hemispheres while failing to affect the spontaneous biorhythms of the nonspecific nuclei of the thalamus, caudate nucleus, dorsal hippocamp and the basolateral nuclei of the tonsil. The drug decreased the electroexcitability of the motor cortex and caudate nucleus, exerted a mild depressant action on the reticular formation of the midbrain, tonsil, and hippocamp and had no effect on the excitability of nonspecific nuclei of the thala…

The spino-bulbar-cerebellar pathway: Activation of neurons projecting to the lateral reticular nucleus in the rat in response to noxious mechanical stimuli.

It is now well established that the cerebellum receives input from nociceptors which may serve to adjust motor programmes in response to pain and injury. In this study, we investigated the possibility that spinoreticular neurons (SRT) which project to a pre-cerebellar nucleus, the lateral reticular nucleus (LRt), respond to noxious mechanical stimulation. Seven adult male rats received stereotaxic injections of the b subunit of cholera toxin in the LRt. Following a 5 day interval, animals were anesthetised with urethane and a noxious mechanical stimulus was applied to the right hind paw. Anima

Recovery of an injured corticoreticular pathway via transcallosal fibers in a patient with intracerebral hemorrhage.

BACKGROUND: Several studies have reported on injury of the corticoreticular pathway (CRP), however, little is known about the mechanism for recovery of an injured CRP. We report on a patient with intracerebral hemorrhage (ICH) who showed recovery of an injured CRP via transcallosal fibers, which was demonstrated by diffusion tensor tractography (DTT). CASE PRESENTATION: A 67-year-old man presented with complete paralysis (Medical Research Council: 0/5) of the left extremities at the onset of a right putaminal hemorrhage. At six weeks after onset, he presented with more severe weakness of prox

Contribution of synapses in the medial supramammillary nucleus to the frequency of hippocampal theta rhythm in freely moving rats.

We have previously shown that in urethane-anesthetized rats the frequency of rhythmical slow activity in the hippocampus ("theta") is controlled by the medial supramammillary nucleus (SuM). In particular, injections of procaine into SuM in urethane-anesthetized animals reduce the frequency of theta. However, it has been reported that, in freely moving animals, lesions of SuM do not affect theta. The present experiments were designed to resolve this anomaly. Injections of procaine or chlordiazepoxide into SuM in urethane-anesthetized animals reduced the frequency of theta elicited by reticular

Rapid improvement of visuoperceptive functions by picoTesla range magnetic fields in patients with Parkinson's disease.

Impairment in perceptual motor or visuospatial tasks is among the most frequently encountered abnormality in neuropsychological testing of patients with Parkinson's disease, being present in up to 90% of cases. Visuoperceptive deficits can result from cortical and subcortical lesions involving the right hemisphere, thalamus, and basal ganglia and are thought to reflect a defect in attentional-arousal mechanisms induced by lesions that interrupt a cortical-limbic-reticular activating loop. Clinically, the presence of visuoperceptive impairment may not be noted by Parkinsonian patients but may c
