Idiopathic retroperitoneal fibrosis (IRF) is a rare urological disease, for which many pathogenic theories have been proposed. The authors report a series of 13 cases of IRF in order to evaluate the clinical, diagnostic, laboratory, therapeutic and prognostic aspects. By analogy with desmoid tumours, tamoxifen has been used in the treatment of idiopathic retroperitoneal fibrosis. The authors report the 6th case of retroperitoneal fibrosis treated by tamoxifen alone, which had not recurred 11 months after stopping treatment.

Idiopathic retroperitoneal fibrosis is exceedingly uncommon in childhood and its etiology is uncertain. Support for an immunological basis for the disease is given by a report of a 14-year-old girl with severe retroperitoneal fibrosis causing progressive azotemia in whom azathioprine and prednisolone were used successfully. This case supports the efficacy of immunotherapy in the treatment of idiopathic retroperitoneal fibrosis.

Two cases of pseudocysts associated with retroperitoneal fibrosis and ureteral obstruction are described. Chemical analysis in the first case identified the retroperitoneal cyst as chylous in origin. In the second case the cystic area within the retroperitoneal fibrosis disappeared following treatment with steroids.

The main goal of surgery for retroperitoneal fibrosis is to free the ureters from sclerotic tissue and pressure them from further extension of the disease; the omentum may be used as an efficient barrier against recurrence of fibrosis. Of 36 patients with idiopathic retroperitoneal fibrosis treated surgically, 8 have died and 26 have suffered 67 post-operative complications during 1 to 24 years of follow-up. Four patients died as a consequence of hypertension. Hypertension and thromboembolism accounted for 49% of the post-operative complications in 69% of the patients who developed complications. Attention to these 2 factors is at least as important as prevention of recurrent obstructive uropathy.

A patient with renal failure and light-chain proteinuria showed roentgenographic evidence of retroperitoneal fibrosis on further examination. Diffuse hyperglobulinemia, low levels of C3 and C4, and elevated C1q binding were noted. Biopsy specimens of the kidney and retroperitoneal tissue disclosed a dense pleomorphic cellular infiltrate with a preponderance of plasma cells. Renal function improved slowly after ureterolysis and institution of prednisone therapy. The case emphasizes the immunologic features of the early phase of idiopathic retroperitoneal fibrosis.

A case is reported in which extensive mesocolonic and retroperitoneal granulomatous and fibrotic disease were apparently due to occult diverticulitis. The patient later had a good result from aggressive but limited operation for obstructions of the ureter and colon.

A case of retroperitoneal fibrosis involving the testicle is presented. The widespread extent of the retroperitoneal fibrosis syndrome is reviewed briefly. The literature contains little information concerning the aetiology of testicular involvement in the disease process, which generally affects the para-aortic tissues. The physical signs mimic testicular neoplasm. A common embryological derivation of the tissues affected is postulated.
