Peripheral nerve blocks can be used routinely in an outpatient setting. The innervation of the face and the technique we use to perform nerve blocks are reviewed. We present four cases of skin cancer and one of rhinophyma in which nerve blocks were used to minimize the discomfort associated with local anesthesia.

A 54-year-old man presented with symmetric, bulbous, nodular enlargement of the nasal tip and alae nasi. At first inspection, the changes appeared to be consistent with rhinophyma, but a biopsy specimen showed noncaseating granulomata that were consistent with sarcoidosis. His laboratory, radiographic, and pulmonary function studies were consistent with systemic involvement.

A space-occupying lesion, which had been growing on the external surface of the nose of a 57-year-old man for 5 years was considered to most likely be an angiofibroma after surgical removal and histological examination, but the final clinical diagnosis was clearly a rhinophyma. This discrepancy is relevant for surgical treatment, because the vessel-rich tumor tended to substantial intraoperative bleeding.

Rhinophyma is an uncommon disease of the nose characterized by irregular skin thickening and nodular deformation. The extensive growth causing 'whisky nose' is due to hyperplasia of the sebaceous glands and the surrounding connective tissue. Other facial regions may concomitantly be affected. We present a case of extensive gnathophyma accompanying minor lesions of the nose. Surgical treatment led to an excellent cosmetic result.

The cutaneous application of lasers today includes more selective and less damaging devices. Carbon dioxide, neodymium:yttrium-aluminum-garnet, potassium titanyl phosphate, argon, and yellow lasers are most prevalent in treating cutaneous lesions. Specific techniques in skin resurfacing, keloid excision, rhinophyma, actinic cheilitis ablation, and excision of superficial cutaneous tumors are discussed. Proper management of cutaneous vascular lesions is also presented.

An 83-year-old woman presented with primary multifocal cutaneous B-cell lymphoma, presenting as discrete nodules on the right pinna and nail-bed of the left middle finger, diffuse swelling and erythema of several other nail-beds of the fingers and toes, with associated pincer nail deformity and rhinophyma. Because of the involvement of several sites not amenable to radiotherapy, she was treated with oral cyclical chlorambucil with good result.

Two patients with nasal skin defects resulting from excision of rhinophyma and multiple angiofibromas were treated with artificial dermis followed by full-thickness skin grafts taken from the postauricular region. The secondary skin grafts took completely in both patients, and the postoperative results were excellent. Although a two-stage operation is required, application of artificial dermis prior to full-thickness skin grafting is a reliable method for resurfacing the nose.

Rosacea is a common chronic inflammatory dermatosis of the face, clinically characterized by erythema, telangiectasia, papules, pustules, and rhinophyma. In January 2022, the updated guideline on rosacea was published. Groundbreaking innovations include the new clinical classification according to phenotypes, extended diagnostic and therapeutic recommendations for ocular rosacea and implications of the gut microbiome on rosacea. Furthermore, the guideline encompasses a new chapter on the psychosocial aspects of rosacea and detailed recommendations for approved and off-label therapies.
