Risk assessment and management in hand and facial tissue transplantation.

PURPOSE: Since hand and facial tissue transplantation are new treatments, risk data must be derived from early reports of the few cases done to date combined with extrapolations from other procedures with similar risks. This manuscript summarizes data from both sources including eight separate studies that measure the real and perceived risks associated with hand and facial tissue transplantation. METHODS: Real Risks: Several large clinical studies describing risk data from kidney transplant recipients (10-years experience) and risk data from 49 human hand transplants in 33 recipients (>10 ye

Community management of anti-malarials in Africa and iatrogenic risk.

Distribution of anti-malarials at the community level is one of the interventions recommended to reduce mortality from febrile illnesses. Inappropriate treatment of fever with anti-malarials may result in missed diagnosis and delays in appropriate treatments including consideration of other illnesses than malaria. We report the case of an 8-year-old black girl receiving prophylaxis with sulfadoxine-pyrimethamine from the caretaker of the community during her holidays in Ivory Coast. A persistent fever suspected to be due to malaria was treated inappropriately with atovaquone-proguanil and then

Assessment and management of high-risk pregnancies in women with thrombophilia.

Pregnancy is an acquired hypercoagulable state which can lead to gestational vascular complications especially in the presence of other prothrombotic risk factors. The use of anticoagulation for prevention of pregnancy complications in women with thrombophilia is becoming more frequent. Efficacy and safety issues of therapeutic and prophylactic regimens use and monitoring of LMWH therapy in pregnancy are discussed in this review. In addition, a scoring system for women with thrombophilia is proposed which includes severity of pregnancy outcomes, thrombotic history and type of thrombophilia. Va

Management of patients with high gastrointestinal risk on antiplatelet therapy.

Increasing use of antiplatelet therapies is associated with increasing GI complications, such as ulceration and GI bleeding. Identification of high-risk patients and, in such patients, incorporation of strategies to reduce their GI risk would be clinically prudent. After assessment and treatment of H pylori in patients with prior ulcer or GI bleeding histories, further reduction in GI risk in other high-risk patients who require antiplatelet agents is primarily accomplished by prescribing drugs that when coadministered with antiplatelet agents protect against mucosal ulceration, primarily prot

Coronary myocardial bridge constitutes a risk: But how to manage it?

Myocardial bridges may cause angina pectoris, myocardial infarction, life threatening arrhythmias and even sudden cardiac death but most of them are harmless. Although the exact management is not well known, beta blockers seem to be the first choice. Stenting is controversial and one must think "twice" before stenting the bridged coronary artery.

An alternative management for high-risk patients with rectal prolapse.

INDICATION: The management of rectal prolapse in presence of high-risk comorbidities can be challenging. We report the use of an anal plug as a noninvasive alternative in patients with contraindication of surgery. METHOD: This small device opens up in about 30 s to fit the contour of the lower rectum avoiding faecal leakage or in our case, the rectal prolapse. Although the plug is poorly tolerated in some cases, this device may be used up to 12 h per day and no adverse effects have been reported. COMPARISON WITH OTHER METHODS: Nonsurgical alternatives described in the literature, such as scle

Inpatient management for a shortened cervix: who is really at risk?

OBJECTIVE: The objective of the study was to identify risk factors associated with spontaneous preterm delivery (SPD) within 2 weeks of admission in patients with a shortened cervix. STUDY DESIGN: We reviewed records of patients hospitalized in 2003-2004 with a cervix of 25 mm or less at less than 32 weeks' gestation. The primary outcome was SPD 2 weeks or less of admission. RESULTS: Sixty-six patients met inclusion criteria. Twelve delivered at 2 weeks or less of admission. There were no differences in maternal and gestational age at admission, history of SPD, and rate of multifetal pregnan

Perineal hernia after proctectomy: prevalence, risks, and management.

PURPOSE: Perineal hernias are infrequent complications of abdominoperineal operations with estimated historic prevalences (from the era where the perineal wound was left open) ranging from 0.6 to 7 percent. The purpose of this study was to identify the modern prevalence of postoperative perineal hernias, factors that may contribute to their development, and examine the methods of repair. METHODS: The Mayo Clinic patient database (1990-2000) was interrogated for the following data identifiers: incisional hernia, perineal hernia, abdominoperineal resection, proctocolectomy, and partial or total

Risk counseling and management in patients with lobular carcinoma in situ.

BACKGROUND: The understanding of lobular carcinoma in situ (LCIS) has evolved since it was first described. LCIS once was thought to be a premalignant condition, but now it is considered a marker for increased risk for developing invasive breast cancer. We evaluated patient perception of risk, counseling, and subsequent management. METHODS: A community cancer registry of 3,605 cases of breast cancer was reviewed. Fifty-five (1.5%) patients with LCIS as their sole diagnosis were identified and these patients were sent a questionnaire. RESULTS: Forty of 55 patients completed the questionnaire

Intensive chemotherapy in patients with lymphoma. Management of the risk of hyperuricemia.

Intensive chemotherapy with stem cell autograft is a well-established salvage treatment for relapsed/refractory lymphoma patients aged less than 65 years and it is also an effective treatment option for high-risk patients at diagnosis. Clinical applicability of autograft has been greatly amplified by the use of peripheral blood progenitor cells (PBPC), whose administration is simple and feasible, and results in lower toxicity. In addition, the development of several prophylactic measures preventing extrahemopoietic toxicities has markedly improved the feasibility and tolerability of the approa

Atypia in the assessment of breast cancer risk: implications for management.

Proliferative disease accounts for as much as one-third of all biopsies for benign disease and 5-10% of proliferative lesions show atypia ductal or lobular hyperplasia. Nearly 40% of women with a family history of breast cancer and atypical hyperplasia subsequently develop breast cancer. A quantitative model developed by Gail and colleagues estimates the probability of developing breast cancer over time. Risk factors in the model include current age, ages at menarche and first live birth, number of previous biopsies, the presence of cellular atypia, and the number of first-degree relatives wit

Risk factors and management of dehiscent wounds in implant dentistry.

Successful implant dentistry is based on adequate training, proper selection of patients, careful surgery, preplanned restorations, and postoperative and supportive care. Although implant therapy is highly successful and predictable, it is not without possible early and/or late complications. The aim of this article is to highlight the importance of a relatively neglected subject of dehiscent wounds in the dental implant literature. This article covers the definitions of wound healing and wound dehiscence and some of the common risk factors related to suture line opening as an immediate postop

The caries balance: the basis for caries management by risk assessment.

Dental caries progression or reversal depends upon the balance between demineralization and remineralization. The 'Caries Balance' is determined by the relative weight of the sums of pathological factors and protective factors. Minimally invasive dentistry aims at the least possible removal of enamel or dentin, including reducing pathological factors and enhancing remineralization to avoid any removal of hard tissues. A structured caries risk assessment should be carried out based upon the concept of the caries balance. Following the risk assessment a treatment plan is devised which leads to t

Management of fever in neutropenic patients with different risks of complications.

Risk stratification of febrile neutropenic patients can have important implications in terms of management. The first prospectively validated risk scoring system was developed in 1992. A subsequent scoring system was developed in 2000, in which a score of < or =21 predicts a <5% risk for severe complications. Oral combination therapy in an ambulatory or home care setting is acceptable for low-risk patients. Hospital admission is mandatory for high-risk patients. Intravenous monotherapy can be given if neutropenia is anticipated to be of short duration; it is also acceptable if neutropenia is e

Genetics and the management of women at high risk for breast cancer.

It is estimated that 5%-10% of all breast cancers in women are associated with hereditary susceptibility due to mutations in autosomal dominant genes, such as BRCA1 and BRCA2, p53, pTEN, and STK11/LKB1. Another 15%-20% of female breast cancers occur in women with a family history but without an apparent autosomal dominant inheritance pattern, and are probably due to other genetic factors with environmental influence. Approximately 7%-10% of ovarian cancers occur in women with hereditary susceptibility, primarily secondary to mutations in BRCA1 and BRCA2, with smaller contributions from mutatio

Hyperphosphataemia as a cardiovascular risk factor -- how to manage the problem.

Hyperphosphataemia is a frequent and important cardiovascular risk factor in patients with chronic kidney disease (CKD). High phosphate levels may influence vascular calcifications by two separate mechanisms: by worsening secondary hyperparathyroidism, which in turn facilitates calcification, and by promoting calcium phosphate deposition in pre-formed endothelial plaques and in the arterial wall. Recent studies have shown that hyperphosphataemia induces the proliferation and differentiation of endothelial vascular cells into osteoblast-like cells, promoting vascular calcification. High phospha

[Update on levofloxacin in the management of acute sinusitis with risk of complications].

THE DIAGNOSIS OF ACUTE BACTERIAL MAXILLARY SINUSITIS: Is based on at least two of three major criteria: sinus pain, unilateral signs, increasingly voluminous and purulent rhinorrhea. Minor criteria can also be retained for diagnosis if they persist for three days. THE NEED FOR ANTIBIOTIC THERAPY: In this diagnostic context is undeniable, similar to the situation after failure of symptomatic treatment or complication. Likewise for unilateral maxillary sinusitis due to homolateral infection of the superior dental archade or for frontal, ethmoidal, or sphemoïdal sinusities. THE MOST FREQUENTLY CA

Risk factors for and management of post-transplantation cardiovascular disease.

The mortality rates due to cardiovascular disease (CVD) in transplant recipients are greater than in the general population. CVD is a major cause of both graft loss and patient death in renal transplant recipients, and improving cardiovascular health in transplant recipients will presumably help to extend both patient and graft survival. Further studies are needed to better evaluate the effectiveness of risk modification on subsequent CVD morbidity and mortality. There is no reason to consider risk factors for CVD such as hyperlipidaemia, hypertension and diabetes mellitus in transplant recipi
