Based on various signs and symptoms, the National Rosacea Society (NRS) Expert Committee has divided the syndrome of rosacea into 4 major subtypes: erythematotelangiectatic, papulopustular (inflammatory), phymatous, and ocular. Although the fundamental pathogenesis of rosacea remains unknown, inflammation is a central process in this disorder. Recent evidence suggests that this inflammation is associated with the generation of reactive oxygen species (ROS) that are released by inflammatory cells such as neutrophils. In vitro studies suggest that certain core therapies for rosacea, including metronidazole and the tetracyclines, show antioxidant effects, and this may be one aspect of their mechanism of action.

DESIGN: Retrospective case-series evaluation of children younger than 13 years with steroid rosacea seen over an 8-year period (1991-1998).

The hair follicle mites Demodex folliculorum and Demodex brevis and their role in the pathogenesis of rosacea have been the subject of much debate in the past. We studied the prevalence of Demodex mites in facial skin biopsies obtained from 80 patients with rosacea, 40 with facial eczematous eruption and 40 with lupus erythematosus discoides. The paper describes clinical features, concepts in the pathogenesis, and therapeutical aspects of rosacea. Particular attention is paid to recent experiences on topical therapy with metronidazole. A Norwegian double-blind, multi-centre clinical trial compares metronidazole 1% cream applied twice daily with placebo cream in the treatment of 97 patients with rosacea. Forty patients with rosacea were treated topically with 0.75% metronidazole gel in a randomized split-face double-blind paired-comparison trial. With twice-daily applications, 36.7, 48.5, and 65.1 mean percent reductions in total papules and pustules were noted over baseline at 3, 6, and 9 weeks, respectively, on the side of the face receiving metronidazole therapy. To determine whether tear pH in ocular rosacea was alkaline or acidic compared to normal, I studied tear pH measured by pH electrode or paper in 12 patients with rosacea without ocular manifestations, 23 patients with mild ocular manifestations, and nine patients with severe ocular manifestations. There was no diagnostically useful difference in tear pH between any of these groups and normal controls. Four patients are described in whom rosacea occurred on the face, and on the scalp in patches of baldness. Histopathological examination of the specimens taken from the scalp showed changes of rosacea and basophilic degeneration of collagen. In two patients direct immunofluorescence studies were performed; in one of these the presence of IgG deposits at the dermo-epidermal junction was demonstrated. These observations confirm the influence of external factors in the pathogenesis of rosacea.
