A polarization-sensitive swept-source optical coherence tomography system (central wavelength: 1,310 nm; A-line rate: 20 kHz) was developed to evaluate the three-dimensional structure of the anterior eye segment with the phase retardation associated with the anterior segment birefringence of the eyes. Evaluation of normal eyes and an eye with necrotizing scleritis was performed. In the sclera of the normal eyes, a striking polarization change was observed in the cumulative phase retardation images and the boundary of the sclera could be readily detected. In the eye with necrotizing scleritis, …

PURPOSE: To describe the occurrence of nodular scleritis in a patient developing tubulointerstitial nephritis and uveitis (TINU) syndrome.
DESIGN: Observational case report.
METHOD: The case notes of a 57-year-old female presenting with recurrent nodular scleritis, who later developed interstitial nephritis with bilateral uveitis, were reviewed.
RESULTS: Common and established causes of scleritis were not present in an adult who developed the TINU syndrome a decade later.
CONCLUSIONS: Although scleritis has not been reported as a tubulointerstitial nephritis-associated ocular inflammation,…

Fungal scleritis is a rare entity. A 50-year-old patient with culture proven Aureobasidium pullulans corneal ulcer underwent therapeutic keratoplasty. He developed scleritis 5 days following surgery. Although the patient had symptomatic improvement after antifungal therapy, surgical debridement, and cryotherapy, visual improvement was only marginal. Aureobasidium pullulans should be looked for as a cause for keratomycosis and scleromycosis, especially in tropical countries.

AIM: To report cases of scleral necrosis after simple pterygium excision in which adjunctive treatment was not used.
METHODS: We reviewed four patients who presented with scleral melt after pterygium excision without the use of adjunctive treatment in the form of beta irradiation, mitomycin C, or thiotepa. Each patient was thoroughly investigated to exclude underlying disease.
RESULTS: Certain similarities were found between our patients with pterygium melt and cases of surgically induced necrotising scleritis including the location of melt, associated inflammation, and its response to stero…

AIMS: To assess the clinical pictures, possible pathogenesis, management, and therapy of patients with infectious scleritis associated with multifocal scleral abscesses following pterygium excision.
METHODS: The records of patients with infectious scleritis after pterygium excision who developed multifocal scleral abscesses presenting from 1988 to the end of 1995 were reviewed. Early culture of abscesses was performed, and topical, systemic antimicrobials, or both were given to all patients. Fourteen eyes were operated on in addition to antimicrobial treatment.
RESULTS: The initial culture r…

PURPOSE: To describe a case of recurrent nodular scleritis that was apparently caused by reactivation of a varicella zoster virus infection.
METHODS: Case report. Immunohistochemistry and polymerase chain reaction were used to detect viral antigen and DNA in the biopsy specimen of inflamed sclera of a patient with a history of recurrent nodular scleritis.

Necrotizing scleritis may occur following ocular surgery, most commonly after cataract extraction. This complication developed in a 60-year-old woman following strabismus surgery for a gaze palsy and sixth-nerve paresis following a stroke. Although an autoimmune process is present in many patients with necrotizing scleritis, none was detected in this patient. Inflammation was controlled with topical and systemic corticosteroids and ibuprofen. Good visual acuity was preserved, and improved ocular alignment was achieved. Transient myopia, not previously reported in necrotizing scleritis, was obs…

Necrotizing scleritis with inflammation of the right eye developed after bilateral eye muscle surgery for thyroid ophthalmopathy. Debilitating pain, delay in onset, and involvement of the sclera distinguish this condition from anterior segment ischemia. The surgery may have acted as a nonspecific trigger in an eye at risk for scleritis. Necrotizing scleritis has occurred infrequently after other types of eye surgery but, to our knowledge, has not been previously reported as a complication of eye muscle surgery.

Light and electron microscopy were used to examine tissue excised during surgery from eight patients with advanced destructive scleral disease. These comprised two cases of scleromalacia perforans, three cases of anterior necrotising scleritis alone or in conjunction with other systemic diseases, and three cases in which scleritis developed following ocular surgery. It was not possible to distinguish between these three categories by histological or cytopathological criteria. All showed extensive granulomatous infiltration of the conjunctiva, episclera, and sclera by plasma cells and lymphocyt…

A case of bilateral necrotizing scleritis due to Aspergillus oryzae is reported. The patient was a former addict of intravenous narcotics treated five years previously for meningitis due to the same organism. A seeding focus in the thoracic spine was eventually found. The patient responded well to combined local and systemic therapy with amphotericin B, flucytosine, and natamycin. This represents, to the best of our knowledge, both the first reported case of ocular disease due to this species of Aspergillus and of isolated scleral, nonintraocular involvement in endogenous oculomycosis.

PURPOSE: To evaluate the effectiveness and safety of upadacitinib in reducing corticosteroid dependence and controlling relapse rate in patients with refractory noninfectious scleritis.
METHODS: A retrospective real-world study. Patients with refractory noninfectious scleritis treated with upadacitinib between January and December 2025 were included. Medical records were retrospectively reviewed. The primary outcome was sustained clinical remission, defined as absence of active scleral inflammation maintained for at least 2 consecutive visits separated by a minimum interval of 28 days. Second…

BACKGROUND: Posterior scleritis (PS) is a rare phenotype of scleritis. Comprehensive epidemiological studies on PS in children are limited. We aimed to report on its clinical and imaging features in one of the largest pediatric series to date.
METHODS: Retrospective review of medical files.
RESULTS: Included were 12 children (7 girls, mean±SD age at diagnosis 13.75 ± 3.3 years,18 eyes, mean follow-up ±SD of 46.7 ± 45.9 months). All patients presented with eye pain followed by headache. Papillitis was the most common presenting sign (16 eyes, 88.9%). Seven patients (58%) showed laboratory mar…

PURPOSE: To report two patients who presented with scleritis associated with tubulointerstitial nephritis and uveitis syndrome (TINU).
RESULTS: A 13-year-old male initially presenting with unilateral panuveitis with scleritis in the fellow eye was ultimately diagnosed with TINU. A 33-year-old male previously diagnosed with TINU presented with recurrent bilateral episcleritis as well as bilateral scleritis eight years after his first episode of bilateral anterior uveitis. Both patients' scleritis responded to local corticosteroids and systemic immunosuppression.
CONCLUSIONS: Scleritis should …

PURPOSE: To report a patient with bilateral scleritis who was initially treated for infectious scleritis, and subsequently diagnosed as granulomatosis with polyangiitis (GPA).
METHOD: Retrospective chart review.
RESULT: A 48-year-old female with a known history of diabetes presented with pain, redness, and blurring of vision in the right eye. She was diagnosed as scleritis with retinal detachment and underwent vitrectomy, with silicone oil tamponade and intravitreal injections of antibiotics, and antifungal agents. She presented with active scleritis with additional multiple pus points. Seve…
