Impact of frailty and physiological severity on early and medium-term surgical outcomes after open abdomen.

BACKGROUND: Open abdomen (OA), mainly indicated for sepsis and second-look surgery, may entail severe complications that translate into high morbidity and mortality. Following the World Society of Emergency Surgery 2018 Guidelines leads to an improvement in surgical outcomes. However, how baseline status and fragility might determine patient´s morbidity and survival has not been described yet. METHODS: Retrospective analysis of a prospectively maintained registry of patients undergoing emergency laparotomy with temporary abdominal closure (January 2020-December 2022) and ≥12-month follow-up. …

Conservative surgery for giant cell tumors of the sacrum. The role of cryosurgery as a supplement to curettage and partial excision.

BACKGROUND: Giant cell tumors (GCTs) of the sacrum are a difficult clinical problem. Wide excision (total sacrectomy) is associated with high morbidity and pelvic/spinal instability. Curettage with or without supplemental radiotherapy is associated with a high recurrence rate. In view of the proven effectiveness of cryosurgery as an adjunct to curettage for extremity GCT, cryosurgery was used for treatment of GCTs of the sacrum. METHODS: Seven patients with GCTs of the sacrum were treated at our institution by conservative surgery from 1973 to 1992. Four patients presented with recurrent tumo

Clinical outcomes of second-look arthroscopic evaluation after anterior cruciate ligament augmentation: comparison with single- and double-bundle reconstruction.

We report the clinical outcome and findings at second-look arthroscopy of 216 patients (mean age 25 years (11 to 58)) who underwent anterior cruciate ligament (ACL) reconstruction or augmentation. There were 73 single-bundle ACL augmentations (44 female, 29 male), 82 double-bundle ACL reconstructions (35 female, 47 male), and 61 single-bundle ACL reconstructions (34 female, 27 male). In 94 of the 216 patients, proprioceptive function of the knee was evaluated before and 12 months after surgery using the threshold to detect passive motion test. Second-look arthroscopy showed significantly bette

Laparoscopic Myomectomy: Sonographic Follow-Up and Second-Look Laparoscopy for the Evaluation of a New Technique.

Between 1988 and 1993, 100 women underwent laparoscopic myomectomy in the Department of Obstetrics and Gynecology of the University of Ulm. Indications varied, although most myomectomies were done for fertility enhancement or preservation. Closure of the incision, especially with an open uterine cavity, was done in two or three layers. Several technical limitations specific to laparoscopic surgery impair complete and satisfactory closure of the myomectomy incision. In this study we evaluate the new technique by sonographic control and second-look laparoscopy. In 22 cases second-look laparoscop

Spontaneous transanal evisceration of the small bowel through perforation of the sigmoid colon: a case report and new surgical approach.

INTRODUCTION AND IMPORTANCE: Transanal evisceration of the small intestine is a rare surgical emergency that requires immediate intervention. Since its initial description in 1827, only three cases of evisceration secondary to perforation of the sigmoid colon have been documented. This case report describes the fourth case worldwide. PRESENTATION OF CASE: An 85-year-old woman presented with transanal small bowel evisceration after straining during defecation. Examination revealed prolapsed, congested but viable small bowel loops through the anal canal, without uterine or rectal prolapse. Emer

Loss of Motor Evoked Potential in the Exposure Stage of Scoliosis Surgery in a Patient with Kyphoscoliosis.

Somatosensory-evoked potential and motor-evoked potential (MEP) are used during complex spinal surgeries to ensure the neurological integrity of the patient. Here, we report a case of a patient with type 3 thoracolumbar scoliosis whose surgery was stopped due to loss of signal in the neuromonitoring over the right lower limb in the exposure stage, before instrumentation and manipulation. The patient had right lower limb weakness and hence the surgery was abandoned. The same abnormality was present when we tried surgery for the second time. Surgery was aborted and the potentials recovered and t

Polytrauma--pathophysiology and management principles.

BACKGROUND: Multiple injury results in a complex pathophysiological and immunological response. Depending on the individual injury pattern, the time elapsed after injury, and the systemic "danger response", the surgical treatment has to be modified. OBJECTIVES: This overview provides new insights in the pathophysiology of the early danger response after polytrauma and outlines the main resulting consequences for surgical management. RESULTS: First, synchronically to the clinical assessment, life-saving procedures need to be performed rapidly, such as control of massive intra-thoracic or abdo

Laparoscopic management of surgical complications after a recent laparotomy.

BACKGROUND: The use of laparoscopy in the scarred abdomen is now well established. However, recent laparotomy and the presence of a fresh abdominal wound usually preclude laparoscopic intervention. Thus, early postlaparotomy complications, which mandate surgical interventions, are usually treated by a second laparotomy. We report our experience with the use of laparoscopy for the treatment of postoperative complications, after open abdominal procedures. METHODS: Fourteen patients were operated for a variety of conditions, and postoperative complications, such as bowel obstruction, intraabdomi

Recurrent neuroblastoma: the role of CT and alternative imaging tests.

One hundred twelve CT scans of 52 patients who were receiving treatment for neuroblastoma were reviewed for accuracy, and findings were correlated with data obtained from other imaging tests, physical examinations, laboratory tests, biopsies, surgery, and long-term clinical follow-up. CT was the most sensitive imaging test for tumor recurrence (85% detection rate), and it was also the most versatile in the ability to define recurrent disease in the retroperitoneum, liver, cranium, mediastinum, lymph nodes, and skeleton. All 30 tumor recurrences were detected by the combination of CT, bone-marr

Gastric necrosis in newborns: a report of 11 cases.

Eleven neonates ranging in gestational age from 34 to 40 weeks presented with gastric necrosis. The 4 full-term neonates showed sudden-onset hemorrage and "coffee-ground" vomiting; in the 7 premature babies the initial clinical finding was abdominal distention. The criteria for diagnosis were: perinatal distress in prematures and transient neonatal respiratory distress in full-term babies. Radiographic evidence of gastric distention was typical and preceded clinical signs of hematemesis and gastric perforation. Surgery was performed in 8 patients; 3 received medical treatment. At surgery 1 tot
