Superior Semicircular Canal Dehiscence Outcomes in a Consecutive Series of 229 Surgical Repairs With Middle Cranial Fossa Craniotomy.

BACKGROUND: Superior semicircular canal dehiscence (SSCD) is the appearance of a third mobile window between the middle fossa and the superior semicircular canal. Surgical management is indicated in patients with persistent and debilitating symptoms. The purpose of this study was to evaluate the association between preoperative variables that may impact postoperative symptomatic resolution. METHODS: A single-institution retrospective analysis was performed on patients who were surgically treated for SSCD. Patients were divided to different cohorts based on unilateral or bilateral nature of th

Video Head Impulse Test (vHIT) Findings in Patients With Superior Semicircular Canal Dehiscence: A Case-Control Study.

OBJECTIVE: To explore the usefulness of the responses of video head impulse testing (vHIT) in assessing symptomatic patients with superior s … group (all comparisons: P > .05). CONCLUSION: SSCD can affect the vestibular responses from all 3 semicircular canals; not necessarily the superior one. Similar responses were found in a control group of normal subjects. Although the use of vHIT in the assessment of SSCD is not diagnosis-specific, it can still help with identifying the impact of surgery on all cana …

A Cohort Study Comparing Importance of Clinical Factors in Determining Diagnosis and Treatment for Superior Semicircular Canal Dehiscence Syndrome.

OBJECTIVE: To determine which clinical factors have the strongest impact on determining diagnosis and decision for surgical repair for superior semicircular canal dehiscence syndrome (SCDS). PATIENTS: A total of 81 patients presented with a suspicion for SCDS between October 2017 and October 2018. 38 patients ultimately were diagnosed with SCDS, and 28 chose surgical repair. METHODS: Clinical factors of subjective patient symptoms (autophony, sensitivity to loud sounds, vertigo induced by loud sounds, dizziness, hearing their own heartbeats or other visceral organs) and audiogram findings (l

Case Report: Preservation of Otolithic Function After Triple Semicircular Canal Occlusion in a Patient With Intractable Ménière Disease.

Operative measures are considered when medical treatment fails to control vertigo in patients with intractable Ménière disease. The present report discusses a case in which triple semicircular canal occlusion was performed in a 30-year-old female patient who responded poorly to previously performed endolymphatic sac surgery. Her vestibular and auditory functions were evaluated both before and after surgery. Class A control of vertigo was achieved during the 76-month postoperative follow-up period. Ocular and cervical vestibular evoked myogenic potentials could be elicited before and after surg

Incidence of intraoperative hearing loss during middle cranial fossa approach for repair of superior semicircular canal dehiscence.

INTRODUCTION: Superior semicircular canal dehiscence is a rare inner ear disorder characterized by an abnormal third opening between the superior semicircular canal and middle fossa. Symptoms include amplification of internal sounds, aural fullness, tinnitus, hearing loss, autophony, sound-induced vertigo (Tullio phenomenon), pressure-induced vertigo (Hennebert sign), disequilibrium, nystagmus, oscillopsia, and headache. While no cure exists for SSCD, surgical treatment has proven to effectively minimize these symptoms. This study reviewed brainstem auditory evoked potentials (BAEPs) that were

The influence of the subarcuate artery in the superior semicircular canal dehiscence and its frequency on stillbirths: illustrative cases and systematic review.

UNLABELLED: Literature describes that on the 25th gestational week the labyrinth is fully formed and with adult size. However, recent studies have shown that the cranial and labyrinth development continues until 3 years of age. OBJECTIVES: To demonstrate through tomographic study the frequency of semicircular canal dehiscence on nine specimens of stillbirths between 32 and 40 weeks and, through literature review, present another possible etiology for its cause. METHODS: Tomographic study of the temporal bone of 9 specimens of stillbirths between 32 and 40 weeks. RESULTS: A frequency of 88.8

Intracochlear schwannoma : Tumor removal via subtotal cochleoectomy and partial cochlear reconstruction with preservation of semicircular canal function.

We report about a patient, who in 2005, as a 37-year-old, experienced a sudden, mild to moderate, mid-frequency sensorineural hearing loss in the right ear, along with tinnitus and mild dizziness. MRI of the temporal bone revealed a very small (1 mm) enhancing lesion in the second turn of the right cochlea after injection of contrast medium. Hearing gradually deteriorated, with complete hearing loss in 2015. At this time, an MRI scan showed a lesion completely filling the cochlea. The tumor was removed through a subtotal cochleoectomy. The cochlea was partially reconstructed with cartilage and

Upper third molar internal structural organization and semicircular canal morphology in Plio-Pleistocene South African cercopithecoids.

Despite the abundance of cercopithecoids in the fossil record, especially in South Africa, and the recent development of morphometric approaches, uncertainties regarding the taxonomic identification of isolated cranio-dental specimens remain. Because cercopithecoids, nearly always found in stratigraphic association with hominin remains in Plio-Pleistocene deposits, are considered as sensitive ecological and chronological biomarkers, a significant effort should be made to clarify their palaeobiodiversity by assessing additional reliable morphological diagnostic criteria. Here we test the releva

Efficacy assessment and complications of surgical management for superior semicircular canal dehiscence: a meta-analysis of published interventional studies.

Superior semicircular canal syndrome (SSCS) includes vestibular and audiological symptoms which result from the introduction of a third mobile window into the osseous cochlea. Surgical repair is considered in cases of incapacitating symptoms. The present paper aims at comparing the different surgical approaches and modes of dehiscence repair, regarding their respective efficacy and potential pitfalls. A systematic literature review and meta-analysis of pooled data were performed. Study selection included prospective- and retrospective-controlled studies, prospective- and retrospective-cohort s

Glucocorticoid regulation of genes in the amiloride-sensitive sodium transport pathway by semicircular canal duct epithelium of neonatal rat.

The lumen of the inner ear has an unusually low concentration of endolymphatic Na+, which is important for transduction processes. We have recently shown that glucocorticoid receptors (GR) stimulate absorption of Na+ by semicircular canal duct (SCCD) epithelia. In the present study, we sought to determine the presence of genes involved in the control of the amiloride-sensitive Na+ transport pathway in rat SCCD epithelia and whether their level of expression was regulated by glucocorticoids using quantitative real-time RT-PCR. Transcripts were present for alpha-, beta-, and gamma-subunits of th

Cochlear changes following destruction of semicircular canal in healthy and previously toxin-exposed rats. An electrophysiological and morphological investigation.

One group of Sprague-Dawley rats (group A, n = 6) was treated by instilling Pseudomonas aeruginosa exotoxin A (PaExoA), and another (group B, n = 6) treated similarly with Haemophilus influenzae type b endotoxin (HiBEndo). In group A a 20 dB hearing loss was observed, predominantly in the high-frequency region, which was reversible within 1 month. In group B no significant hearing impairment was noted. Between 1 and 6 months later, the lateral and posterior semicircular canals (SCCs) were ablated unilaterally. Control rats (group C, n = 8) were subjected to ablation only. All rats were cochleo…

Hearing results following posterior semicircular canal injury during antesigmoid retrolabyrinthine selective vestibular nerve section.

The disastrous hearing results in six patients whose posterior semicircular canal was entered during antesigmoid mastoid craniotomy for retrolabyrinthine selective vestibular nerve section were reviewed. The injury occurred during skeletonization of the posterior semicircular canal to maximize intracranial exposure of the eighth nerve. All patients had Meniere's disease. Four patients were deafened, and in two, there was either no serviceable hearing or severe hearing impairment in the operated ear. Caution is urged in posterior semicircular canal labyrinthotomy for vertigo and attempted heari

Evaluation of Semicircular Canal Function Using the Video Head Impulse Test in a Case of Relapsing Polychondritis and Otitis Media With Effusion.

Relapsing polychondritis (RP) is a rare, immune-mediated disorder characterized by recurrent inflammation of cartilaginous structures, often affecting the ears, nose, and respiratory tract. Vestibular dysfunction in RP is uncommon but clinically significant, resulting from inner ear involvement. We describe a case of an 81-year-old man with RP who presented with fever, bilateral hearing loss, vertigo, and ocular inflammation. Bilateral otitis media with effusion (OME) was identified on imaging and otoscopy, precluding the use of caloric testing. Video head impulse test (vHIT) revealed bilatera
