BACKGROUND: Sexual violence is a global public health problem with wide-ranging and long-term health impacts. Survivors may experience further harm when healthcare settings replicate dynamics of sexual violence (re-traumatisation), underscoring the need to understand survivor perspectives on how healthcare settings can prevent harm and promote healing (trauma-informed approaches). We synthesised qualitative research exploring healthcare experiences and expectations among female survivors of sexual violence in adulthood.
METHODS: Systematic review and meta-ethnography. We searched 14 electroni

INTRODUCTION: Military sexual trauma (MST) has been associated with long-term negative outcomes such as increased rates of cardiovascular disease, post-traumatic stress disorder (PTSD), and suicidal thoughts and behaviors. While evidence supports the effectiveness of psychotherapeutic approaches as treatments for MST and related PTSD symptoms, these interventions have limited impact, attributed to perceived stigma with high dropout rates in female Veterans. Complementary and integrative health (CIH) interventions provide an alternative that may be more acceptable and can help transition Vetera…

Interpersonal violence and opioid use disorder are significant and intersecting public health concerns in the United States. The current study evaluated the consequences associated with opioid use as a function of history of interpersonal trauma, specifically physical and sexual violence. Participants were 84 trauma-exposed individuals recruited from the community who use opioids (M age = 43.5 50% men; 55% white). Whereas no significant differences emerged in the consequences of opioid use based on a history of physical violence, individuals with a history of sexual violence demonstrated highe

Sexual trauma (ST), which includes both sexual harassment and sexual assault, is associated with a variety of adverse mental and physical health outcomes in military and civilian populations. However, little is known about whether certain individual or military attributes or prior experiences may modify the relationship between recent ST and mental or physical health outcomes. Data from a longitudinal cohort study of current and former military members were used to examine whether individual and military factors modify the association between recent ST and health outcomes (posttraumatic stress…

INTRODUCTION: Posttraumatic stress symptoms (PTS) and risky alcohol use (RAU) frequently co-occur among those with a history of sexual assault, and this co-occurrence has been linked to severe psychosocial and functional impairment.
OBJECTIVE: The current study examined the roles of negative social reactions to sexual assault disclosure and trauma-related shame in the association between PTS severity and RAU. This was tested by examining the separate and sequential indirect effects of negative social reactions and trauma-related shame in the PTSD-RAU relation among individuals with a history

OBJECTIVE: Posttraumatic stress disorder (PTSD) is a common psychiatric disorder that frequently presents alongside other comorbid diagnoses. Although several evidence-based psychotherapies have been well-studied for PTSD, limited research has focused on the influence of diagnostic comorbidity on their outcomes. The present study sought to investigate the influence of comorbid social anxiety disorder on treatment outcomes in patients with PTSD.
METHODS: One hundred and twelve treatment-seeking female veteran participants with PTSD completed baseline assessments and received 12-15 sessions of

People living with HIV (PLHIV) in the United States (US) are more likely to experience traumatic events than non-affected peers. Sexual violence is a unique trauma that has important implications for HIV-related treatment and care. The aim of the current study was to examine links between sexual violence and Quality of Life (QoL) among PLHIV in South Carolina - a Southern state that is disproportionately impacted by the HIV epidemic. Specifically, we surveyed 402 PLHIV about their past exposure to sexual traumas and their current QoL across multiple domains. Results indicated that women living…

HIV-infected women who have experienced sexual violence face unique challenges in their HIV care engagement and adherence to antiretroviral medications (ARVs). Improving AIDS Care after Trauma (ImpACT) is a brief counseling intervention aimed at reducing the negative impact of sexual trauma and HIV, building coping skills, and improving long-term HIV care engagement. We conducted a randomized controlled pilot trial of ImpACT with 64 women initiating ARVs in Cape Town, South Africa, with results suggesting the intervention can reduce PTSD symptoms and increase motivation to adhere to ARVs. For …

This paper describes the development and preliminary trial run of ImpACT (Improving AIDS Care after Trauma), a brief coping intervention to address traumatic stress and HIV care engagement among South African women with sexual trauma histories. We engaged in an iterative process to culturally adapt a cognitive-behavioral intervention for delivery within a South African primary care clinic. This process involved three phases: (a) preliminary intervention development, drawing on content from a prior evidence-based intervention; (b) contextual adaptation of the curriculum through formative data c…

Sexual trauma is an understudied but regrettably significant problem among male Veterans. As in women, sexual trauma often results in serious mental health consequences for men. Therefore, to guide potential future interventions in this important group, we investigated associations among self-efficacy, male rape myth acceptance, devaluation of emotions, and psychiatric symptom severity after male sexual victimization. We collected data from 1,872 Gulf War era Veterans who applied for posttraumatic stress disorder (PTSD) disability benefits using standard mailed survey methods. The survey asked…

The dual epidemics of HIV and sexual violence against women complicate HIV care engagement in South Africa. In settings with limited mental health treatment, robust measures to enhance screening and intervention for mental health specific to people with HIV are needed. The current study developed a 42-item assessment for coping with HIV and trauma in isiXhosa. In a randomized controlled trial, 350 participants completed measures of coping, anxiety, traumatic stress, and depression at their baseline assessments. Participants were women with HIV residing in Khayelitsha, South Africa, initiating

BACKGROUND: Early traumatic experiences, particularly early sexual trauma (EST), are associated with panic disorder (PD). Using connectome-based analyses of diffusion tensor imaging, we examined the effects of EST on the brain structural networks and the associations with symptom severity and short- and long-term treatment responses in patients with PD compared to healthy controls (HCs).
METHODS: 188 participants (82 PD patients and 106 HCs) were enrolled. Brain structural networks were constructed using white matter tractography. Network-based statistics with permutation tests examined EST-r…

Women with posttraumatic stress disorder (PTSD) often report problems with sexuality. Relationship dimensions and psychological dimensions of sexuality seem to be impaired. We examined whether trauma focused therapies improve relationship and psychological dimensions of sexuality. In a randomized controlled trial that took part between 2014 and 2016 in Germany, N = 193 cisgender women with PTSD after child abuse (mean age = 36.3 years) completed the Multidimensional Sexuality Questionnaire, assessing psychological dimensions of sexuality and the Resources in Sexuality and Partnership, assessin

Child trauma plays an important role in the etiology of Bordeline Personality Disorder (BPD). Of all traumas, sexual trauma is the most common, severe and most associated with receiving a BPD diagnosis when adult. Etiologic models posit sexual abuse as a prognostic factor in BPD. Here we apply machine learning using Multiple Kernel Regression to the Magnetic Resonance Structural Images of 20 BPD and 13 healthy control (HC) to see whether their brain predicts five sources of traumas: sex abuse, emotion neglect, emotional abuse, physical neglect, physical abuse (Child Trauma Questionnaire; CTQ).…

Extending a strategy previously used by , we administered a neutral and a trauma-related version of the Deese-Roediger-McDermott paradigm to a sample of women reporting recovered (n=23) or repressed memories (n=16) of childhood sexual abuse (CSA), women reporting having always remembered their abuse (n=55), and women reporting no history of abuse (n=20). We found that individuals reporting recovered memories of CSA are more prone than other participants to falsely recalling and recognizing neutral words that were never presented. Moreover, our study is the first to show that this finding even

The current study examined exposure to multiple traumas as mediators of the relationship between childhood sexual abuse and negative adult mental health outcomes. Participants were 174 women interviewed in the third wave of a longitudinal study of the consequences of child sexual abuse. Child sexual abuse victims reported a lifetime history of more exposure to various traumas and higher levels of mental health symptoms. Exposure to traumas in both childhood and adulthood other than child sexual abuse mediated the relationship between child sexual abuse and psychological distress in adulthood.

BACKGROUND: The absence of detectable genital injury is a frequent source of medico-legal misunderstanding in child sexual abuse (CSA) evaluation. Reported vaginal penetration may occur without documented traumatic hymenal findings, and marked hymenal distensibility may be one factor associated with this absence.
OBJECTIVE: To evaluate whether a study-defined highly elastic/dilated hymenal phenotype is associated with a lower prevalence of documented traumatic hymenal findings among girls reporting vaginal penetration.
METHODS: This retrospective observational analytical study included 192 g

BACKGROUND: Military sexual trauma (MST) can have significant adverse effects on mental health and well-being, often leading to posttraumatic stress disorder (PTSD) symptoms and maladaptive beliefs. Although effective psychotherapies exist, stigma, confidentiality concerns, and systemic barriers often hinder help-seeking among service members and veterans. Mobile mental health apps offer an accessible and anonymous support alternative, potentially addressing such barriers. However, app effectiveness depends on user engagement and emerging evidence suggests that engagement may be shaped by symp…

U.S. military veterans are at a greater risk of suicide than non-veteran adults, and this risk is exacerbated in women veterans who have experienced military sexual trauma (MST). The Three-Step Theory (3ST) is an evidence-based model which describes the transition from suicidal ideation (SI) into action. It suggests that SI arises when physical and/or psychological pain is paired with hopelessness; SI increases when the pain and hopelessness outweigh feelings of connectedness. Informed by the 3ST, this paper examines PTSD, hopelessness, disconnection, and SI in 220 women veterans with MST who …

Veterans with a history of military sexual trauma (MST+) are more likely to have substance use disorders (SUD) and may be at an elevated risk of premature death, driven in part by acute causes of death associated with SUD, such as suicide and overdose, as well as those associated with MST (e.g., suicide). However, the risks of veteran status, SUD, and MST+ have generally been examined independently. The current study analyzed data from 111,222 veteran decedents (2,913 women, 108,309 men) identified via Veterans Health Administration (VHA) electronic medical record data, who had both a document…
