Trend, disparities, and projection analysis of public data on shoulder fractures in Sweden: a retrospective analysis of two hundred and sixty two thousand, four hundred and forty four fractures.

PURPOSE: We aimed to identify temporal trends, seasonal changes and regional differences in shoulder fractures in Sweden during 2008-2022. METHODS: Data from the Swedish National Board of Health and Welfare were used to assess incidence rates per 100,000 people, categorized by sex, age, and month. RESULTS: Results showed an average of 17,496 fractures annually, with a decline in 2020 followed by a resurgence in 2021-2022. Elderly women, especially those over 65, had higher rates. Winter months exhibited increased incidence. CONCLUSIONS: Projection analysis indicated a gradual decrease in fr…

Reverse shoulder arthroplasty using an onlay humeral prosthesis, offset humeral tray, and augmented glenoid baseplate: surgical technique and review of the impact of prosthesis design on scapular fractures following reverse shoulder arthroplasty.

Reverse total shoulder arthroplasty (RSA) has become the most utilized form of arthroplasty of the shoulder. Acromial stress fractures and scapular spine stress fractures are rare, yet well-recognized complications of RSA with ongoing studies identifying whether patient factors or prosthetic designs serve as risk factors. Specifically, it remains unclear if or how the position of the humeral tray (inlay or onlay) in RSA affects rates of periscapular fractures. The purpose of this article is to describe our technique for RSA using an onlay prosthesis, a variable-offset humeral tray, and an augm

Identifying surgeon-controlled factors associated with acromial fractures after reverse total shoulder arthroplasty: a case-control approach for individual surgeons.

PURPOSE: Patient characteristics are the major determinants of acromial stress fracture risk after reverse total shoulder arthroplasty; the contribution of surgeon-controlled arthroplasty geometry is less certain. This study presents a method by which an individual surgeon can analyze their own fracture cases by comparing the geometry of each case with matched non-fracture controls from the same practice. MATERIALS AND METHODS: Four cases of acromial stress fracture after primary reverse total shoulder arthroplasty by an individual surgeon were analyzed. Two were Levy Type IIB fractures that

Operative Treatment of Proximal Humeral Fractures with Reverse Total Shoulder Arthroplasty in Patients ≥65 Years Old: A Critical Analysis Review.

»: The majority of proximal humeral fractures (PHFs) in patients who are ≥65 years of age are treated nonoperatively, but certain complex fracture patterns benefit from surgical intervention. However, there continues to be debate regarding the indications for surgery and the optimal surgical treatment (repair versus replacement) in this population. »: Reverse total shoulder arthroplasty (RTSA) has grown in popularity for surgical treatment of fracture-dislocations and displaced complex PHFs in patients who are ≥65 years of age; it has definite advantages over surgical repair and hemiarthropla

In-hospital Complications Are More Likely to Occur After Reverse Shoulder Arthroplasty Than After Locked Plating for Proximal Humeral Fractures.

BACKGROUND: Currently, there seems to be a paradigm change in the surgical treatment of proximal humeral fractures in patients aged 65 years and older, with a considerable increase in the use of reverse total shoulder arthroplasty (RTSA) compared with angular stable internal fixation (locking plate fixation). However, even among shoulder specialists there is controversy regarding the best treatment strategy. QUESTIONS/PURPOSES: To evaluate for (1) a greater risk of in-hospital major adverse events, (2) a greater risk for in-hospital surgical complications, and (3) a greater risk of 30-day mor

Increased scapular spine fractures after reverse shoulder arthroplasty with a humeral onlay short stem: an analysis of 485 consecutive cases.

BACKGROUND: Scapular stress fractures after reverse shoulder arthroplasty (RSA) are a potentially serious complication with modern lateralized and onlay implants. The aim of this study was to report the scapular spine stress fracture rate after RSA with an onlay, 145° humeral stem, analyzing potential fracture risk factors and clinical outcomes in a large cohort of patients. METHODS: A consecutive series of 485 RSAs were implanted with the Aequalis Ascend Flex stem. Data collection included preoperative and postoperative clinical and radiographic assessment findings (rotator cuff Goutallier g

Reverse total shoulder arthroplasty for acute head-splitting, 3- and 4-part fractures of the proximal humerus in the elderly.

BACKGROUND: Anatomic reduction and stable internal fixation of complex proximal humeral fractures in the elderly is challenging. Secondary displacement, screw perforation, and humeral head necrosis are common complications. The outcome of hemiarthroplasty is unpredictable and strongly dependent on the uncertain healing of the greater tuberosity. This multicenter study retrospectively analyzes the midterm results of primary reverse total shoulder arthroplasty for the treatment of acute, complex fractures of the humerus in an elderly population. METHODS: Fifty-two shoulders in 51 patients with

[Marburg shoulder radiography splint (MSR splint) for standardized and high quality plain film radiography in fractures of the proximal humerus].

To ensure safe, quick, pain-relieving, standardized, and reproducible high quality plain film radiography in fractures of the proximal humerus, the MSR splint was introduced into clinical practice. With the rectangular and completely radiolucent splint the shoulder radiographs are obtained in supine position by a sole radiographer. Two radiographs are taken in projection at 90 degrees to one another: the true anteroposterior and axillary views, the most important views for fracture visualization and assessment. The smooth flat bottom part of the splint glides easily across the X-ray table belo

Case Report: Revision surgery for a missed posterior dislocation of the humeral head in a shoulder fracture-dislocation and literature review.

BACKGROUND: Posterior shoulder dislocation is uncommon and therefore prone to being missed. When accompanied by a proximal humeral fracture, complex fracture lines may obscure the radiographic signs of dislocation. Inadequate standardization of the imaging workup may consequently result in inappropriate management and treatment failure. CASE PRESENTATION: We report a young patient in whom posterior shoulder dislocation was missed at the initial assessment because axillary and scapular Y-view radiographs were not obtained and the available imaging was insufficiently interpreted. The dislocatio

Modified lever extraction technique for management of a rare femoral prosthesis fracture at the shoulder-neck junction: A case report.

Fractures involving the shoulder-neck junction of the femoral component in total hip arthroplasty are extremely rare with only 25 reported cases identified in existing literature by Van Doesburg et al. Extraction of a well-fixed fractured femoral component, particularly when cemented, is a technically demanding and time-intensive procedure that carries a considerable risk of damage to the host bone. Despite the availability of various described techniques, removal of a broken femoral stem while preserving host bone integrity remains one of the most challenging aspects of revision hip arthropla

Reverse total shoulder replacement after traumatic acute ischemia of the upper limb due to proximal humerus fracture.

Proximal humerus fractures represent approximately 5% of all fractures, predominantly affecting elderly patients with osteoporotic bone. While many cases are minimally displaced and treated conservatively, complex fracture patterns-particularly those involving multiple fragments or head-splitting components-pose significant diagnostic and therapeutic challenges. One of the rare but serious complications of these injuries is vascular involvement, most notably affecting the axillary artery due to its anatomical proximity. Although uncommon, acute ischemia of the upper limb secondary to such trau

Consensus statement on the treatment of proximal humerus fractures: a Delphi approach by the Neer Circle of the American Shoulder and Elbow Surgeons.

BACKGROUND: The treatment of patients who sustain a proximal humerus fracture (PHF) remains controversial. The purpose of this study was to find consensus among experts using a validated iterative process in the treatment of patients after a PHF. METHODS: The Neer Circle is an organization of shoulder experts recognized for their service to the American Shoulder and Elbow Surgeons. Consensus among 86 identified experts from this group was sought with a series of surveys using the Delphi process. The first 3 surveys included vignettes with 2-, 3-, and 4-part fractures, under 2 scenarios: (1) a

Postoperative Opioid Dependence in Patients Undergoing Either Total or Reverse Shoulder Arthroplasty for Proximal Humerus Fracture Fixation.

INTRODUCTION: In elderly patients who have proximal humerus fractures, treatment commonly involves total shoulder arthroplasty (TSA) or reverse shoulder arthroplasty (RSA). Following these procedures, patients often require opioids for postoperative analgesia. This common scenario is of clinical and societal importance, as increased postoperative opioid usage has been shown to worsen outcomes and increase the likelihood for dependence. We aimed to compare postoperative opioid use in patients undergoing either TSA or RSA for fixation of their proximal humerus fracture. Specifically, we assessed

Good functional results with open reduction and internal fixation for locked posterior shoulder fracture-dislocation: a case series.

BACKGROUND: There is no standardized therapeutic strategy for locked posterior shoulder fracture-dislocation (PSFD), and no consensus exists on the analysis of preoperative factors. This retrospective study aimed to evaluate functional results and complications in a series of PSFD cases managed with open surgical treatment. METHODS: Patients diagnosed with locked PSFD who underwent open surgical treatment with reduction and osteosynthesis between April 2016 and March 2020 were included. All participants were treated with open reduction and internal fixation. Functional assessment used the mod

Are there any differences between the shoulder-arm sling and figure-of-eight bandage in the conservative treatment of paediatric clavicle fractures?

PURPOSE: Clavicle fractures are treated conservatively in the paediatric age group, except in rare types of fractures. We investigated whether there was a difference between using shoulder-arm sling and figure-of-eight bandage in this age group. METHODS: This study was designed as a retrospective study. In all, 41 children among 53 who underwent conservative treatment with a shoulder-arm sling or figure-of-eight bandage between 2014 and 2019 were included in the study and divided into two groups. Treatment results were compared clinically with respect to pain intensity, muscle strength and ra

Tensioning device increases biomechanical stability of tuberosity fixation technique with cerclage sutures in reverse shoulder arthroplasty for fracture.

BACKGROUND: Complex proximal humeral fractures in elderly patients are increasingly treated with primary reverse total shoulder arthroplasty. Many surgeons use cerclage sutures for tuberosity fixation in reverse total shoulder arthroplasty for proximal humeral fractures. In this study, we hypothesized that sutures fixated with a tensioning device would achieve higher initial fixation stability of the tuberosities compared with manually knotted cerclage sutures in a biomechanical model. METHODS: A 4-part fracture was created in 7-paired human cadaver proximal humeri. The tuberosities were redu

Comparability of Patient-reported Outcome Measures and Clinical Assessment Tools for Shoulder Function in Patients with Proximal Humeral Fracture.

BACKGROUND: Patient-reported outcome measures (PROMs) are gaining increasing importance in the context of quality management. Different PROMs and scoring tools are available to assess shoulder function after proximal humeral fracture (PHFx). In Europe, these include the Constant-Murley Score (CS), Neer Score (NS), Oxford Shoulder Score (OSS), University of California at Los Angeles Score (UCLA) and Disabilities of the Arm, Shoulder and Hands Score (DASH). In addition, the health-related quality of life can be assessed by the PROMs Short Form 36 (SF-36) and EuroQol (EQ-5D). Although all these t

Is Use of a Psychological Workbook Associated With Improved Disabilities of the Arm, Shoulder and Hand Scores in Patients With Distal Radius Fracture?

BACKGROUND: Symptom intensity and magnitude of limitations correlate with stress, distress, and less effective coping strategies. It is unclear if interventions to target these factors can be used to improve outcomes after distal radius fracture in either the short- or longer term. QUESTIONS/PURPOSES: (1) Are there any factors (including the use of a workbook aimed at optimizing psychological response to injury, demographic, radiographic, medical, or psychosocial) associated with improved Disabilities of the Arm, Shoulder and Hand (DASH) and Numerical Rating Scale pain (NRS pain) scores at 6
