Cerebral Reorganization in Patients with Brachial Plexus Birth Injury and Residual Shoulder Problems.

The functional outcome after a brachial plexus birth injury (BPBI) is based on changes in the peripheral nerve and in the central nervous system. Most patients with a BPBI recover, but residual deficits in shoulder function are not uncommon. The aim of this study was to determine cerebral activation patterns in patients with BPBI and also residual symptoms from the shoulder. In seven patients (six females and one male, aged 17-23 years) with a BPBI and residual shoulder problems (Mallet score IV or lower), the cerebral response to active movement of the shoulder and elbow of the injured and he

Humeral Torsion as a Risk Factor for Shoulder and Elbow Injury in Professional Baseball Pitchers.

BACKGROUND: Numerous studies have demonstrated that humeral retrotorsion is increased in the dominant arms of throwing athletes. No study has clearly defined the relationship between humeral retrotorsion and shoulder and elbow injury. HYPOTHESIS: Uninjured professional pitchers will display more dominant humeral torsion (HT) than professional pitchers who sustain shoulder injuries but less than pitchers who sustain elbow injuries. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: Pitchers from the Colorado Rockies professional baseball organization were recruited for particip…

Statistical Evaluation of Causal Factors Associated with Astronaut Shoulder Injury in Space Suits.

INTRODUCTION: Shoulder injuries due to working inside the space suit are some of the most serious and debilitating injuries astronauts encounter. Space suit injuries occur primarily in the Neutral Buoyancy Laboratory (NBL) underwater training facility due to accumulated musculoskeletal stress. We quantitatively explored the underlying causal mechanisms of injury. METHODS: Logistic regression was used to identify relevant space suit components, training environment variables, and anthropometric dimensions related to an increased propensity for space-suited injury. Two groups of subjects were a…

Biomechanics of windmill softball pitching with implications about injury mechanisms at the shoulder and elbow.

Underhand pitching has received minimal attention in the sports medicine literature. This may be due to the perception that, compared with overhead pitching, the underhand motion creates less stress on the arm, which results in fewer injuries. The purpose of this study was to calculate kinematic and kinetic parameters for the pitching motion used in fast pitch softball. Eight female fast pitch softball pitchers were recorded with a four-camera system (200 Hz). The results indicated that high forces and torques were experienced at the shoulder and elbow during the delivery phase. Peak compressi

Major cerebral vessels injury caused by a seatbelt shoulder strap: case report.

Major cerebral arterial injury may result from penetrating or blunt trauma. In blunt trauma, clinical suspicion of such injury may not be raised, especially if the cranial CT scan is negative. We report a case of a seatbelt shoulder strap to the neck resulting in injury to three major cerebral vessels as demonstrated by cerebral angiography. Although the initial cranial CT scans were negative, a cerebral infarction ultimately developed. The patient was managed conservatively and recovered most of her functions. The importance of clinical suspicion and cerebral angiography is stressed.

Treatment of chronic radiation injury over the shoulder with a latissimus dorsi myocutaneous flap.

We report our experiences in treating chronic radiation injury about the shoulder, a complication of radiation after mastectomy. Left untreated, these can result in chronic infection and/or amputation. The coverage of a large shoulder area presents certain unique problems, which severely limit the usefulness of traditional procedures. We have found that the remarkable size and versatility of the latissimus dorsi myocutaneous flap enables one to use it with relative simplicity and safety. A further great advantage is that it brings new permanent blood supply into this ischemic area, thus favori

Innovative diagnostic framework for shoulder instability: a narrative review on machine learning-enhanced scapular dyskinesis assessment in sports injuries.

A common shoulder problem that significantly detracts from patients' quality of life is shoulder instability (SI). Abnormal scapular positioning and movement are closely associated with rotator cuff injuries and SI, as shown by several studies. The aetiology of scapular dyskinesia (SD) adversely affects shoulder stability and function, including postural abnormalities, musculoskeletal problems, and neurological conditions. Presently, there is a paucity of studies on scapular kinetic alterations in SI rehabilitation. This paper rigorously examines the correlation between SI and scapular kinetic

Functional outcomes following nerve transfers for shoulder and elbow reanimation in brachial plexus injuries: a 10-year retrospective study.

Brachial plexus injuries are rare but highly disabling, with major implications for upper limb function and quality of life. Nerve transfers have emerged as a key reconstructive technique, particularly valuable in cases where primary repair or grafting is unfeasible or delayed. This retrospective study analysed functional outcomes following nerve transfers in 37 patients with brachial plexus injury. Motor recovery was assessed using the Medical Research Council scale. Patients were stratified by age, timing of surgery, injury severity, and type of nerve transfer performed. The majority of our
