Transorbital endoscopic approaches to the skull base: current concepts and future perspectives.

INTRODUCTION: Transorbital endoscopic surgery is one of the most recent fields of skull base surgery. This paper presents the emerging applications of transorbital endoscopic approaches to the skull base and their current results on the treatment of selected extradural and intradural lesions, based on a review of meta-analysis and recent clinical series. EVIDENCE ACQUISITION: A PubMed, Cochrane and Ovid search for articles published from 2000 to 2015 was performed using the search terms "endoscopic skull base surgery", "transorbital endoscopic approach", and "transorbital neuroendoscopic surg

[Combination of posterior median and transoral approaches in surgical treatment of giant chordoma of the craniovertebral junction].

BACKGROUND: Surgical treatment of skull base and craniovertebral junction chordomas remains a challenging task in modern neurosurgery. Currently, various anterior and posterior approaches are used to remove such lesions. OBJECTIVE: To present a 14-year-old patient with giant chordoma at the level of intact clivus and C1-C3 vertebrae. Surgery implied combination of approaches without disturbing stability of craniovertebral junction. RESULTS. T: Umor in lateral parts of spinal canal was removed via posterior median access, and ventral fragment was excised through transoral access. Resection of

Isolated Abducens Nerve Palsy as the Sentinel Sign of Clival Metastasis in High-Grade Neuroendocrine Carcinoma: A Case Report.

We report a rare case of clival metastasis from a high-grade neuroendocrine carcinoma (NEC) of unknown primary origin. A 71-year-old man, previously cured of prostate adenocarcinoma, presented with abdominal pain leading to the diagnosis of metastatic NEC (Ki67 >90%) involving the liver and bones. After three lines of chemotherapy and immunotherapy, he developed sudden diplopia and right abducens nerve palsy. A brain contrast-enhanced MRI revealed a clival mass compressing the sixth cranial nerve, which was undetectable on prior imaging. The patient received palliative radiotherapy and cortico

Neoplastic nerve lesions.

Though metastasis and malignant infiltration of the peripheral nervous system is relatively rare, physicians should have a familiarity with their presentations to allow for prompt diagnosis and initiation of treatment. This article will review the clinical presentations, diagnostic evaluation, and treatment of neoplastic involvement of the cranial nerves, nerve roots, peripheral nerves, and muscle. Due to the proximity of the neural structure traversing the skull base, metastasis to this region results in distinctive syndromes, most often associated with breast, lung, and prostate cancer. Meta

Pathological axis fracture secondary to a solitary bone plasmacytoma: Two cases and a literature review.

BACKGROUND: Solitay bone plasmocytoma (SBP) account for just 5-10% of all plasma cell neoplasms. They are infrequent in the cervical spine, especially involving the C0-C2 segment. In this article we conducted a literature review and present the diagnosis, management and long term course of two patients with SBP of C2 causing cervical instability. METHODS: We assessed the clinical records of two patients with SBP in C2 and cervical instability attributed to SP-B involving C2. Both patients presented with progressive, severe cervicalgia, and the "sensation" of skull instability. Magnetic resona

Isolated hypoglossal nerve palsy due to a jugular foramen schwannoma.

Introduction - Although the involvement of the hypoglossal nerve together with other cranial nerves is common in several pathological conditions of the brain, particularly the brainstem, isolated hypoglossal nerve palsy is a rare condition and a diagnostic challenge. Case presentation - The presented patient arrived to the hospital with a history of slurred speech and an uncomfortable sensation on his tongue. Neurological examination showed left-sided hemiatrophy of the tongue with fasciculations and deviation towards the left side during protrusion. Based on the clinical and MRI findings, a d

Skull base meningiomas mimicking otitis media.

INTRODUCTION: Meningiomas are slow-growing, benign tumours originating from the arachnoid villi of the meninges. They account for 13 to 26 per cent of all intracranial neoplasms. Less than 1 per cent of all meningiomas are primary extracranial tumours of the ear and temporal bone. Intracranial meningiomas extending to the middle-ear cleft are uncommon, with fewer than 100 cases reported to date. Presenting symptoms of the latter condition may include hearing loss, tinnitus, dizziness, vertigo and facial palsy. Otitis media with effusion secondary to eustachian tube dysfunction has also been re

Paragangliomas of head and neck: a treatment option with CyberKnife radiosurgery.

Paragangliomas are highly vascular and predominantly benign neoplasms that have traditionally been treated by surgery, embolization and/or external beam radiotherapy (EBRT). The aim of this study is to evaluate the short-term local tumor control and safety of CyberKnife radiosurgery for these lesions. Nine patients, eight with jugular glomus paragangliomas and one with a carotid body paraganglioma, were treated. The target contouring was performed on merged CT and MR images. Eight patients were treated with doses ranging from 11 to 13 Gy (mean 12.5 Gy) in a single fraction and one with 24 Gy i

Extended anterior craniofacial resection for intracranial extension of malignant tumors.

OBJECTIVE: To review our experience with anterior craniofacial resection for malignant neoplasms with intracranial extension. Survival was analyzed in terms of presence of intracranial extension, extent of intradural disease, tumor histology, and histological status of margins. PATIENTS: In a retrospective review made at a tertiary cancer facility, 26 of the 115 consecutive patients undergoing craniofacial resection for malignant lesions of the anterior skull base had intracranial extension, defined as dural and/or brain extension. Survival was evaluated with the Kaplan-Meier product limit me

A case of Ollier's disease associated with two intracranial gliomas.

Ollier's disease or multiple enchondromatosis is a deforming dysplastic disease of cartilage involving primarily the metaphyses and diaphyses of long bones. It is only rarely associated with sarcomatous degeneration of the enchondromas or other generalized neoplasms. A related disease, Maffucci's syndrome, is, however, associated with generalized tumors. We present the case of a 29-year-old, albino, black man with Ollier's disease who, as a child, underwent a number of orthopedic procedures for multiple limb deformities and fractures. At age 25, he developed hydrocephalus, progressive cranial

Olfactory Carcinoma, Similar yet Distinct to Olfactory Neuroblastoma: A Case Report and Literature Review.

INTRODUCTION: Olfactory carcinoma (OC) is a rare tumor that is often misdiagnosed as olfactory neuroblastoma (ONB). Due to its rarity, there is no established standard treatment for OC, and there have been no reports of successful long-term follow-up treatment yet. CASE PRESENTATION: Two male patients, aged 54 and 48 years, presented with similar symptoms of nasal obstruction, anosmia, facial swelling, and epistaxis. Imaging in both cases revealed large, locally advanced sinonasal neoplasms. Initial biopsies were concerning for ONB; however, final pathological analyses confirmed high-grade OC

Metastatic Malignant Glomus Jugulare Tumor: A Rare Case Report with Clinical Manifestations and Treatment Approach.

BACKGROUND Paragangliomas are neuroendocrine neoplasms derived from paraganglia of the sympathetic and parasympathetic nervous systems. Parasympathetic ganglia-derived tumors, also called non-chromaffin, are located almost exclusively in the neck and skull base and are usually non-secretory and inactive. A case of malignant glomus jugulare with a metastatic cervical lymph node is described here. CASE REPORT A 24-year-old woman was referred to an otolaryngology clinic for concern of voice change for 1 month, which was associated with right progressive hearing loss, pulsating tinnitus, and right

[Endoscopic Surgery for Skull Base Tumors].

With the development of endoscopic and peripheral instruments, endonasal or transcranial endoscopic surgery for skull-base tumors has become more common. Preoperative simulation makes it relatively easy to understand the anatomical relationship between skull base tumors and the surrounding vital structures, which vary with each case. This may lead to the avoidance of complications and an improvement in the removal rate. Especially in cases of skull base tumors where multiple surgical approaches are possible, the three-dimensional model can be used to confirm the surgical field for each approac

[Expanded Endoscopic Endonasal Surgery for Skull Base Tumors].

With the development of endoscopic and surgical instruments, expanded endoscopic endonasal surgery for skull base tumors has been widely performed. Compared with conventional microscopic transsphenoidal surgery, endoscopic surgery provides a wider view and clearer images, owing to the introduction of a high-resolution camera. However, for safe and reliable surgery, it is necessary to create an appropriate surgical field and avoid complications. This article discusses the basic knowledge needed for expanded endoscopic endonasal surgery, focusing on surgical anatomy and techniques and how to clo

[An anatomical study on the applicability of direct endoscopic exclusion of the ethmoid arteries for devascularization of the anterior skull base structures].

UNLABELLED: The anterior skull base structures are the site of initial growth of histologically different tumors. The difficulties in their removal are often associated with significant vascularization, which may limit the amount of resection due to abundant intraoperative blood loss. Midline tumors are primarily fed by the ethmoid arteries that are not accessible to embolization. The aim of this work was a comparative experimental study of various direct approaches to the ethmoid arteries. MATERIAL AND METHODS: The study was conducted on anatomical specimens of 12 cadaveric heads of deceased

[Biopsy and resection of skull base tumors using transorbital endoscopic approaches: primary results].

AIM: Transorbital neuroendoscopic surgery is a new skull base surgery technique that uses the orbit as an artificial corridor to the anterior and middle skull base. The space is created between the periorbita and orbital walls by their additional resection and gentle traction of the orbital contents. Skull base structures are reached using cosmetic incisions. The major advantages of transorbital endoscopic approaches include their variety, possibility of their combination, and access to the central and lateral skull base lesions. The aim of this study was to analyze the primary results of tran

A panoramic view of the skull base: systematic review of open and endoscopic endonasal approaches to four tumors.

Endoscopic endonasal surgery has been established as the safest approach to pituitary tumors, yet its role in other common skull base lesions has not been established. To answer this question, we carried out a systematic review of reported series of open and endoscopic endonasal approaches to four major skull base tumors: olfactory groove meningiomas (OGM), tuberculum sellae meningiomas (TSM), craniopharyngiomas (CRA), and clival chordomas (CHO). Data from 162 studies containing 5,701 patients were combined and compared for differences in perioperative mortality, gross total resection (GTR), c

Chondroblastoma of the skull and facial bones.

A series of 30 chondroblastomas was reviewed: 21 had occurred in the lateral part of the temporal bone, 6 in the mandible, 1 in the parietal bone, and 2 in the region that included the temporal bone and mandible. Of the 30 patients in the series, 20 were males and 9 were females; the sex of 1 patient was not stated. The ages of 29 patients ranged from 2 years 11 months to 70 years (mean, 43.5 years). Radiologic findings were not suggestive of a specific diagnosis, although the lesions appeared to be benign. Histologically, most tumors were classic chondroblastomas. However, some showed aneurys

Lipomatosis of the scalp and macrocephaly.

In summary, a case of macrocephaly and lipomatosis of the scalp and forehead has been presented. The phenotypic and clinical features found in our patient are in accordance with previous reports. The differential diagnosis with other hamartoneoplastic syndromes has been evaluated and discarded, concluding that the present report is in accordance with Bannayan-Zonana syndrome. However, as in other inherited traits, finding a genetic test to diagnose this entity remains a challenge.
