Measurement of speech quality as a tool to optimize the fitting of a hearing aid.

The purpose of the present research was to develop a theoretical basis for the adjustment of hearing aid frequency response based on speech quality measurements. Speech quality measurements were made using continuous discourse and a category rating procedure for the following dimensions: intelligibility, pleasantness, loudness, effort, noisiness, and total impression. Speech quality ratings were obtained from a group of listeners with hearing loss who wore hearing aids. The stimulus conditions simulated hearing aid frequency response alterations within a frequency response range where intellig

Acoustic analysis in the assessment of postoperative speech impairment in oral cancer patients.

PURPOSE: Acoustic analysis is an objective and quantitative method in diagnosing speech disorders and evaluating treatment efficacy. Although many reports have used acoustic analysis for oral cancer patients, few studies have focused on the extent of tongue resection. The aim of this study was to evaluate the usefulness of acoustic analysis in the assessment of postoperative speech impairment in oral cancer patients. METHODS: Postoperative speech data as well as Speech Handicap Index (SHI) and 5-point speech intelligibility scale were collected from 65 patients including 31 cases of oral canc

Speech outcome in complete unilateral cleft lip and palate - a comparison of three methods of the hard palate closure.

The aim of this study was to compare the speech in subjects with cleft lip and palate, in whom three methods of the hard palate closure were used. One hundred and thirty-seven children (96 boys, 41 girls; mean age = 12 years, SD = 1·2) with complete unilateral cleft lip and palate (CUCLP) operated by a single surgeon with a one-stage method were evaluated. The management of the cleft lip and soft palate was comparable in all subjects; for hard palate repair, three different methods were used: bilateral von Langenbeck closure (b-vL group, n = 39), unilateral von Langenbeck closure (u-vL group,

Normative Performance Functions for the Modified Connected Speech Test.

PURPOSE: The Connected Speech Test (CST) assesses an individual's ability to understand everyday contextualized running speech amidst competing background babble. To minimize accent effects on speech perception scores and reduce the noise floor of the original recordings, an updated version was developed by Saleh et al. (2020). The updated recordings feature a speaker with a General American accent to replace the Southern U.S. accent in the original test, and modern recording equipment was used to achieve a lower noise floor. The aim of this study was to collect normative data, characterizing

Longitudinal Evaluation of Auditory and Speech Performance in Pediatric Cochlear Implant Recipients Using IT-MAIS, MESP, CAP, and SIR Scales.

Considerable variability in auditory and speech outcomes among pediatric cochlear implant (CI) recipients has raised ongoing debate regarding whether early auditory perception directly determines later speech intelligibility, particularly in tonal language environments where pitch cues are critical for communication. This longitudinal study evaluated 64 prelingually deaf children aged 1-5 years over 12 months following unilateral implantation, integrating four standardized behavioral scales-the Infant-Toddler Meaningful Auditory Integration Scale (IT-MAIS), Categories of Auditory Performance (

Effect of stimulus level on speech masking by multitalker noise in a cochlear model.

Previous research shows that intelligibility of speech in noise (SIN) with a constant speech-to-noise ratio (SNR) changes in dependence on overall stimulus level. It has been hypothesized that these changes were associated with nonlinear responses of the cochlear basilar membrane, however, the exact role of auditory periphery in this behavior is not clear. We explore this phenomenon using a nonlinear cochlear model with adjustable gain to examine how multi-talker (babble) noise affects the speech signal at the output of the auditory periphery. The amount of distortion at various intensities ca

Outcome measurements after oral cancer treatment: speech and speech-related aspects--an overview.

INTRODUCTION: Oral cancer and its surgical treatment impair speech quality by an alteration of the vocal tract. Local size and stage of the tumour as well as surgical and adjuvant treatment modalities have an impact on the functional outcome of patients' speech rehabilitation. It was the aim of this overview to specifiy speech and speech-related aspects as well as to delineate measurement methods of speech outcome in patients with oral cancer by a review of the literature. METHODS: The review is based on a Medline Search on "speech", "cancer", "oral cancer", "malignoma mouth", "intelligibilit

Speech outcome following late primary palate repair.

OBJECTIVE: Early palate repair is recommended to minimize the development of disordered speech. We studied the speech outcome of late palate repair in 131 patients. The success of late palate repair is questioned because of the persistence of learned, compensatory misarticulations that are difficult to correct in spite of the establishment of correct palatal anatomy and a competent velopharyngeal mechanism. The objective of this study is to highlight the speech results following late primary repair of the palate. SETTINGS AND DESIGN: Retrospective analysis of speech outcomes in 131 patients w

Speech-evoked brain activity is more robust to competing speech when it is spoken by someone familiar.

When speech is masked by competing sound, people are better at understanding what is said if the talker is familiar compared to unfamiliar. The benefit is robust, but how does processing of familiar voices facilitate intelligibility? We combined high-resolution fMRI with representational similarity analysis to quantify the difference in distributed activity between clear and masked speech. We demonstrate that brain representations of spoken sentences are less affected by a competing sentence when they are spoken by a friend or partner than by someone unfamiliar-effectively, showing a cortical

Home-based biofeedback speech treatment improves dysarthria in repeat-expansion SCAs.

CAG repeat-expansion spinocerebellar ataxias (CAG-SCAs) are genetically defined multisystemic degenerative diseases, resulting in motor symptoms including dysarthria with a substantial impact on daily living. Whilst speech therapy is widely recommended in ataxia, very limited evidence exists for its use. We evaluated the efficacy of a home-delivered, ataxia-tailored biofeedback-driven speech therapy in CAG-SCA in 16 individuals with SCA1, 2, 3, or 6. Treatment was delivered intensively over 20 days. Efficacy was evaluated by blinded ratings of intelligibility (primary) and acoustic measures (s

The effectiveness of clear speech as a masker.

PURPOSE: It is established that speaking clearly is an effective means of enhancing intelligibility. Because any signal-processing scheme modeled after known acoustic-phonetic features of clear speech will likely affect both target and competing speech, it is important to understand how speech recognition is affected when a competing speech signal is also spoken clearly. In 2 experiments, the authors investigated whether listeners would experience improved intelligibility when both target and nontarget speech were spoken clearly. METHOD: Listeners' recognition of sentences in competing sounds

"Sushi roll" technique for precise total tongue functional reconstruction using a pre-sutured femoral anterolateral myocutaneous flap.

OBJECTIVES: Reconstruction of the total tongue after cancer resection remains one of the challenges in head and neck surgery. Inadequate reconstruction after subtotal or total glossectomy defects leads to poor quality of life. The aim of this study was to explore an economical, practical and effective flap design for functional tongue reconstruction. MATERIAL AND METHODS: Sixty patients were randomly divided into two groups, namely, a "Sushi roll" technique group (30 patients) and a conventional surgery group (30 patients). Then, the patients underwent total or subtotal tongue reconstruction.

Validation of an Intelligibility Test Based on Acoustic-Phonetic Decoding of Pseudo-Words: Overall Results from Patients with Cancer of the Oral Cavity and the Oropharynx.

OBJECTIVES: Loss of intelligibility is a major complaint for patients with speech disorders, as it affects their everyday communication and thus contributes to a decrease in their quality of life. Several tests are available to measure intelligibility, but these tests do not take into account the evaluators' ability to restore distorted sequences. Due to this ability, the evaluator will tend to recognize words despite phonetic distortions, and speech production deficit can go undetected. The results of these tests therefore overestimate the intelligibility of patients and may mask real functio

The long-term speech outcome in Flemish young adults after two different types of palatoplasty.

OBJECTIVE: The success of cleft palate surgery is specifically determined by the subsequent speech characteristics. There are several types of surgical techniques to repair the palate. The surgeon chooses his or her own technique according to the principles (s)he have established based on experience. The main purpose of this study is to determine and to compare the long-term speech outcome (18 years after surgery) regarding overall intelligibility, articulation, resonance, and voice after one-stage Wardill-Kilner palatoplasty or two-stage Furlow palatoplasty. The authors hypothesized that a de
