The Role of Biological Age in Adult Spinal Deformity Surgery.

➢ Adult spinal deformity (ASD) outcomes are poorly predicted by chronological age alone; biological age markers, including frailty indices, sarcopenia measurements, and bone quality metrics, provide better risk assessment. ➢ Systemic frailty tools (modified Frailty Index, ASD-Frailty Index [ASD-FI], Risk Analysis Index) predict perioperative morbidity and discharge disposition more accurately than age-based models do. ➢ Musculoskeletal assessments, psoas and paraspinal muscle cross-sectional areas, fatty infiltration grading, and computed tomography-derived Hounsfield unit thresholds identify

Morphology, pathology, and the vertebral posture of the La Chapelle-aux-Saints Neandertal.

Although the early postural reconstructions of the Neandertals as incompletely erect were rejected half a century ago, recent studies of Nea … his advanced age, osteoarthritis, and lordosis. Our findings challenge the view of generally small spinal curvatures in Neandertals. Setting aside the developmentally abnormal Kebara 2 vertebral column, La Chapelle-aux-Saints 1 is joined by other Neandertals with sufficient vertebral remains in providing them with a fully upright (and human) axial posture.

Radiological severity of hip osteoarthritis in patients with adult spinal deformity: the effect on spinopelvic and lower extremity compensatory mechanisms.

PURPOSE: Sagittal spinal deformity (SSD) patients utilize pelvic tilt (PT) and their lower extremities in order to compensate for malalignment. This study examines the effect of hip osteoarthritis (OA) on compensatory mechanisms in SSD patients. METHODS: Patients ≥ 18 years with SSD were included for analysis. Spinopelvic, lower extremity, and cervical alignment were assessed on standing full-body stereoradiographs. Hip OA severity was graded by Kellgren-Lawrence scale (0-4). Patients were categorized as limited osteoarthritis (LOA: grade 0-2) and severe osteoarthritis (SOA: grade 3-4). Patie

Adult cervical deformity: radiographic and osteotomy classifications.

Cervical spine deformity represents a broad spectrum of pathologies that are both complex in etiology and debilitating towards quality of life for patients. Despite advances in the understanding of drivers and outcomes of cervical spine deformity, only one classification system and one system of nomenclature for osteotomy techniques currently exist. Moreover, there is a lack of standardization regarding the indications for each technique. This article reviews the adult cervical deformity (ACD) and current classification and nomenclature for osteotomy techniques, highlighting the need for furth

Camptocormia in Parkinson's disease.

Camptocormia is defined as an abnormal, severe and involuntary forward flexion of the thoracolumbar spine, which becomes manifest during standing and walking and subsides in the recumbent position. It was originally described as a psychogenic disorder, particularly in soldiers involved in long-term trench service during World War 1. It is becoming increasingly recognized as a prominent and disabling phenomenon during the course of Parkinson's disease (PD). In our experience, there is no clear correlation between camptocormia and levodopa treatment. In a few patients, the abnormal posture impro

Association Between Lumbar Lordosis, Thoracic Kyphosis, and Muscle Activations During Different Lower Back Exercises: An Observational Study.

Background and Objectives: Angular modifications in the physiological curvatures of the spine have been associated with spinal dysfunction and altered biomechanics, which may contribute to musculoskeletal complaints. The main objective of this experimental study was to investigate the association between lumbar lordosis and thoracic kyphosis angles and muscle activations during three different lower back exercises. Materials and Methods: Participants were divided into a hyperlordotic lumbar angle group-with increased thoracic kyphosis (n = 11)-and a normal lordotic lumbar thoracic angle group

Impaired glycine neurotransmission causes adolescent idiopathic scoliosis.

Adolescent idiopathic scoliosis (AIS) is the most common form of spinal deformity, affecting millions of adolescents worldwide, but it lacks a defined theory of etiopathogenesis. Because of this, treatment of AIS is limited to bracing and/or invasive surgery after onset. Preonset diagnosis or preventive treatment remains unavailable. Here, we performed a genetic analysis of a large multicenter AIS cohort and identified disease-causing and predisposing variants of SLC6A9 in multigeneration families, trios, and sporadic patients. Variants of SLC6A9, which encodes glycine transporter 1 (GLYT1), r

Spinal posterior movement after posterior cervical decompression surgery: clinical findings and factors affecting postoperative functional recovery.

This study investigated the posterior movement of the spinal cord after posterior decompression surgery and evaluated factors affecting postoperative functional recovery in patients with cervical spondylotic myelopathy (CSM). Thirty-two patients with CSM underwent posterior decompression from C3 to C7 through laminectomy (n=12) and single, open-door laminoplasty (n=20). There were no significant differences between laminectomy and laminoplasty in degree of spinal posterior movement, recovery rate, and curvature index. Japanese Orthopedic Association (JOA) scores improved from preoperative (10.

Evaluating the Effectiveness of sEMG Biofeedback for Posture Training and Scoliosis Management.

Digital devices, particularly smartphones and tablets, are used by the vast majority of the global population. This usage often causes the head to tilt forward, placing significant strain on the neck and shoulders. While this posture adversely affects healthy individuals, it is particularly problematic for scoliosis patients, exacerbating spinal deformation. Therefore, this study evaluates the effectiveness of a 30-session surface electromyography (sEMG) biofeedback posture training program for managing spinal curvature progression in adolescents with mild scoliosis. The program is aimed at re

A cervical lordosis curvature decline in Chinese asymptomatic populations from 2016 to 2024: a comparative analysis.

BACKGROUND: There is increasing concern regarding the effects of contemporary lifestyles on spinal health. This study aimed to evaluate recent trends in cervical lordosis (CL) curvature over the past eight years. METHODS: Asymptomatic individuals who underwent cervical spine radiography at four hospitals were randomly selected from hospital databases for a comparative analysis of data spanning from 2016 to 2024. The curvature of CL was assessed using three methods: the C2-C7 Cobb angle (Cobb A), the C2-C7 Harrison posterior tangent angle (Harrison A), and Borden’s method for evaluating CL dept

Effect of personalized spinal profile on its biomechanical response in an EMG-assisted optimization musculoskeletal model of the trunk.

Recent developments in musculoskeletal (MS) modeling have been geared towards model customization. Personalization of the spine profile could affect estimates of spinal loading and stability, particularly in the upright standing posture where large inter-subject variations in the lumbar lordosis have been reported. This study investigates the biomechanical consequences of changes in the spinal profile. In 31 participants (healthy and with back pain), (1) the spine external profile was measured, (2) submaximal contractions were recorded in a dynamometer to calibrate the EMG-driven MS model and

Scoliosis in the Rett syndrome: natural history and treatment.

The Rett syndrome (RS) is associated with a neurological form of scoliosis. From 1985 we have instituted a postal survey of families with Rett girls. The prevalence of scoliosis in the survey population is 64%. The age at onset of scoliosis has a normal distribution about a peak age of 8 years, with 72% of cases occurring before age 8. The scoliosis in RS is typically a long thoracolumbar curvature that progresses rapidly in girls over the age of 10 years. Operative treatment is successful in reducing the curvature, preventing curve progression and improving spinal balance for sitting and walk

Imbalanced development of anterior and posterior thorax is a causative factor triggering scoliosis.

OBJECTIVE: Scoliosis is a common disease characterized by spinal curvature with variable severities. There is no generally accepted theory about the physical origin of the spinal deformation of scoliosis. The aim of this study was to explore a new hypothesis suggesting that the curvatures in scoliosis may be associated with the imbalance growth between thoracic vertebral column and sternum. METHODS: We undertook a comparative computed tomography (CT) based morphology study of thoracic vertebrae and sternum of patients with adolescent idiopathic scoliosis (AIS) and age-gender matched normal su
