Spontaneous rupture of the spleen occurs as a serious but relatively infrequent complication of infectious mononucleosis. Over 100 such cases have been reported. In all but one of the cases previously reported, diagnosis of splenic rupture was made at laparotomy. Three new cases are reported where the diagnosis of splenic rupture was made using sonography. The patients were followed by sonography and none required laparotomy. Aspiration of the hematoma under sonographic control confirms the diagnosis and may be curative.

Patients with major hematologic disorders who have hypersplenism and alterations in their immune mechanism are subject to a higher incidence of bacteremia after embolization procedures. In certain instances, these infectious complications can be fatal. Medical splenectomy for hematologic disorders is sometimes complicated by massive splenic infarction and spontaneous rupture; spontaneous rupture appears to be a function of both infarct size and underlying infectious complications. Prophylactic measures can be employed to avoid these complications after interventional splenic embolization.

An 80-year-old man was admitted to our hospital for recurrent chest discomfort and shortness of breath over the past 2 years, with symptoms … re at the upper pole of the spleen, requiring emergency surgical exploration. Intraoperatively, the splenic rupture site was identified, and substantial intraperitoneal hemorrhage was noted. Therefore, total splenectomy was performed. The patient's postoperative recovery was initially uneventful; however, chemotherapy was not administered due to poor physical sta …

Atraumatic or spontaneous splenic rupture is a rare but life-threatening emergency. Common causes include hematologic malignancies and infections, but tuberculous involvement of the spleen is exceedingly uncommon. To our knowledge, this represents the third reported case of spontaneous rupture of a tuberculous spleen. We report a 50-year-old human immunodeficiency virus (HIV) -positive woman who presented with cough, fever, and abdominal distension, with abdominal examination suggesting peritoneal irritation and laboratory tests showing severe anemia. Emergency laparotomy revealed a ruptured s…

Background: Leukemic non-nodal variant mantle cell lymphoma (nnMCL) is an uncommon subtype of mantle cell lymphoma that lacks lymphadenopathy and generally follows an indolent clinical course. Adverse genetic alterations such as TP53 inactivation and del(13q) may have prognostic significance. Clinical findings such as splenomegaly may serve as a clue to the diagnosis and should prompt further evaluation. Case Presentation: We describe a 91-year-old woman who presented with a one-month history of anemia (hemoglobin 12.3 g/dL), mild thrombocytopenia (platelets 136 × 109/L), isolated splenomegaly

Splenic artery aneurysm (SAA) is an uncommon and life-threatening vascular disease. This pathology is frequently diagnosed during pregnancy. The major complication of this aneurysm is rupture, which happen frequently in pregnant women due to physiological changes. We report, two rare cases of sudden death due to SAA rupture occurring during the first two trimesters of pregnancy and discovered during autopsy. Case 1: A 26-weeks pregnant woman consulted the Emergency for a sudden onset of abdominal pain and dizziness. An ultrasound showed fetal death. She underwent two surgeries, and the origin

Splenic rupture following colonoscopy, although uncommon, is a potentially life-threatening complication that all medical doctors should consider in their differential diagnosis. Several risk factors contribute to this condition, including exogenous elements such as the endoscopist's experience, patient positioning, medications, mechanical force during endoscope advancement, and technique at the splenic flexure, as well as endogenous factors like patient anatomy, perisplenic adhesions, inflammatory or infectious diseases, hematological disorders, malignancy, and interventions such as biopsy or…

Splenic rupture is a recognized complication of acute Plasmodium falciparum and Plasmodium vivax malaria, but the risk of splenic rupture in chronic asymptomatic infections is unknown. In Timika, Papua, Indonesia, we determined the proportion of PCR-detectable asymptomatic peripheral parasitemia in patients undergoing trauma-related splenectomy (2015-21) and found it was more than twice the proportion compared to a 2013 household survey of the general population (87.9% [29/33] vs 38.6% [697/1807]; P < .0001). Our findings suggest asymptomatic parasitemia with either P. falciparum or P. vivax i…

EBV is a gammaherpesvirus that infects a large fraction of the human population. Many people infected with the virus remain asymptomatic. The most common clinical manifestation of EBV is linked to infectious mononucleosis (IM). IM usually resolves over a period of weeks or months without sequelae. However, splenic rupture is another rare complication of IM. Splenic rupture of this case occur without any traumatic cause.

Infectious mononucleosis, caused by Epstein-Barr virus (EBV), is typically self-limiting but can lead to rare, life-threatening complications such as atraumatic splenic rupture (ASR). We report the case of a 16-year-old female presenting with left upper quadrant pain and peritoneal signs. The initial laboratory findings revealed grade I anemia and leukocytosis. Images obtained by simple CT revealed the presence of free intra-abdominal fluid and a perisplenic hematoma. This raised the diagnostic suspicion of splenic rupture in the context of an acute EBV infection. Emergency splenectomy was req…

INTRODUCTION: Spontaneous splenic rupture (SSR) is an uncommon and potentially life-threatening cause of acute abdominal pain, often linked to underlying pathology. Because its presentation can mimic more common conditions, diagnosis is frequently delayed. We report a case from a resource-limited setting where SSR initially mimicked peptic ulcer disease, underscoring diagnostic challenges.
CASE PRESENTATION: A 57-year-old Ethiopian woman with chronic reflux presented after 5 days of severe epigastric pain radiating to the left shoulder, with nausea and vomiting but no trauma or fever. Initial…

BACKGROUND Extracorporeal shockwave lithotripsy (ESWL) is a common procedure, and splenic rupture is a rare complication of ESWL. Depending on the stage of injury and patient's condition, treatment options include non-operative management (NOM) and emergency splenectomy. Diagnosis is not difficult with symptoms such as deteriorating hemodynamic and hematologic indices, localized physical signs of peritoneal irritation in the left hypochondriac region, and confirmation provided by signs of free fluid (hemoperitoneum) seen on ultrasound or computed tomography (CT). Prompt diagnosis and treatment…

A rare case of an unexpected iatrogenic splenic rupture during a cesarean section is reported. The trauma was recognized early and treated conservatively without delay; thus, further complications were avoided. A 28-year-old woman with a history of previous cesarean sections was submitted for an elective cesarean section. Intra-operatively, minor bleeding from the left abdomen was noted and eventually assigned to an inferior pole splenic trauma treated conservatively without splenectomy. Although unclear, the injury was considered iatrogenic, probably due to the abdominal pressure for fetal de…

Mantle cell lymphoma (MCL) is a type of non-Hodgkin (B-cell) lymphoma (NHL) with manifestations ranging from indolent to aggressive disease. This type of NHL is predominately found in western countries and affects men more often than women (M:F 2:1). The median age of diagnosis with the disease is around 60 years of age. In this report, the patient is a 68-year-old female who had an atraumatic splenic rupture with no past medical history of trauma. She presented to the emergency department with severe abdominal pain in her left upper quadrant. An emergency splenectomy was executed successfully…

Spontaneous splenic rupture is a life-threatening condition leading to a rapidly progressing hypovolemic shock due to intra-abdominal blood loss, with a mortality rate of about 10%. Spontaneous splenic rupture can be caused by widely different disorders including acute and chronic infections, neoplastic disorders, and inflammatory noninfectious disorders. In this case report, we present a 67-year-old male patient with hemorrhagic shock caused by an acute bleeding from the splenic artery. The patient was massively transfused with blood products and fluids and underwent laparotomy for hemostatic…

Pancreatic pseudocyst is a common complication of acute and chronic pancreatitis. However, spleen involvement in pancreatitis is rare. We present a patient with a pancreatic tail pseudocyst with splenic extension and rupture. Due to initial stability, conservative management was decided. However, he developed tachycardia with severe abdominal pain associated with signs of peritoneal irritation, requiring an emergency laparotomy. A large pancreatic tail pseudocyst was identified in addition to a ruptured spleen. Splenectomy and double layer hand-sewn gastrocystic anastomosis were performed. The
