Improving Splenomegaly Segmentation by Learning from Heterogeneous Multi-Source Labels.

Splenomegaly segmentation on computed tomography (CT) abdomen anatomical scans is essential for identifying spleen biomarkers and has applications for quantitative assessment in patients with liver and spleen disease. Deep convolutional neural network automated segmentation has shown promising performance for splenomegaly segmentation. However, manual labeling of abdominal structures is resource intensive, so the labeled abdominal imaging data are rare resources despite their essential role in algorithm training. Hence, the number of annotated labels (e.g., spleen only) are typically limited w…

Massive splenomegaly due to B-cell lymphoma: A case report.

INTRODUCTION: Massive splenomegaly is indicated by spleen weight exceeding 1000 g and largest spleen dimension greater than 20 cm Poulin et al. (1998). In many cases, splenectomy is the treatment of choice for massive splenomegaly because it releases the pressure on adjacent organs and also provides a definitive histopathological diagnosis of the underlying cause Iriyama et al. (2010), Radhakrishnan (2018). PRESENTATION OF CASE: Herein we present a clinical case of disseminated diffuse large B - cell lymphoma, clinical stage IV, with massive splenomegaly. A 53 - year old man complaining of un…

Splenomegaly Segmentation on Multi-Modal MRI Using Deep Convolutional Networks.

The findings of splenomegaly, abnormal enlargement of the spleen, is a non-invasive clinical biomarker for liver and spleen diseases. Automated segmentation methods are essential to efficiently quantify splenomegaly from clinically acquired abdominal magnetic resonance imaging (MRI) scans. However, the task is challenging due to: 1) large anatomical and spatial variations of splenomegaly; 2) large inter- and intra-scan intensity variations on multi-modal MRI; and 3) limited numbers of labeled splenomegaly scans. In this paper, we propose the Splenomegaly Segmentation Network (SS-Net) to introd…

Palliative splenic irradiation for symptomatic splenomegaly in non-Hodgkin lymphoma.

INTRODUCTION AND AIMS: Splenic marginal zone lymphoma, an uncommon subtype of non-Hodgkin lymphoma (NHL), is usually present with symptomatic splenomegaly. Although splenectomy has long been considered the first-line therapy in symptomatic or cytopenic patients, it can lead to significant morbidity and mortality. Splenic irradiation is an option for patients who have a poor response to systemic therapy and/or are not surgical candidates. In this paper, we present a case report of a patient who received splenic radiotherapy for symptomatic splenomegaly. METHODS: An 85-year-old Caucasian man wi…

Praziquantel Targets M1 Macrophages and Ameliorates Splenomegaly in Chronic Schistosomiasis.

Splenomegaly is a common feature of many infectious diseases, including schistosomiasis japonica. However, the immunopathogenesis and the treatment of splenomegaly due to schistosomiasis have been largely neglected. Praziquantel (PZQ), a classical schistosomicide, has been demonstrated by us and others to have antifibrotic and anti-inflammatory activities against schistosomiasis. In this study, we investigated the effect of PZQ on alleviating the splenomegaly caused by Schistosoma japonicum infection in mice. The results showed that the number of macrophages, especially the number of M1 macrop…

Hyperreactive Malarial Splenomegaly Syndrome--Can the Diagnostic Criteria Be Improved?

Hyperreactive Malarial Splenomegaly Syndrome (HMSS) was described and defined before sensitive tests for malaria were available. We present a series of seven individuals who were referred to our clinics with possible HMSS. Chronic malaria was demonstrated in those successfully treated but not in those who failed to respond to therapy. This observation suggests that the newer molecular malaria assays have a role to play in the identification of individuals who are likely to respond to treatment for HMSS in non-endemic regions.

Immunological characteristics of hyperreactive malarial splenomegaly syndrome in sudanese patients.

Hyperreactive Malarial Splenomegaly (HMS) is defined as a massive enlargement of the spleen resulting from abnormal immune responses after repeated exposure to the malaria parasites. This study was carried out in Khartoum, Sudan. Sudan is considered to be one of the countries where HMS is quite prevalent. The objective of the study was to determine the incidence of HMS in patients who reported to the Omdurman Tropical Diseases Hospital (OMTDH) in Sudan and to investigate the basic laboratory and immunological characteristics of this condition in these patients. A cross-sectional study was carr…

Isolated splenic peliosis presenting with giant splenomegaly and severe coagulopathy.

We herein describe a case of successful surgical treatment of isolated splenic peliosis presenting with giant splenomegaly and severe coagulopathy. Pathological features of the spleen included multinodular, blood-filled, cyst-like lesions distributed throughout the whole organ. The pathogenesis of isolated splenic peliosis is controversial, and no definitive disease mechanism has been reported. To our knowledge, this is the first report in English of a case like this.

Seventy-four-year-old woman with myelodysplastic syndrome and splenomegaly.

We present a 74-year-old woman with a recent diagnosis of myelodysplastic syndrome who presented with left upper quadrant abdominal pain and fatigue with significant splenomegaly, anemia, and thrombocytopenia. She underwent splenectomy and bone marrow biopsy. Pathology of both spleen and bone marrow revealed an unusual diagnosis. A review of the differential diagnosis, laboratory tests, nature of the underlying disease, and treatment are provided.

Non-tropical idiopathic splenomegaly (Dacie's syndrome): report of 5 cases.

We describe 5 patients, aged 22-69 years, with massive splenomegaly of unknown origin an features of hypersplenism. Splenectomy corrected the cytopenia(s) and abolished the symptoms in each case; the histological features of the spleen were non-specific and included congestive changes and lymphoid hyperplasia. 2 of these patients developed non-Hodgkin's lymphoma (NHL) 2 and 6 years after spenectomy. A total of 46 cases of splenomegaly of unknown origin have been reported from UK, USA, and Australia so far, and in 9, NHL developed 8 to 80 months after splenectomy. The questions relating to the …

Massive splenomegaly in Northern Zambia. I. Analysis of 344 cases.

Massive splenomegaly is common in Northern Zambia. Nearly 40% of 344 patients were diagnosed as Tropical Splenomegaly Syndrome of malarial origin. Portal hypertension, lymphoreticular and haemopoietic malignancies, and schistosomiasis accounted for another 46%. The common causes of splenomegaly show differences of frequency within Zambia and also between Zambia and other African countries.

Translocator Protein Deficiency Promotes Splenomegaly with Extramedullary Hematopoiesis after Sepsis.

The translocator protein 18 kDa (TSPO) is expressed in immune cells such as macrophages and hematopoietic progenitors and is involved in mod … with increased spleen weight. Histological analysis revealed that TSPO deletion was associated with splenomegaly with red pulp expansion at the expense of white pulp and increased production of ectopic megakaryocytes. We observed that TSPO deletion led to the gene expression profile enriched in enhanced erythropoiesis and reduced immune responses in the spleen …

Sarcoidosis presenting with massive splenomegaly: 2 cases without pulmonary involvement.

Sarcoidosis is a chronic systemic granulomatous disease presenting in the lungs and mediastinal lymph nodes. Extrapulmonary disease alone is rare and involvement of the spleen alone, in the absence of the lung, is even rarer. Splenomeglay is unusual as a first symptom and gives a diagnostic problem, especially in areas with high prevalence of infectious or haematologic diseases. We describe 2 young adults who had massive splenomegaly, and minimal systemic symptoms, with no evidence of either clinical or radiologic thoracic sarcoidosis. Both patients were thoroughly investigated for infection, …

Galectin-8 deficiency promotes chronic splenomegaly persistence in Chagas disease.

Galectins (Gals) are mammalian lectins with affinity for β-galactosides, which drive the immune response through several mechanisms. The specific role of Gal-8 in the development of inflammation remains controversial, as it has been shown to induce either T cell proliferation or regulation in different models. During the acute phase of Trypanosoma cruzi infection, a characteristic splenomegaly is induced that is associated with both antigen-specific and non-specific polyclonal lymphocyte proliferation. This splenomegaly resolves as the infection transitions to the chronic phase. While the path…
