The time interval from first clinical symptoms to definite diagnosis of ankylosing spondylitis (Morbus Bechterew) is still too long. Thus, many years for essential therapeutic interventions are unequivocally lost. Therefore, it is most important to improve early diagnosis. It is therefore interesting that serum polymeric IgA, a precursor of secretory IgA, was statistically elevated in active AS (n = 35) and that levels were comparable to those found in yersiniosis (n = 12); this might indicate antigenic stimulation by bacteria which are present in the intestines of AS patients. A case of ankylosing spondylitis with aortic valve disease and hearing loss is described. A series of radiographic and audiometric investigations showed the hearing loss to be of a conductive type. It seemed most likely that the hearing loss was related to an inflammatory involvement of the ossicular joints due to the primary disease. No other case of conductive hearing loss has previously been reported due to otoarthritis in ankylosing spondylitis. This is important both theoretically and practically.

Slight elevations in circulating immune complexes (CIC) were found in 10 ankylosing spondylitis patients when compared to healthy subjects, whilst higher levels of CIC and other complement products were found in patients with Reiter's syndrome and reactive arthritis. It is suggested that complement studies should be carried out at different stages of the disease; firstly during episodes of infection and secondly during the stage of chronic inflammatory disease.

Ankylosing spondylitis can present a difficult diagnostic challenge. Not only is its etiology unknown, but its clinical manifestations are myriad and sometimes precede classic low back pain by years. The foremost aid in diagnosis is an awareness of these manifestations, coupled with a willingness to make a tentative (possible or probable) diagnosis of the disease. One hundred and twenty-eight of 145 patients with ankylosing spondylitis (AS) were found to be HLA B27 positive. Five patients had evidence of a sero-negative peripheral arthritis resembling peripheral psoriatic arthritis and 3 of these were B27 negative. 278 patients with ankylosing spondylitis have been studied in order to find a basis for prognosis by setting a mobility function index of the vertebral column. This index does not depend on manifestation age or on the blood sedimentation rate. It increases with progression of x-ray findings. A coincidence of impaired mobility and overweight is relatively frequent and function index and electrophoresis are correlated. In contrary to this study a study of the course gives probably more prognostic indications.

Thirty patients with spondylitis were treated by decompression of the cord (31 operations). The spondylitis was tuberculous in 28 of septic unspecific aetioloty in two cases. The results with respect to the neurological symptoms were complete recovery in 24 cases, imcomplete recovery in four cases, and failure in three cases, or in ten per cent.
