Colonization of medical devices by staphylococci.

The use of medical devices in modern medicine is constantly increasing. Despite the multiple precautionary strategies that are being employed in hospitals, which include increased hygiene and sterilization measures, bacterial infections on these devices still happen frequently. Staphylococci are among the major causes of medical device infection. This is mostly due to the strong capacity of those bacteria to form device-associated biofilms, which provide resistance to chemical and physical treatments as well as attacks by the host's immune system. Biofilm development is a multistep process wit

Rifaximin intake leads to emergence of rifampin-resistant staphylococci.

OBJECTIVES: Rifaximin is a poorly absorbed non-systemic antimicrobial agent used in various gastrointestinal disorders. Rifampin is pivotal for the treatment of staphylococcal foreign body infections and resistance develops rapidly during monotherapy. The close structural relation of rifaximin to rifampin may lead to cross-resistance. The aim of our study was to determine whether rifampin-resistance emerges in human skin staphylococci during or after oral intake of rifaximin. METHODS: Rifampin resistance of skin staphylococci in healthy volunteers during and after intake of rifaximin was dete

From the toe to the head: metastatic endophalmitis following staphylococcal purpura fulminans.

Staphylococcal infections may present in variable ways. Staphylococcal sepsis can cause significant morbidity and mortality. We report on the case of a 42-year-old man without any co-morbidities who presented with fever and purpura fulminans and later developed metastatic endophthalmitis. He was treated with intravenous and intravitreal vancomycin. The case is reported because of the rare co-occurrence of purpura fulminans and endophthalmitis.

Antimicrobial resistance in staphylococci. Epidemiology, molecular mechanisms, and clinical relevance.

Staphylococcal infections continue to pose important clinical problems in children and adults. Antibiotic resistance among the staphylococci has rendered therapy of these infections a therapeutic challenge. Despite early, uniform susceptibility to penicillin, staphylococci acquired a gene elaborating beta-lactamase that rendered penicillin inactive and that is borne by nearly all clinical isolates. "Penicillinase-resistant beta-lactams," such as methicillin, were introduced in the early 1960s, but resistance to them has become an increasing concern. The mechanism of the so-called "methicillin …

Immunoglobulin A-Dominant Post-Infectious Glomerulonephritis Mimicking Henoch-Schönlein Purpura Nephritis in a Patient With Methicillin-Sensitive Staphylococcus aureus Bacteremia.

Staphylococcal infections remain a prominent cause of hospital- and community-acquired infections in the United States. Glomerulonephritis with predominant Immunoglobulin A (IgA) deposition following staphylococcal infection has been described as IgA-dominant postinfectious glomerulonephritis (IgA-PIGN). This clinical entity can mimic Henoch-Schönlein purpura nephritis (HSPN) given that it may also be preceded by staphylococcal infection, have similar kidney biopsy findings, and present with similar signs and symptoms. Our patient presented with a palpable purpuric rash and acute kidney injury…

Antistaphylococcal IgE in patients with atopic dermatitis.

Levels of IgE antibodies to Staphylococcus aureus and Staphylococcus epidermidis were determined in eleven patients with typical atopic dermatitis, with no history of furuncles or severe staphylococcal infection. Increased IgE binding to S. aureus but not to S. epidermidis was observed. Fifteen patients with hyperimmunoglobulinemia E-staphylococcal abscess syndrome had increased IgE binding not only to S. aureus but also to S. epidermidis. Other control groups of patients with elevated IgE levels or recurrent staphylococcal infection had normal IgE binding activity to both strains of staphyloc

CD9 co-operation with syndecan-1 is required for a major staphylococcal adhesion pathway.

Epithelial colonization is a critical first step in bacterial pathogenesis. Staphylococcus aureus can utilize several host factors to associate with cells, including α5β1 integrin and heparan sulfate proteoglycans, such as the syndecans. Here, we demonstrate that a partner protein of both integrins and syndecans, the host membrane adapter protein tetraspanin CD9, is essential for syndecan-mediated staphylococcal adhesion. Fibronectin is also essential in this process, while integrins are only critical for post-adhesion entry into human epithelial cells. Treatment of epithelial cells with CD9-d

In vitro time-kill kinetics of dalbavancin against Staphylococcus spp. biofilms over prolonged exposure times.

Staphylococcus aureus and Staphylococcus epidermidis are leading pathogens of biofilm-related infections and represent the most common cause of osteomyelitis and biomedical implants infections. Biofilm-related infections usually require long-term antibiotic treatment, often associated to surgical interventions. Dalbavancin is a newer lipoglycopeptide approved for the treatment of acute skin and skin-structure infections caused by Gram-positive pathogens. In addition, dalbavancin has recently been considered as a potential option for the treatment of staphylococcal osteomyelitis and orthopedic

Antimicrobial resistance in staphylococci in small animals.

Staphylococcal antimicrobial resistance presents an emerging challenge for both human and veterinary medical professionals. Infections associated with methicillin- and multidrug-resistant staphylococci are increasingly encountered by veterinarians and are frequently associated with empiric therapeutic failures and limited systemic antimicrobial options. This article addresses mechanisms of antimicrobial resistance in common staphylococcal pathogens and implications for clinical practice, including indications for culture and susceptibility testing, rational antimicrobial selection, and potenti

Septic arthritis. Staphylococcal induction of chondrocyte proteolytic activity.

We report herein that cartilage proteolytic activity increased in bovine and rabbit articular cartilage after treatment with a purified staphylococcal culture medium or intraarticular infection with Staphylococcus aureus. Staphylococcal culture medium increased the release of gelatinolytic, collagenolytic, and caseinolytic activity into the medium of isolated chondrocytes or cartilage organ culture. The proteolytic activities were determined in assays using radiolabeled substrate and sodium dodecyl sulfate-polyacrylamide gel electrophoresis. Staphylococcal culture medium was proteolytically in

Toll-like receptor 2 ligands on the staphylococcal cell wall downregulate superantigen-induced T cell activation and prevent toxic shock syndrome.

Staphylococcal superantigens are pyrogenic exotoxins that cause massive T cell activation leading to toxic shock syndrome and death. Despite the strong adaptive immune response induced by these toxins, infections by superantigen-producing staphylococci are very common clinical events. We hypothesized that this may be partly a result of staphylococcal strains having developed strategies that downregulate the T cell response to these toxins. Here we show that the human interleukin-2 response to staphylococcal superantigens is inhibited by the simultaneous presence of bacteria. Such a downregulat

The impact of Staphylococcus aureus-associated molecular patterns on staphylococcal superantigen-induced toxic shock syndrome and pneumonia.

Staphylococcus aureus is capable of causing a spectrum of human illnesses. During serious S. aureus infections, the staphylococcal pathogen-associated molecular patterns (PAMPs) such as peptidoglycan, lipoteichoic acid, and lipoproteins and even intact S. aureus, are believed to act in conjunction with the staphylococcal superantigens (SSAg) to activate the innate and adaptive immune system, respectively, and cause immunopathology. However, recent studies have shown that staphylococcal PAMPs could suppress inflammation by several mechanisms and protect from staphylococcal toxic shock syndrome,

Case report: toxic shock syndrome arising from cellulitis.

Toxic shock syndrome is a febrile, multiorgan illness related to toxins elaborated by staphylococcal or streptococcal infections. In the 1980s, most cases were associated with menstruation. More recently, many cases now are unrelated to menses. In this article, the authors describe a case of a nonmenstruating woman with toxic shock syndrome, associated with cellulitis of her arm. Cultures of the arm grew Staphylococcal aureus, which produced enterotoxin B.

Staphylococcal scalded skin syndrome in an immunocompromised adult.

Staphylococcal scalded skin syndrome, a generalized exfoliative dermatitis complicating infections by exfoliative toxin-producing strains of Staphylococcus aureus, is rarely observed in adults. In contrast to mortality in infants, mortality in adults is usually high. A case of generalized staphylococcal scalded skin syndrome in an immunocompromised woman is reported. Culture of skin biopsy and pleural fluid yielded identical strains of staphylococcus aureus belonging to phage group II. Exfoliative toxins A and B were detected in both isolates. As far as can be determined, this is the first rep

Effect of inoculum and of beta-lactamase on the anti-staphylococcal activity of thirteen penicillins and cephalosporins.

Because there are few persuasive data for selecting one semisynthetic penicillin or cephalosporin over another for treatment of serious staphylococcal infections, 118 recent clinical isolates of Staphylococcus aureus were studied to determine to what extent the presence of beta-lactamase affected the relative anti-staphylococcal activity of six penicillins and seven cephalosporins. In addition, the effect of inoculum was studied for its possible effect on the anti-staphylococcal activity of the 13 beta-lactam antibiotics. By all criteria, methicillin and nafcillin were clearly more resistant t…

Comparison of Staphylococcus aureus isolates associated with food intoxication with isolates from human nasal carriers and human infections.

Staphylococcus aureus represents an organism of striking versatility. While asymptomatic nasal colonization is widespread, it can also cause serious infections, toxinoses and life-threatening illnesses in humans and animals. Staphylococcal food poisoning (SFP), one of the most prevalent causes of foodborne intoxication worldwide, results from oral intake of staphylococcal enterotoxins leading to violent vomiting, diarrhea and cramps shortly upon ingestion. The aim of the present study was to compare isolates associated with SFP to isolates collected from cases of human nasal colonization and c
