Examination of 328 patients with vertical squint has revealed peculiarities of its clinical forms. Their characteristics are: I. Concomitant vertical squint; II. A mixed form--concomitant convergent or divergent squint with a vertical component; III. Paretic or paralytic vertical squint; IV. Atypical kinds of vertical squint. Special attention is drawn to the presence of torticollis, difference in lid slit width, pseudoptosis, diplopia. Basic principles of treatment of vertical squint corresponding the clinical form are suggested. Results of treatment in patients with different forms of vertic

Anterior segment circulation was assessed in 35 adults one day after squint surgery by clinical observation and low-dose fluorescein iris angiography. Seventeen patients had primary vertical rectus muscle surgery and all showed angiographic evidence of ischaemia. No ischaemia was found in the 15 patients who had secondary vertical rectus muscle surgery, or any horizontal rectus muscle surgery. The staged group had intermediate findings between the above two. Age, dysthyroid eye disease and type of conjunctival incision did not correlate with fluorescein iris angiographic sector-filling delay o

Fifty-four consecutive surgical cases of congenital ptosis were carefully evaluated to determine the incidence of refractive errors, strabismus and amblyopia. Of these patients, 70% had a refractive error, 43% had an astigmatism of more than 1.0, 55% had anisometropia, 27.5% had concomitant squint and 50% amblyopia.

A series of 38 patients with regressed retinopathy of prematurity were compared with control groups of babies having similar gestational age who did not have retinopathy of prematurity, and to a control group of full-term babies. A substantially higher incidence of strabismus, amblyopia, and high refractive errors was found in the group with retinopathy of prematurity as compared with both other groups. In general, these patients had a successful response to standard amblyopia and strabismus management. No substantial difference was found between the refractive errors of the babies in the prem…

Ocular strabismus, a common condition in the present generation is an absolute risk factor for amblyopia and blinding premorbid visual loss. Despite the availability of new optometry tools with eye-tracking data, the issues persist in attaining accuracy and reliability in diagnosing strabismus. These two concerns are specifically accommodated in this study by the proposed novel approach that involves CNNs with eye-tracking datasets from subjects. The presented work aims to improve the accuracy of diagnostics in ophthalmology utilizing the integration of the further proposed algorithms into an …

Addressing ocular misalignment secondary to partial and complete oculomotor nerve palsy presents a special challenge to the strabismus surgeon, particularly when treating patients with binocular diplopia. We review the reported surgical options and, through illustrative cases, provide our own perspective on managing this disorder.

The advent of integrated intraoperative ocular coherence tomography (i2OCT) has opened the door for safer more complex surgeries of the retina and cornea. However, to limit its use to just two subspecialties within ophthalmology is an opportunity lost for many other subspecialties. Here, we describe the use of i2OCT in strabismus surgery. It can be used to identify extraocular muscles, especially in cases of strabismus reoperations, thereby augmenting the surgical technique. Its use to help train strabismus surgery is invaluable, allowing the trainee to understand the depth of the scleral pass…

AIMS: To compare the effects of topical diclofenac sodium with those of fluorometholone on intraocular pressure (IOP) and conjunctival inflammation after strabismus surgery.
METHODS: We retrospectively analysed 60 Korean children who underwent strabismus surgery for intermittent exotropia in an institutional referral centre. Patients received topical diclofenac 0.1% or fluorometholone 0.1% after surgery for up to 4 weeks. IOP, patient discomfort, conjunctival inflammation and conjunctival injection were evaluated at 1, 2, 3 and 4 weeks after surgery.
RESULTS: In the fluorometholone group, 23…

Motility disorders and strabismus after vitreo-retinal surgical intervention have multiple etiologic factors. The main focus here is strabismus following scleral buckling surgery. The preoperative evaluation of these patients must be inclusive for restrictions, redirected muscle forces, adherence syndromes, muscle weakness, and visual sensory disturbance. Restoration of binocular function is generally the goal of surgical intervention, but paramount to surgery is assessing the potential for binocular function. Surgical techniques for overcoming the barriers to fusion are discussed with clinica…

The paradigm that an underacting extraocular muscle (EOM) is always atrophic or hypoplastic, and an overacting EOM should always be enlarged, leads to inconsistencies with clinical observations. It is inconsistent with the findings of normal extraocular muscle diameters in patients with apparent superior oblique muscle palsy, "overacting" inferior oblique muscles, and the superior rectus muscle overaction / contracture syndrome, among other clinical entities. These inconsistencies can be reconciled if one accepts the possibility that EOM contractile activity may reflect a change in neural inpu

PURPOSE: To investigate diplopia after orbital decompression in patients with thyroid-related orbitopathy.
METHODS: We retrospectively analyzed medical records of 45 consecutive patients who had undergone orbital decompression surgery (DS) and a subgroup of 10/45 patients who underwent disinsertion of the inferior oblique (IO) muscle during DS. Diplopia was assessed before DS and 1 and 3 months after DS. Residual strabismus defects were treated surgically approximately 3 months after DS with follow-up visits 1 week, 3 months, and 6 months postoperatively. Patients with horizontal or vertical …

Eye movement abnormalities constitute an important clinical sign that can be a manifestation of dysfunction of cranial nerves III, IV, and VI (the 3 ocular motor nerves). Specific motility deficits often have highly localizing value within the neuroaxis, serving to refine a differential diagnosis and guide management. This article reviews the key anatomic concepts, clinical presentation, differential diagnosis, and management of ocular motor nerve palsies. Dysfunction of an ocular motor nerve must be distinguished from other causes of abnormal eye movements, such as myasthenia gravis or thyroi

PURPOSE: To evaluate the clinical features of strabismus that present after cataract surgery and determine the motor and sensory results after surgical correction of the strabismus.
SETTING: Department of Ophthalmology, the Samsung Medical Center, Sungkyunkwan University School of Medicine, Seoul, Korea.
METHODS: Thirty-one patients who had strabismus surgery after cataract surgery between January 1996 and June 2004 were included in the study. The clinical features of strabismus and the factors contributing to successful strabismus surgery results were retrospectively analyzed. Sensory funct…

We evaluated the one-stage intraoperative adjustment strabismus surgery with adjustable suture under topical anesthesia. Medical records of the patients who had intraoperative adjustment surgery under topical anesthesia for horizontal or vertical strabismus in our hospital from March 1997 to March 2003 with follow-up of 6 months were analyzed retrospectively. Of the 71 patients, 48 patients had exotropia, 16 had esotropia, and 7 had hyper- or hypotropia. The overall success rates were 85.9% at 1 week, 83% at 1 month, 78.9% at 3 months, and 76.1% at 6 months after surgery. The procedure was not…
