Management of noncardiovascular comorbidities in patients with heart failure with reduced ejection fraction.

Patients with heart failure with reduced ejection fraction often have one or more noncardiovascular comorbidities. The presence of concomitant disease states is associated with worse outcomes, including increased risk of mortality, decreased quality of life, and increased healthcare resource utilization. Additionally, the presence of heart failure with reduced ejection fraction complicates the management of these comorbidities, including varying safety and efficacy of therapies compared to those without heart failure. This article will review the literature on the pharmacologic management of c

Immune Responses and Anti-inflammatory Strategies in a Clinically Relevant Model of Thromboembolic Ischemic Stroke with Reperfusion.

The poor clinical relevance of experimental models of stroke contributes to the translational failure between preclinical and clinical studies testing anti-inflammatory molecules for ischemic stroke. Here, we (i) describe the time course of inflammatory responses triggered by a thromboembolic model of ischemic stroke and (ii) we examine the efficacy of two clinically tested anti-inflammatory drugs: Minocycline or anti-CD49d antibodies (tested in stroke patients as Natalizumab) administered early (1 h) or late (48 h) after stroke onset. Radiological (lesion volume) and neurological (grip test)

Prognostic Significance of Infarct Size and Location: The Case of Insular Stroke.

The prognostic relevance of strokes in different locations is debated. For example, insular strokes have been associated with increased mortality, but this association could reflect their greater severity. In two independent cohorts of patients with supratentorial ischemic stroke (n = 90 and 105), we studied the prognostic consequences of lesion size and location using voxel-based lesion-symptom mapping before and after volume control, which better accounts for total lesion volume. Strokes affecting the insula were larger than non-insular strokes (28 vs 2cc and 25 vs 3cc, p < 0.001 in both coh

Post-stroke losartan and captopril treatments arrest hemorrhagic expansion in SHRsp without lowering blood pressure.

We assessed the ability of poststroke losartan and captopril treatment to attenuate hematoma expansion and plasma extravasation after intracerebral hemorrhagic stroke in Kyoto-Wistar stroke-prone hypertensive rats (SHRsp). Cerebrum volume, herniation and surface areas exhibiting new and old hemorrhages and albumin extravasation were measured prior to and after stroke and following 30 and 60 days of post-stroke losartan or captopril treatment in Evans Blue dye perfused brains. Lesion morphology was studied in serial sections. Losartan or captopril treatment initiated at stroke prevented death f

Prediction of stroke recurrence for patients with middle cerebral artery atherosclerotic disease by CT perfusion.

BACKGROUND: This paper aimed to test the feasibility of computed tomography perfusion (CTP) in predicting stroke recurrence in patients with symptomatic middle cerebral artery atherosclerotic disease. METHODS: This is a retrospective study. 322 patients from 4 advanced stroke centers diagnosed as symptomatic middle cerebral artery atherosclerotic disease, including severe stenosis and occlusion were recruited. All patients underwent head CTP, digital subtraction angiography (DSA), MR 14-21 days after onset. Stroke recurrence of patients within 3 months was recorded. Patients were divided into

Visually-induced hemodynamic response and white matter hyperintensities in middle-aged women with ischemic stroke or migraine with aura.

BackgroundMigraine, especially with aura (MA), increases the risk of brain white matter hyperintensities (WMH) and ischemic stroke. Altered microvascular function has been proposed as a shared underlying mechanism, potentially involving endothelial dysfunction. Visually-induced hemodynamic response describes the process by which cerebral blood flow is locally increased to meet the metabolic demands of neuronal activity and may serve as a surrogate measure of cerebral endothelial function. We investigated the relationship between visually-induced hemodynamic responseand WMH volume in middle-age

Association of laterality and size of perfusion lesions on neurological deficit in acute supratentorial stroke.

BACKGROUND: The influence of lesion size and laterality on each component of the National Institutes of Health Stroke Scale has not been delineated. The objective of this study was to use perfusion-weighted imaging to characterize the association of ischaemic volume and laterality on each component item and the total score of the <National Institutes of Health Stroke Scale. METHODS: We analysed consecutive right-handed patients with first-ever supratentorial acute ischaemic strokes who underwent acute perfusion-weighted imaging at a single centre. Perfusion deficits were defined as mean trans

Combined Echocardiographic and Cardiopulmonary Exercise to Assess Determinants of Exercise Limitation in Chronic Obstructive Pulmonary Disease.

BACKGROUND: Current methods do not allow a thorough assessment of causes associated with limited exercise capacity in patients with chronic obstructive pulmonary disease (COPD). METHODS: Twenty patients with COPD and 20 matched control subjects were assessed using combined cardiopulmonary and stress echocardiographic testing. Various echocardiographic parameters (left ventricular [LV] volumes, right ventricular [RV] area, ejection fraction, stroke volume, S', and E/e' ratio) and ventilatory parameters (peak oxygen consumption [Vo2] and A-Vo2 difference) were measured to evaluate LV and RV fun…

Tracking the changes in longitudinal MRI-detected perivascular spaces following ischaemic stroke.

INTRODUCTION: Stroke is a leading cause of mortality and morbidity worldwide. Magnetic resonance imaging-visible perivascular spaces (PVS) are an emerging marker of cerebral small vessel disease and may have prognostic value in stroke. We aimed to characterise longitudinal changes in PVS volume and cluster count following ischaemic stroke. PATIENTS AND METHODS: Perivascular space volumes and cluster counts were compared between stroke survivors (n = 124; 39 women; median [Q1, Q3] age = 70 [62, 76] years) and healthy controls (n = 39; 15 women; median age = 69 [66, 72.5] years). Impact of hospital volume on long-term neurological outcome in patients undergoing carotid artery stenting.

BACKGROUND: The impact of hospital volume on long-term outcome after carotid artery stenting (CAS) remains unknown. OBJECTIVES: We designed a nationwide cohort study to elucidate the impact of hospital volume on the incidence of stroke after CAS. METHODS: The Taiwan National Health Insurance Research database was used to identify all patients admitted for CAS from 2008 to 2012. We defined high-volume hospitals as those performing more than 20 CAS per year. The primary outcome was new ischemic stroke after discharging from the index CAS. Propensity score-matching was performed to create two m

Assessment of fluid responsiveness in mechanically ventilated cardiac surgical patients.

BACKGROUND AND OBJECTIVE: Accurate assessment of preload responsiveness is an important goal of the clinician to avoid deleterious volume replacement associated with increased morbidity and mortality in mechanically ventilated patients. This study was designed to evaluate the accuracy of simultaneously assessed stroke volume variation and pulse pressure variation using an improved algorithm for pulse contour analysis (PiCCO plus, V 5.2.2), compared to the respiratory changes in transoesophageal echo-derived aortic blood velocity (deltaVpeak), intrathoracic blood volume index, central venous pr…

The risk of stroke recurrence in patients with atrial fibrillation and reduced ejection fraction.

BACKGROUND: Atrial fibrillation (AF) and congestive heart failure often coexist due to their shared risk factors leading to potential worse outcome, particularly cerebrovascular events. The aims of this study were to calculate the rates of ischemic and severe bleeding events in ischemic stroke patients having both AF and reduced ejection fraction (rEF) (⩽40%), compared to ischemic stroke patients with AF but without rEF. METHODS: We performed a retrospective analysis that drew data from prospective studies. The primary outcome was the composite of either ischemic (stroke or systemic embolism)

Primary heart failure with preserved ejection fraction: The intertwined pathophysiology and treatment of some of the most relevant phenotypes.

Heart failure with preserved ejection fraction (HFpEF) is characterized by diverse underlying pathophysiological mechanisms and can be divided into two subgroups based on the identification of the specific cause: primary and secondary HFpEF. Primary HFpEF is caused by primary impairments in myocardial relaxation or compliance with the contribution of several risk factors. Therefore, we have reviewed current literature on pathophysiology and treatment in patients with primary HFpEF. Patients with primary HFpEF represent specific "phenotypes" and are usually elderly, more commonly women, and oft

Sepsis and the Heart: In the Quest for Noninvasive Pressure-Volume Loops at the Bedside.

BACKGROUND: Sepsis-induced cardiomyopathy (SICM) is prevalent yet remains difficult to diagnose using conventional echocardiography primarily due to its dependence on loading conditions, the dynamic nature of sepsis, and varied cardiovascular phenotypes. Recent advancements in noninvasive myocardial work (MW) analysis, particularly through left ventricle pressure-strain loop (LV PSL), offer a promising strategy for evaluating myocardial function by combining strain imaging with blood pressure data. This technique may address the limitations inherent in traditional measures such as ejection fra

[Diastolic heart failure: myth or fact?].

Diastolic heart failure is a frequent diagnosis, but now it is more acurate to call this heart failure with preserved ejection fraction. The diagnosis can be made non invasively by the echocardiography and measuring BNP (brain natriuretic peptide). Consensus about treatment are less evidence-based than for heart failure with diminished ejection fraction. It is however certain that control of hypertension and heart rate are important. Fluid overload must also be treated with diuretics.
