A new method of percutaneous treatment of some fractures of the fingers is being described. With the use of a magnification table, fitted onto the X-ray intensifier, evulsion fractures of extensor tendons or collateral ligaments can be reduced and fixed by means of a K-wire. After insertion, the end of the wire is being bent in the shape of a hook ("wire hook"), into which a pull-out wire is inserted. By anchoring the wire-hook at the opposite side of the finger under tension, the fragment is held in position by the hook. After four weeks the wire-hook is cut below its anchorage and can easily

BACKGROUND: Port-access minimally invasive cardiac surgery has been developed with the aim of reducing surgical invasion. We conduct port-access cardiac surgery, generally through an intercostal space. To ensure sufficient visual field, either the upper or lower rib may be divided. In the process of restoration of divided ribs, we have developed a bone fixation technique using a new osteosynthesis device by way of prevention of pseudoarthrosis.
METHODS: Forty-five consecutive patients underwent right-sided anterior intercostal thoracotomy under the port-access method between July 2006 and Jul

Traditional surgical interventions for frontal sinus fractures necessitate a cut on the forehead skin, and extant closed reduction techniques aimed at enhancing accessibility continue to grapple with secure tool fixation, stable bone elevation, and screw breakage risk. To address these challenges and augment surgical efficiency, this study introduces novel surgical devices. Design parameters for models with spiral or L-shaped tips are established, considering practical medical requirements and constraints, and subsequently validated through finite element method numerical simulations using com

In this article, we aim to review the diagnosis, classification, and treatment of acromioclavicular joint injury by mainly describing the progress of surgical treatment for acromioclavicular joint injury, providing a reference for clinical diagnosis and treatment. Acromioclavicular joint injury is a common sports injury, which is more common in male than in female patients. The mechanism of the injury is mainly caused by direct or indirect violence, which can be clearly diagnosed by radiography. According to the time after injury, it can be divided into acute injury and chronic injury. The Roc

INTRODUCTION: Nonunion treatment decisions are influenced by the classification of nonunion patterns identified. Surgical intervention, frequently incorporating compression devices, is a prevalent strategy utilized in the management of nonunion fractures. This report aimed to explain the result of treatment of hypertrophic nonunion with compression plate.
CASE PRESENTATION: A 65-year-old female patient presented with chronic pain and limited range of motion in her left arm subsequent to an inadequately healed humerus fracture. A proximal humerus compression plate with autologous bone graft wa

INTRODUCTION: Ipsilateral proximal and shaft femoral fractures typically occur in young adults after high-energy trauma. No consensus exists regarding the optimal internal fixation device or surgical strategy for these complex fractures. Our main objective is to identify differences on outcomes and complications between patients treated with one or combined implants.
MATERIAL AND METHOD: This is a single-center retrospective cohort study in patients with associated fractures of the proximal (31 AO) and shaft femur (32 AO). We divided the patients into two groups according to the use of single

A 2-year-old, egg-laying maroon-bellied conure (Pyrrhura frontalis) was referred for treatment of bilateral femoral fractures believed to be secondary to recent egg laying and nutritional and husbandry deficiencies. On radiographs, all skeletal components were diffusely osteopenic, precluding surgical fixation. A novel device to stabilize the bilateral femoral fractures was manufactured with polyvinyl chloride piping, which braced the bird's feet at the level of the perch and supported its upper body weight via a breast plate, thus maintaining the bird in a physiologically appropriate perching

BACKGROUND: Management of displaced acromioclavicular joint (ACJ) injuries remains contentious. It is unclear if delayed vs. acute reconstruction has an increased risk of fixation failure and complications. The primary aim of this study was to compare complications of early vs. delayed reconstruction. The secondary aim was to determine modes of failure of ACJ reconstruction requiring revision surgery.
METHODS: A retrospective study was performed on all patients who underwent operative reconstruction of ACJ injuries over a 10-year period (Rockwood III-V) using suspensory devices with or withou

Medial collateral ligament injury of the first metatarsophalangeal (MTP) joint is rare. If it is missed, chronic instability and traumatic hallux valgus develop, requiring surgical treatment. Different methods have been reported in the limited available literature aiming to restore the balance between the lateral and medial stabilizers by tightening the medial joint capsule with or without additional tendon graft. Our described method utilizes a suture button device (Mini TightRope, Arthrex, Naples, Florida) for reconstruction. This device applies tension to hold the hallux in the correct posi

Pectus excavatum (PEX) is the most frequent congenital deformity of the anterior chest wall and is defined as the dislocation of the medial or inferior portion of the sternal region toward the spinal column. There are various ways to measure the deformity. In this study, we present an objective method of assessing such deformity, the anthropometric index for PEX (AI-PEX). The AI-PEX was developed in the Thoracic Surgery Department of the Heart Institute - University of São Paulo School of Medicine Hospital das Clínicas. The anthropometric measurements are taken during the physical examination.

Distraction osteogenesis is a technique utilizing natural healing mechanisms to generate new bone; it is commonly used to lengthen the hypoplastic mandible. Distraction of the maxilla and mandible as a unit is an obvious extension of the technique. We describe the application of a semiburied distractor to simultaneously lengthen the mandible and maxilla and level a canted occlusal plane in three cases. The indications for bimaxillary distraction are reviewed, including its advantages, disadvantages and limitations.

Thoracolumbar (TL) burst fracture is one of the most common indications for minimally invasive percutaneous pedicle screw fixation. Although the indication for surgical treatment of neurologically intact TL fractures remains under debate, studies have demonstrated that posttraumatic malalignment may lead to a deterioration in the patient's quality of life. For burst fractures with malalignment or fragments in the spinal canal, a reduction technique using ligamentotaxis is commonly used to improve long-term outcomes. The sagittal adjusting screw (SAS) system is a monoaxial screw system with a f

BACKGROUND Surgical treatment of acetabular fractures is one of the greatest challenges for orthopedic surgeons. Fixation of most displaced fractures requires extensive exposure, which may lead to complications, including blood loss, neural or vascular injury, postoperative infection, wound healing problems, and heterotopic bone formation. MATERIAL AND METHODS This study was conducted to certify an anatomic plate with an anterior column lag screw guiding device to repair the posterior acetabulum. Complete pelvic spiral computed tomography (CT) scan data were collected from 56 patients. The pos

We report a case of sudden cardiovascular collapse several weeks following surgical repair of a traumatic diaphragmatic hernia. The patient presented with features of circulatory shock without a clear diagnosis, therefore an urgent computed tomography scan of the chest and abdomen was undertaken, which revealed a pericardial effusion with evidence of cardiac tamponade. Ultrasound-guided needle pericardiocentesis with aspiration of blood from the pericardial sac in the Emergency Department provided an immediate response and her cardiac output improved. On review of the imaging, it is likely a s

Anterior cruciate ligament reconstruction is a frequently performed procedure in arthroscopic surgery, with rising rates paralleling the incidence of anterior cruciate ligament ruptures worldwide. Synthetic grafts, especially hybrid constructs combining artificial devices with autologous tissue, offer biomechanical advantages and potential faster recovery. This article describes a modified technique for primary all-inside anterior cruciate ligament reconstruction using a 4 × 2-strand hybrid graft composed of a tubular synthetic device combined with the gracilis tendon. This technique aims to d

We present a case of a severe osteoporotic octogenarian who sustained serious flail chest from a traffic accident. The 3rd-9th ribs of the right chest wall were fractured. Non-operative management was unsuccessful. We performed a surgical fixation using a bio-absorbable and bio-active mini-plating set. This plating set is unsintered hydroxyapatite (u-HA) particles/poly-L-lactide (PLLA) composite osteosynthesis device commonly used for cranial, oral, and maxillofacial surgeries. The use of the u-HA/PLLA device for chest wall reconstruction has previously been reported, but no long-term results

In a relatively short period of time, transcatheter and endovascular approaches to treat thoracic aortic and structural heart disease have exploded onto the scene. New device frontiers already being forged in the experimental stages include expanded indications and variations of fenestrated and branch stentgrafting to treat thoracoabdominal and arch disease, endovascular ascending and aortic root repair, and all of the cardiac valves. A fundamental concept to optimize durability of endovascular repair is the need for fixation and seal in healthy tissue. Before long, the entire vascular and car
