Quality of end-of-life care for Vietnam-era Veterans: Implications for practice and policy.

BACKGROUND: In federal response to the aging population of Vietnam-era Veterans, Congress directed the Department of Veterans Affairs (VA) to create a pilot program to identify and develop best practices for improving hospice care for this population. A first step in VA's response was to identify whether the end-of-life (EOL) care needs and outcomes of Vietnam-era Veterans differed from previous generations. METHODS: Using medical records and bereaved family surveys, we examined clinical characteristics, healthcare utilization, and EOL quality indicators for Vietnam-era Veterans who died in V

Caring, sharing, preparing and declaring: how do hospices support prisons to provide palliative and end of life care? A qualitative descriptive study using telephone interviews.

BACKGROUND: Older adults in prison have complex healthcare needs, and many will need palliative care before their sentence ends. Compared with prison-based hospices, little is known about the role played by community-based hospices in providing palliative care to people in prison. AIM: To describe the roles Scottish hospices have adopted to support prisons to provide palliative care, and to discuss the international relevance of these findings in addressing the knowledge gap around community hospices supporting people in prison. DESIGN: A qualitative descriptive study using semi-structured t

[Psychiatric disorders in palliative care and at the end of life].

Patients confronted to advanced organic diseases at a palliative stage can present psychological distress that might announce the occurrence of genuine psychiatric disorders. Some frequent and comprehensible symptoms such as sadness, mild agitation, anxiety or more disturbing such as hallucinations, delusions or suicidal ideations must alert the clinician who should not minimize them by attributing them in a reactive way to the consequences of the evolution of physical disease or treatment's side effects. Literature data regarding psychiatric disorders (mainly anxiety disorders, delirium and d

[Palliative care in the light of legal and regulatory requirements in Germany].

In Germany, palliative care has developed rapidly since the establishment of the first palliative care unit in 1983. More improvements in patient-centered care are only possible if legal requirements as well as education of physicians, nurses, and other professionals involved in palliative care are adapted to current needs. This paper provides an overview of legal and regulatory requirements including a critical appreciation of their influence on palliative care in Germany. Only recently has medical education in palliative medicine been implemented as an integral part of medical studies at Ger

Clinical priorities, barriers and solutions in end-of-life cancer care research across Europe. Report from a workshop.

AIM: The PRISMA project is aiming to co-ordinate research priorities, measurement and practice in end-of-life (EOL) care in Europe. As part of PRISMA we undertook a questionnaire survey and a subsequent workshop to (1) identify clinical priorities for EOL care research in Europe and propose a future research agenda and (2) identify barriers to EOL care research, and possibilities and solutions to improve the research. METHODS: Thirty participants selected among the principally medical survey responders from 25 European countries attended. Twenty-six answered a preparatory pre-workshop questio

The palliative effects of octreotide in malignant disease.

Octreotide is a synthetic analogue of somatostatin with a longer half-life than the native peptide. It has been used extensively in a variety of clinical settings for some years. More recently, its uses in malignant disease processes have been studied and it is proving to be an excellent addition to the palliative care pharmacy. We look at the current uses of octreotide for the palliation of malignant disease with particular emphasis on inoperable malignant bowel obstruction. Octreotide may palliate nausea and vomiting in this distressing condition when other therapies fail. Octreotide may als

Transferring patients home to die from intensive care units: A scoping review.

INTRODUCTION: A substantial proportion of patients spend their final days in intensive care units (ICUs), where ICU nurses are tasked with providing end-of-life care in critical situations. When patients express a preference to spend their final days at home, direct discharge from the ICU may be appropriate. However, this process presents significant risks and complexities. OBJECTIVES: The aims of this review were to investigate the prevalence of transfer from ICUs to home and to explore and summarise known facilitators and barriers to this process, as well as its impacts on patients, familie

Case Series: Episodes of Care in an Intensive Care Garden-Does Fresh Air Matter?

BACKGROUND: Access to nature is found to have significant mental and physical health benefits, but there is minimal evidence of the potential impact on patients, staff and relatives from intensive care. AIMS: It is the purpose of this paper to explore a case series of 2 patients from an intensive care in the United Kingdom (UK), utilising a garden space for end-of-life care and rehabilitation practices. STUDY DESIGN: In the first case, care at end-of-life is explored from a relative and staff perspective. The second case explored the impact of utilisation of a garden space for the care of a

Perceptions of Palliative and End-Of-Life Care Capacity Among Frontline Staff and Administrators in Long-Term Care Homes During the COVID-19 Pandemic in Ontario, Canada: A Mixed-Methods Evaluation.

OBJECTIVES: The COVID-19 pandemic has greatly affected the morbidity and mortality of residents in long-term care (LTC) homes. However, not much is known about its impact on staff's perception of their capacity to provide palliative and end-of-life (EOL) care for LTC residents over the course of the pandemic. We investigated changes in self-reported confidence among LTC workers and their experience in providing palliative and EOL care to residents before and during the COVID-19 pandemic. DESIGN: Mixed-methods evaluation using a survey (n = 19) and semistructured interviews (n = 28). SETTING

The holistic management of malignant bowel obstruction in women with advanced ovarian cancer at end of life.

Bowel obstruction is commonly a pre-terminal event in women with advanced ovarian cancer. Management of symptoms will often be the focus rather than surgical intervention. Determining the patient's end-of-life wishes is paramount - because the prognosis for these patients can be short, advanced care planning is key. This case study will explore the management of nausea and vomiting associated with malignant bowel obstruction and demonstrate how a patient's psychological and social wellbeing is as important as managing the physical symptoms. It will discuss how skilled and effective communicati

The Influence of Spirituality in the Care of Patients with Advanced Chronic Illnesses and at the End of Life: An Integrative Review.

Spirituality is a central yet often overlooked component of care, particularly for people facing advanced chronic illness or approaching the end of life. This integrative review examined evidence on how spiritual care influences emotional, existential, and quality of life outcomes, and identified factors that facilitate or hinder its integration into clinical practice. We searched EMBASE, PubMed, Scopus, Web of Science, BvS, Cochrane, and gray literature for peer-reviewed studies published between 2015 and 2025. Inclusion criteria included adults (≥ 18 years) with advanced or terminal illness,

[End-of-life care in nursing homes and the spiritual triad: an essential convergence for holistic care].

During end-of-life care, a delicate convergence takes place between three spiritual dimensions: that of the caregiver, that of the cared-for and that of the loved ones. This subtle process calls for deep, enlightened introspection on the part of the caregiver, enabling him or her to accompany the patient with kindness and authenticity. This spiritual dimension, which transcends technical care, soothes the dying and their loved ones by offering them a space to express their fears and hopes. Inherited from hospital tradition, taking spirituality into account ensures dignity and humanity in this

When is it time to bid farewell to anticoagulation?

Decisions about antithrombotic therapy in cancer patients at the end of life can be challenging. This cohort study examined prescribing behaviour and outcomes relating to antithrombotic therapy in this patient group, finding that deprescribing was uncommon. Greater awareness of this neglected but high-risk aspect of patient care is required along with interventions to enable shared decision-making in this setting. Commentary on: Aldridge et al.
