[Daptomycin--a new antibiotic for the treatment of skin and soft tissue infections].

Daptomycin (Cubicin) is a bactericidal lipopeptide antibiotic that was approved in February 2006 for the European market, including Norway, for the treatment of patients with complicated skin and soft tissue infections caused by Gram-positive cocci. Daptomycin is administered intravenously and eliminated mainly by the kidneys. Dose reduction is therefore necessary in patients with impaired renal function. The drug is active only against Gram-positive cocci. Daptomycin therapy of patients with complicated skin and soft tissue infections caused by Gram-positive cocci, is as effective as standard

Association Between Local Factors and Implant-Related Complications: A Population-Sampled Cross-Sectional Analysis.

AIM: To investigate the role of local factors and the association with peri-implantitis and peri-implant soft-tissue dehiscence (PISTD). MATERIALS AND METHODS: A randomly selected sample of Norwegian patients who underwent implant therapy in 2014 was examined (242 patients; 732 implants). Clinical and radiographic parameters were recorded, including soft-tissue phenotype, mucosal margin mobility, connection type, platform switching, emergence angle, implant location and presence of adjacent teeth. To account for clustering of implants within patients, mixed-effects logistic regression models

Complicated skin, skin structure and soft tissue infections - are we threatened by multi-resistant pathogens?

Tissue infections or skin, skin structure, and deep seated soft tissue infections are general terms for infections of the entire skin layer including the subcutaneous and muscle tissue layers and their respective fascia structures. Infections of the different mediastinal fascias (mediastinitis) and retroperitoneal fascia infections also belong to this category. Due to the variability of their clinical presentation, skin and soft tissue infections can be classified according to different features. The following aspects can be used for classification: - anatomical structures, - pathogens, - nece

Intravenous immunoglobulin in necrotizing fasciitis - A case report and review of recent literature.

INTRODUCTION: Necrotizing fasciitis (NF) is an inflammatory disease of the soft tissue, which causes local tissue destruction and can lead to lethal septic shock. The therapy consists of early surgical treatment of the septic focus and an accompanying broad spectrum antibiotic therapy. Recent literature considers the additional use of immunoglobulin therapy in severe soft skin and tissue infections. PRESENTATION OF CASE: In this report, we describe the case of a 33-year-old male patient treated at a university hospital intensive care unit because of an NF of his left leg. The patient rapidly

Preoperative idoxuridine and radiation for large soft tissue sarcomas: clinical results with five-year follow-up.

BACKGROUND: Local control remains an important issue in the management of large soft tissue sarcomas. Radiation is the main adjuvant to surgery for local therapy of sarcomas, but it requires relatively high doses, hitherto considered prohibitive in areas such as the retroperitoneum. We developed a preoperative treatment approach to large soft tissue sarcomas that would deliver a high total dose of radiation administered in conjunction with the halogenated pyrimidine radiosensitizer idoxuridine (IdUrd). METHODS: Thirty-seven patients with large sarcomas of the head and neck, mediastinum, retro

[Necrotizing fasciitis in the head and neck region-three case reports and a review of the literature].

Necrotizing fasciitis (NF) is a life-threatening, usually bacterial, soft tissue infection that rarely affects the head and neck region. Rapid progression and systemic illness are typical for the disease and complicate diagnosis. Immediate surgical and antimicrobial therapy is required. Three cases of cervicofacial NF are presented in this case report. All patients showed a rapidly progressing, painful swelling and redness in the cervicofacial region. After diagnosing NF based on clinical symptoms, laboratory parameters, and CT, the patients were treated with systemic antibiotics and multiple

A case report: axillary ALK-positive anaplastic large-cell lymphoma mimicking an infectious abscess in an adolescent.

BACKGROUND: Abscess-like presentations are common in surgical practice, but malignant soft-tissue and lymphoid disorders can occasionally imitate infection and delay definitive treatment. Anaplastic large-cell lymphoma (ALCL) is a CD30-positive T-cell lymphoma that may involve skin, lymph nodes, or soft tissue and can show extensive necrosis, inflammatory change, and systemic symptoms. CASE PRESENTATION: A 17-year-old female student presented with a 3-week history of progressive painful erythema and swelling of the left axilla after ineffective intravenous antibiotic treatment elsewhere. On a

"Jelly VAC" for Negative-Pressure Therapy for Complex Wound Closure: A Report of Two Cases.

CASE: A 35-year-old man sustained an open calcaneal fracture with bone loss, and a 57-year-old woman sustained an avulsion of the entirety of the plantar skin. Both patients were treated with multiple debridements and soft-tissue coverage. "Jelly-VAC" (vacuum-assisted closure) therapy was used after each debridement and during the soft-tissue coverage. CONCLUSION: Jelly-VAC therapy is a promising alternative that allows negative-pressure therapy with the use of ultrasound jelly to prevent air leakage into the wound. We propose using this technique in areas where obtaining a seal is difficult,

Effectiveness of oritavancin for management of skin and soft tissue infections in the emergency department: A case series.

Skin and soft tissue infections, such as cellulitis, are commonly diagnosed in the emergency department and these patients are often admitted to the hospital for intravenous antibiotic therapy. Oritavancin is a novel antibiotic approved for the treatment of skin and soft tissue infections that is administered as a one-time infusion. While oritavancin has demonstrated comparable efficacy with multi-dose parenteral antibiotics in clinical trials and has been proposed as an alternative to admission for emergency department patients, there is a paucity of available real world effectiveness data. I

Introital stenosis requiring pelvic resection and soft tissue reconstruction.

BACKGROUND: Introital stenosis from both bony and soft tissue contracture is an unusual clinical problem not well addressed in the literature. CASE: A woman with a history of pelvic irradiation at age 1 for malignancy presented with severe introital stenosis unresponsive to conservative topical and dilatational therapy. She ultimately required staged bony resection of her infantile pelvis and soft tissue reconstruction to reestablish her introital aperture to an adequate and functional size. CONCLUSION: Introital stenosis from childhood requires a different treatment because development of t

Advanced limb salvage reconstruction following complicated myxoid liposarcoma resection using hybrid gastrocnemius flap and sub-flap irrigating vacuum assisted closure therapy: a case report.

Myxoid liposarcoma is a malignant soft-tissue sarcoma with a high risk of local recurrence. Repeated surgery and adjuvant treatment may compromise soft tissues and therefore predispose patients to chronic infection, creating challenges for limb salvage. We report a case of recurrent myxoid liposarcoma complicated by chronic surgical-site infection that was successfully managed using gastrocnemius flap reconstruction combined with sub-flap irrigating vacuum-assisted closure therapy. A 33-year-old man presented with recurrent myxoid liposarcoma of the posterior left knee following multiple previ

Evaluation and Management of Necrotizing Soft Tissue Infections.

Despite advances in antibiotic and surgical management and supportive care for necrotizing soft tissue infections, morbidity and mortality remain substantial. Although there are clinical practice guidelines in place, there still remains much variability in choice and duration of antibiotic therapy, time to initial surgical debridement, and use of adjuvant medical therapies. This article offers an overview of necrotizing soft tissue infections with a focus on current diagnostic and treatment modalities.

[Reconstruction of soft and hard tissue around the tooth in aesthetic areas: a case report].

Gingival recession or root exposure is associated with the loss of soft and hard tissue around the root. Gingival recession not only leads to aesthetic problem, but also affects tooth function, and even causes the loss of tooth. A case was diagnosed as gingival recession of upper incisor and periodontal- endodontic combined lesion. This report included the history of disease, clinical examination, X-ray film, treatment planning, root canal treatment and sequence of periodontal disease treatment (initial therapy, local drug therapy, bone graft and guided tissue regeneration combined with double

The nasolabial flap revisited as an adjunct to floor-of-mouth reconstruction.

Composite free tissue reconstruction for floor-of-mouth defects are thought of as single-stage procedures. However, postoperative wound complications often require additional soft-tissue coverage to salvage the initial reconstruction. Nasolabial flaps interpolated into the oral cavity offer an expedient solution to soft-tissue deficits encountered during complicated floor-of-mouth reconstructions. The records of 39 patients undergoing free tissue reconstruction, from July 1995 to December 1999 at Shands Hospital and the Gainesville VA Medical Center, for floor-of-mouth defects were reviewed. Pentoxifylline effects on acute and late complications after radiotherapy in rabbit.

Soft tissue damages after radiotherapy are an uncommon but serious complication. Late damage after radiation is the principal dose-limiting factor in radiation therapy today and is dependent on vascular pathology as a result of radiation. Pentoxifylline is a methylxanthine derivative that produces dose-related improvement in blood flow, lower blood viscosity, improved erythrocyte flexibility, and increased tissue oxygen levels. An agent that increases blood flow and tissue oxygen content may contribute to enhanced healing of soft tissue pathology. Sixteen adult New Zealand rabbits were separat

Soft Tissue Augmentation in Periodontics and Implantology: Current Concepts, Techniques, and Clinical Outcomes.

Oral soft tissue augmentation plays a crucial role in enhancing esthetics, managing gingival recession, establishing an adequate band of keratinized mucosa, and supporting both periodontal and implant therapy. Traditionally, autogenous grafts such as free gingival grafts (FGGs) and subepithelial connective tissue grafts (SCTG) have been considered the gold standard due to their predictable outcomes and long-term stability. However, their limitations, including donor site morbidity, increased surgical time, and limited tissue availability, have driven the development of alternative biomaterials

Pedicle flap coverage for infected ventricular assist device augmented with dissolving antibiotic beads: Creation of an antibacterial pocket.

Infectious complications following left ventricular assist device implantation can carry significant morbidity and mortality. The main tenet of treatment is source control which entails local wound care, intravenous antimicrobial therapy, surgical debridement, and at times, soft tissue flap coverage. The mode of therapy depends on the severity, etiology, and location of infection as well as the clinical status of the patient. We describe a case of a 46-year-old male who underwent left ventricular assist device placement complicated by pump thrombosis, recurrent infection, and hardware exposure

Effects of different surgical techniques and displacement distances on the soft tissue profile via orthodontic-orthognathic treatment of class II and class III malocclusions.

BACKGROUND: Orthognathic surgery can be carried out using isolated mandibular or maxillary movement and bimaxillary procedures. In cases of moderate skeletal malocclusion, camouflage treatment by premolar extraction is another treatment option. All these surgical procedures can have a different impact on the soft tissue profile. METHODS: The changes in the soft tissue profile of 187 patients (Class II: 53, Class III: 134) were investigated. The treatment approaches were differentiated as follows: Class II: mandible advancement (MnA), bimaxillary surgery (MxS/MnA), upper extraction (UpEX), or
