Postoperative chylothorax is a potentially lethal complication after esophagectomy. A 53-year-old woman underwent subtotal esophagectomy. The thoracic duct was resected due to swollen lymph nodes. Postoperative chylothorax was diagnosed but conservative treatment was ineffective. Lipiodol lymphangiography revealed leakage from a duplicated left-sided thoracic duct. Left-sided video-assisted thoracoscopic ligation of the left-sided thoracic duct was performed. Because anatomical variations in the thoracic duct contribute to refractory chylothorax, lymphangiography is useful in detecting the pos

Complex thoracic vertebral tumours remain a surgical challenge in terms of the surgical approach to ensure a complete en bloc vertebrectomy with healthy margins, along with optimal control of the thoracic structures next to the spine. A combined three-port left thoracoscopic posterior approach, with the patient placed in a prone position with selective double-lumen intubation, can be performed in patients with spinal tumours involving soft tissues, for direct access to the thoracic structures, even with T10-T11 vertebral tumours next to the diaphragm. The video thoracoscopic technique with an

STUDY DESIGN: A Systematic Review of published series of patients with adolescent idiopathic scoliosis treated with anterior thoracoscopic instrumentation.
OBJECTIVE: To conduct a systematic review of the results of thoracoscopic surgery and to compare them with those of open anterior and posterior spine instrumentation to enable surgeons judge the applicability of the method.
SUMMARY OF BACKGROUND DATA: Instrumentation through video-assisted thoracoscopic surgery is an attractive alternative for the treatment of thoracic adolescent idiopathic scoliosis. The advantages claimed by its propone

Surgical video thoracoscopy represents a new surgical approach to thoracic disease. Its objective is to limit thoracotomy trauma to the pleural wall and at the same time to eliminate the consequences of post-thoracotomy pain and post-operative respiratory dysfunction. There are certain indications which are already accepted as the gold standard, others still require validation and the inverse that certain interventions will probably be excluded from the domain of video thoracoscopy with acquired experience. The best indications are: the treatment of spontaneous pneumothorax in a young person,

OBJECTIVE: Solitary fibrous tumours of the pleura (SFTP) are rare and can histologically be differentiated into benign and malignant forms. The aim of this study is to present new cases, and discuss up-to-date preoperative examinations, the role of video-assisted thoracic surgery and long-term outcome.
METHODS: Between 1993 and 2006, 27 SFTPs were diagnosed (14 females, mean age+/-SD, 62.3+/-9.6 years) at our institution. Medical records were reviewed, and follow-up was obtained by repeated examinations or contact with general practitioners.
RESULTS: SFTPs were associated with symptoms in 63

Thoracoscopic sympathectomy has been used for the treatment of sympathetic dysfunction since it was first described in the 1940s. With the advent of video-assisted thoracic surgery, the procedure has become more widely applied. Video-assisted thoracic surgery allows excellent visual acuity as well as the possibility of doing the procedure more quickly and with fewer complications. This article presents a review of existing approaches and techniques as well as the author's summary and preferences. The review of the articles on thoracoscopic sympathectomy only included those from the English lit

Intralobar pulmonary sequestration (IPS) is a rare developmental abnormality of the lower respiratory system. Lobectomy by video-assisted thoracic surgery using a three-port or four-port technique is the generally accepted operative approach. However, lobectomy can also be accomplished with a single incision even if the IPS is large. This report describes the first case of a patient with a large IPS undergoing uniportal video-assisted thoracic lobectomy in a semiprone position.

BACKGROUND: Over the last decades, several techniques have been implemented to reduce the invasiveness of thoracic surgery. Omitting chest tubes can lead to less postoperative pain and a shorter length of hospital stay. This study examines the extent to which German surgeons use the tubeless technique and what experience they have had with it.
MATERIALS AND METHODS: We conducted a nationwide survey, supported by the German Society of Thoracic Surgery (DGT). A digital questionnaire was sent to all leading thoracic surgeons with DGT membership between July and September 2020.
RESULTS: 63 of 16

OBJECTIVE: We present the combination of spontaneous ventilation and double-lumen tube intubation in thoracic surgery.
METHODS: At the beginning of the procedures, the patients with a body mass index of ≤ 30 were relaxed for a short time, and a double-lumen tube was inserted. After the utility incision or thoracotomy, the vagus nerve was blocked (in right side in the upper mediastinum; in left side in the aorto-pulmonary window) with 3-5 ml of 0.5% bupivacaine. The patients had a bispectral index of 40-60. After the short relaxation period, the patients were ventilating spontaneously without

OBJECTIVE: To compare the results of conventional mini axillary thoracotomy with video-assisted thoracic surgery in the treatment of spontaneous pneumothorax.
METHODS: The retrospective study was conducted at Izmit Seka State Hospital, Kocaeli, Turkey, and Canakkale Onsekiz Mart University Teaching Hospital, Çanakkale, Turkey, and comprised data from November 2011 to May 2019 of patients who underwent surgery for spontaneous pneumothorax either with video-assisted thoracic surgery, who were placed in Group A, or axillary thoracotomy, who were placed in Group B. Data gathered related to age, g

We present a case in which video-assisted thoracic resection for intralobar pulmonary sequestration (ILPS) was successfully performed. A 36-year-old woman had repeated pneumonia. Chest computed tomography (CT) showed a round mass in the right lower lobe of the lung. Subsequent three-dimensional CT revealed that a large anomalous artery arising from the descending thoracic aorta was distributing to the posterior basal segment containing the lesion and was draining into the inferior pulmonary vein. The patient was diagnosed with ILPS and underwent surgery. The anomalous artery was divided, and t

By the 1980s, endoscopy was in use by some groups in sympathetic denervation of the upper limbs with vascular indications. Low morbidity, cosmetic results, reduction in the incidence of Horner syndrome, and the shortened time in hospital made video-assisted thoracic sympathectomy (VATS) better accepted by those undergoing treatment for hyperhidrosis. Over the last 25 years, this surgical procedure has become routine in the treatment of hyperhidrosis, leading to a significant increase in the number of papers on the subject in the literature.

Current videoendoscopic technology and percutaneous techniques of exposure and dissection have been successfully applied to abdominal surgery with favorable results. Application of this technology to our practice of thoracic surgery is the basis of this report. Video-assisted thoracic surgery was performed in 36 patients for the following indications: Raynaud's syndrome, undefined pulmonary nodule, persisting spontaneous pneumothorax, T1 bronchial carcinoma, and mediastinal cyst. Videoendoscopic surgical procedures were accomplished using double-lumen endotracheal anaesthesia and a percutaneou

INTRODUCTION: Iatrogenic injuries to the trachea and main bronchi present one of the most dramatic complications traditionally treated by thoracotomy and transcervical-transtracheal approaches but almost never by video-assisted thoracic surgery.
AIM: To evaluate our experience in a video-assisted thoracic surgery repair of iatrogenic tracheal lacerations.
MATERIAL AND METHODS: The group under analysis consisted of 5 consecutive patients (1 male, mean age: 52 years, range: 32-56 years) who were treated for postintubation and intraoperative damage to the tracheobronchial tree using video-assis
