We report a case of cutaneous cryptococcosis due to Cryptococcus neoformans in a pediatric patient with hyper IgM syndrome with scalp lesions that resembled tinea capitis on gross examination and mimicked juvenile xanthogranuloma on histologic examination. This case highlights the importance of considering cutaneous cryptococcosis in patients with hyper IgM syndrome.

Tinea infections are among the most common dermatologic conditions throughout the world. To avoid a misdiagnosis, identification of dermatophyte infections requires both a fungal culture on Sabouraud's agar media, and a light microscopic mycologic examination from skin scrapings. Topical antifungals may be sufficient for treatment of tinea corporis and cruris and tinea nigra, and the shaving of hair infected by piedra may also be beneficial. Systemic therapy, however, may be required when the infected areas are large, macerated with a secondary infection, or in immunocompromised individuals. P

Presented herein are two cases of dermatomycosis that occurred in Indiana. The causative agent in each case was Trichophyton verrucosum. The first patient had onychomycosis, tinea pedis, and tinea cruris. Although of long duration, his condition cleared readily after the administration of griseofulvin. The second patient, a recent immigrant from Yugoslavia, had erythematous, minimally scaling lesions was more typical, since T. verrucosum is commonly found on exposed parts of the skin. In this patient, also, lesions cleared after the administration of griseofulvin therapy.

Scaly scalp is a common problem in the pediatric and adult population. The possible causes range from the commonly seen tinea capitis, seborrheic dermatitis and psoriasis to rare diseases such as lupus erythematosus and lichen planus. In all cases a thorough history and physical examination are important first steps to successful diagnosis and treatment.

In an ongoing study of Lamisil in children, 21 patients have so far been enrolled and preliminary data are reported in this paper. Eighteen have tinea capitis and 1 has tinea corporis. So far, Trichophyton tonsurans has been identified in 17 and Trichophyton violaceum in 2. Lamisil has so far proved to be well tolerated, efficacious and cost-effective in a 2-week course in those children with a Trichophyton species.
